Vyntus PFT

JBSA Lackland-Wilford Hall Ambulatory Surgical Center

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

This is a request for quote (RFQ) under solicitation number FA3106-21-U-0265. The North American Industry Classification System (NAICS) Code for this requirement is: NAICS Code 339112- Surgical and Medical Instrument Manufacturing with a size standard of 1000. This announcement constitutes the only solicitation; a written solicitation will not be issued. This document incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC), Number 2021-07, and effective 10 SEP 2021. This acquisition shall be solicited as 100% small business set-aside. *It is highly encouraged that offerors in or near the San Antonio, TX area or experienced in providing this service to the San Antonio area respond to this combined synopsis-solicitation.

1. Submission of offers.

Offeror must complete the pricing schedule and/or provide their own price list as part of this request for quote (RFQ). Additionally, offeror must provide their method(s) or process for delivery, installation, and training in a way that fulfills the government’s needs within the Salient /Essential Characteristics.

Offeror must be actively registered in System for Award Management (www.sam.gov) when providing quotes in response to this RFQ and provide all information related to the business entity providing a quote (to include DUNS/Cage). *If offerors are from out of state, they must provide their plan to provide service to the delivery location provided in this RFQ.

ADDENDUM TO FAR 52.212-2 Evaluation Commercial Items (Oct 2014)

2. The Government Basis of Award. At the close of the request for quote, the government shall review the completed price schedule, technical certification information, vendor information (DUNS/Cage), and active registration in System for Award Management for completeness. The offeror’s quote shall include the requested brand name items in the salient characteristics and bid schedule. An award shall be made to a capable, responsive offeror who meets all requirements within the RFQ and above mentioned salient characteristics that is Lowest Price Technically Acceptable (LPTA)

3. Quote Due Date: Quotes must include unit and extended prices for all line items and estimated delivery date. All quotes are due no later than 4:30 PM Central Daylight Time (CDT), 27 September 2021.

Contractor shall include completed price sheet (attachment 3) and any corresponding documents, all items must be e-mailed to joshua.lugan@us.af.mil and Mr. David A. Delgado at david.a.delgado.6@gmail.com

The attached provisions and clauses are applicable to this RFQ and can be viewed through Internet access at the Federal Acquisition Regulation (FAR) site, https://www.acquisition.gov/ . **Below clauses are provided based on initial need of the government and subject to change at time of award.
