REQUEST FOR QUOTE # FA301618U0210

MTS Bionix Electromechanical (EM) Torsion Test System

Solicitation Number FA301618U0210 is issued as a Request for Quote (RFQ) IAW FAR Subpart 12.6 and in conjunction with FAR Subpart 13.3 as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC), No. 2005-99, Effective 16 July 2018 and Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice 20180629 (Effective 29 June 2018) Edition.

This requirement is being solicited on FedBizOpps as brand name or equal and as unrestricted. The North American Industry Classification System (NAICS) Code is 339114, Dental Equipment and Supplies Manufacturing which has a Small Business Size Standard of 750 employees.

The Government intends to award one Firm-Fixed Price (FFP) purchase order for brand name or equal Bionix Electromechanical (EM) Torsion Test System, Manufacturer: MTS Systems Corporation, P/N:

MTS Bionix EM in support of USAF Dental Evaluation & Consultation Services (DECS), Wilford Hall Ambulatory Surgical Center (WHASC), JBSA Lackland, TX 78236.

Special Notes and Instructions:

1. Basis for Award. 502 CONS/JBKBA will issue a purchase order to the vendor who is determined to offer the best value to the Government. The Government reserves the right to award this effort based on the initial quote, as received.

2. Best Value determination. Best value is offeror who is deemed responsible and responsive, lowest evaluated price, and technically acceptable. To be considered technically acceptable vendors shall provide the BRAND NAME OR EQUAL item requested, a quotation must be able to make an offer on all line items, and provide all required information. If the quotation received does not provide pricing for all, then it will be determined technically unacceptable and will be not considered for award. All quotations received will be ranked in the order from the lowest to highest evaluated price and technically acceptable. Award will be made to the responsible offeror whose quotation is the lowest evaluated price among technically acceptable quotes.

3. Mandatory Registrations. To be eligible for award, registration with the System for Award Management (SAM) must be current (legacy for On-line Registration and Certifications Application (ORCA) and Central Contractor Registration (CCR) registrations). To register or update former CCR and

Reps & Certs that are not current go to https://www.sam.gov/portal/public/SAM/ and provide mandatory information. The offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications—Commercial Items (Nov 2017) -- Alternate I (Oct 2014).

5. Notice to Offeror. The Government reserves the right to cancel this purchase request, either before or after suspense date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. The contractor shall not request, nor will the Government make, payments for any preliminary measurement, quotes, bids, site visit, time, or proposal cost incurred.

52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR

EXECUTIVE ORDERS--COMMERCIAL ITEMS (DEVIATION 2013-O0019) (NOV2017)

(a) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.

Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(b)

(1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

(a) In accordance with sections 8124 and 8125 of Division A of the Consolidated Appropriations Act, 2012,(Pub. L. 112-74) none of the funds made available by that Act may be used to enter into a contract with any corporation that—

(1) Has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless the agency has considered suspension or debarment of the corporation and made a determination that this further action is not necessary to protect the interests of the Government.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (MAY 2013)

(a) Definitions. As used in this clause— “Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF.

Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:

(1) Document type. The Contractor shall use the following document type(s).

(2) Inspection/acceptance location. The Contractor shall select the following inspection/acceptance location(s) in WAWF, as specified by the contracting officer.

(4) Payment request and supporting documentation. The Contractor shall ensure a payment request includes appropriate contract line item and subline item descriptions of the work performed or supplies delivered, unit price/cost per unit, fee (if applicable), and all relevant back-up documentation, as defined in DFARS Appendix F, (e.g. timesheets) in support of each payment request.

(5) WAWF email notifications. The Contractor shall enter the e-mail address identified below in the “Send Additional Email Notifications” field of WAWF once a document is submitted in the system.

5352.201-9101 OMBUDSMAN (JUN 2016)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, [Insert names, addresses, telephone numbers, facsimile numbers, and e-mail addresses of Center/MAJCOM/DRU/AFRCO/SMC ombudsman/ombudsmen]. (d) The ombudsman has no authority to render a decision that binds the agency.

5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the http://www.whitehouse.gov/omb/circulars_a076_a76_incl_tech_correction/ http://www.whitehouse.gov/omb/circulars_a076_a76_incl_tech_correction/ contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with any additional requirements to comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment. (End of clause)

MTS Bionix Electromechanical (EM) Torsion Test System

1. DESCRIPTION OF SERVICES / GENERAL INFORMATION. The contractor shall provide all management, transportation, tools, supplies, equipment, and labor necessary to provide, install, and program a brand name MTS Bionix EM Torsion Test System for Air Force Dental Evaluation and Consultation Service (AFDE&CS), JBSA Fort Sam Houston, TX 78234. The MTS Bionix EM Torsion Test System is to be installed at the AFDE&CS as described in paragraph 1.1 of this SOW. This is for a turnkey project to include all cabling, connectors, hardware, and installation. All equipment shall be delivered at time of install. No drop shipping is permitted.

1.1.1.2. MTS Bionix EM Torsion Test System. The contractor shall configure the MTS Bionix EM Torsion Test System to accurately apply monotonic and multicycle torsion, in conjunction with an adjustable static axial load, to test orthopaedic bone screws, constructs and tools, and medical device components such as tubing, catheters, torsion springs, and lead wires to provide a turn-key precision mechanical test system in accordance with commercial practices.

1.1.1.2.1. The MTS Bionix EM Torsion Test System shall be integrated with a compact electromechanical load frame, MTS digital controls and powerful TestSuite TWTM application software to provide safe, easy and reliable testing of components and tools subject to torque loading in biomedical service environments.

1.1.1.2.2. The MTS Bionix EM Torsion Test System shall be compatible with biomedical applications, a versatile system that can also be used to test electronics, general materials and fasteners.

1.1.1.2.3. The MTS Bionix EM Torsion Test System specifications shall include rated torque: +/- 50 N-m; test space: 530 mm (21 inches); maximum specimen diameter: 200 mm (8 inches); workstation footprint: 1175 mm x 460 mm (46 inches x 18 inches); maximum rated speed: 100 rpm at 120 VAC;

integrated test area enclosure; integral axial preload assembly with 2 kg deadweight included; integrated preload area enclosure; brushless servomotor; advanced sine drive amplifier; angular resolution 7.9 arc sec; and CE Compliant.

1.1.1.2.4. The MTS Bionix EM Torsion Test System shall provide for torque measurement in positive or negative rotation, TEDS enabled. Rated Torque: +/- 50 N-m.

1.1.1.2.5. The MTS Bionix EM Torsion Test System shall provide for system load train alignment when utilizing 50 N-m, 20 N-m, or 10 N-m torque cell.

1.1.1.2.6. The MTS Bionix EM Torsion Test System shall provide active measurement for readout and data acquisition of applied static deadweight. The system shall include TEDS enabled 250 N (50 lbf) self-ID axial load cell, factory set full range transducer sensitivity, electronic conditioner module, force transducer cable assembly, and mounting hardware.

1.1.1.2.7. The MTS Bionix EM Torsion Test System shall provide active measurement for readout and data acquisition of axial motion carriage assembly. The system shall include digital encoder, electronic conditioner module, displacement transducer cable assembly, and factory set full range gain.

1.1.1.2.8. The MTS Bionix EM Torsion Test System shall provide handset with holder to include multi-functional handset with LCD display and cable and on-frame holder.

1.1.1.2.9. The MTS Bionix EM Torsion Test System shall provide deadweight 5 kg (11 lbs).

1.1.1.2.10. The MTS Bionix EM Torsion Test System shall provide MTS computer-WIN7, 64 bit, 8GB RAM, 2x500GB hard drive, desktop (P/N 100325044). The MTS system PC shall provide real-time test control system, configured and tested, to assure seamless integration with test system hardware and software. The MTS system PC, no substitute, shall be used solely for test system-related activities, to maximize available processing power and ensure long-term software stability for years of reliable Operation. The MTS system PC shall include Microsoft Excel, computer power cord, North America 100-120 vac, NEMA 5-15.

1.1.1.2.11. The MTS Bionix EM Torsion Test System shall provide MTS supplied monitor – 23 inches

LCD (P/N 100330544).

1.1.1.2.12. The MTS Bionix EM Torsion Test System shall provide MTS TestSuite™ TW Elite Software – EM (P/N 100243332). This software shall provide the ability to design, run, analyze, and report on monotonic and cyclic tests. This software provides easy-to-use interface and powerful test creation capabilities reduce test creation time and streamline test execution. The software shall include pre-defined templates for tension, compression, bend and peel testing, graphical drag-and-drop test flow design, test template creation, data acquisition (timed, p/v, level crossing, cyclic/logarithmic), sine, square, triangle, ramp, hold and custom wave activities, parallel branches for test execution and logical operators (if/then, while), limit sensing, sequencing triggers and interface to digital I/O, data export to ASCII, report generation during-test and post-test, PC configuration: 8GB RAM and Windows 7 64-bit.

This software shall enable creating and editing report templates, Microsoft Excel 2003 or newer to view reports. PC is not included in this line item.

1.1.1.2.13. The MTS Bionix EM Torsion Test System shall provide MTS TestSuite TW Strain 1 & 2 option (P/N 100243336). MTS TestSuite TW Strain 1 & 2 option shall provide the ability to collect data from two hardware channels for extensometers, load cells or high-level signals. MTS TestSuite TW Strain 1 & 2 option shall be compatible with MTS TestSuite TW Elite, TW Essential, or TW Express for test execution. Required software is not included in this line item.

1.1.1.2.14. The MTS Bionix EM Torsion Test System shall provide MTS TestSuite™ Reporter Add-in (P/N 100205364) for easy report design and generation. The MTS TestSuite™ Reporter Add-in shall be compatible for use with Microsoft Excel, enable to organize data and create report with little time investment or manual intervention. The MTS TestSuite™ Reporter Add-in shall enable to generate reports from existing MTS TestSuite test data, crates test reports that require no post-processing, supports text, charts, and calculations. The MTS TestSuite™ Reporter Add-in shall be compatible with MTS TestSuite Elite, MP Express, TW Elite, TW Essential, or TW Express software for test execution and automated report generation, Microsoft 2003 or newer to view reports. The contractor is responsible for initial installation of Microsoft Excel, included in MTS system PC.

1.1.1.2.15. The MTS Bionix EM Torsion Test System shall provide MTS TestSuite™ TW Multi-Cycle Template Package – EM (P/N 100243270). MTS TestSuite™ TW Multi-Cycle Template Package shall provide tensile, compressive and bending tests with user-defined cycles, enables cycling between two endpoints (load, stress, strain, or displacement), allows for holding at endpoint for a specified time, provides ability to adjust the hold endpoint control parameters, and configurable cycle saving frequency. MTS TestSuite™ TW Multi-Cycle Template Package include special report template for reporting multi-cycle results, allow user-configured definition of a cycle and what is being calculated on a per cycle basis, and enable test templates to be run edited in TestSuites TWS. MTS TestSuite™ TW Multi-Cycle Template Package shall be compatible with MTS TestSuite Elite, MP Express, TW Elite, TW Essential, or TW Express software for test execution.

1.1.1.2.16. The contractor shall provide onsite calibration of the MTS Bionix EM Torsion Test System.

The contractor shall provide configuration to include: onsite calibration, LVDT/TEMPO Displacement Transducer; Rotary Position Transducer calibration; and Torque calibration, 0-6800 Nm (0-60K in-lbs).

The contractor shall include calibration standards fee in the quote.

1.1.1.2.17. The contractor shall provide onsite basic operator training of the MTS Bionix EM Torsion Test System.

2.0. OTHER RELATED INFORMATION.

2.1. This is a stand-alone system. The contractor shall work in conjunction with organizational departments to ensure satisfactory implementation of any and all hardware or software system components that will attach to or install on existing network systems or devices. Any requirement for network connectivity or software installation - the contractor shall contact the Government representative 30 days prior to installation date and must be specifically stated in the proposal.

**Prior to contract award, any necessary software or hardware to be installed on the government network that does not have a Certificate of Networthiness (CoN) will need to be scanned by the government. Network connectivity and/or remote access to installed devices from devices outside the facility shall not be permitted.

2.2. The contractor shall provide a binder and electronic copy of all operation manuals, list of all equipment (including manufacturer, model, and serial number), and any other relevant or necessary documentation upon completion of project. Product acceptance shall not be granted until these are delivered and annotated in para. 1.1. of this SOW.

2.3. The contractor shall deliver and install all equipment at time of installation. No equipment shall be drop shipped to the customer. The Government will not provide storage. See room layout drawing in Attachment 1.

2.4. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (Jan 2017), for contractor’s failure to correct nonconforming services.

3.0. ON-SITE RESPONSIBILITIES.

3.1. The contractor shall conduct themselves in a professional, courteous manner, maintaining acceptable appearances and dress. The contractor shall report in at the beginning of each day as well as check out and provide project status at the end of the day.

3.2. The contractor shall be responsible for transportation to and from the jobsite as required.

Building/area access will be provided to the Contractor by the Government representatives listed in para.

4.1.

3.3. The contractor shall participate in any safety or training sessions that may be required to meet building or company regulations for contractors (not to exceed 1 hour of onsite training).

3.4. The contractor shall maintain a clean and safe work environment. All unused materials, containers, tools and equipment shall be removed whenever possible and all trash and debris shall be removed at the end of each day.

3.5. The contractor shall take all precautions to protect all floors, walls, windows and other surfaces from stains, marring or other damage and shall be responsible for any necessary repairs.

3.6. The contractor shall acquire, assemble, deliver, and test all specified equipment and components to provide a fully functional system.

4.0. GENERAL INFORMATION.

4.1 Government Representatives. The Government Representatives are: (Primary) A1C Nathan S.

Dabichan, 210-292-5405 and (Alternate) SSgt. Mychal Sturdivant, 210-292-6398.

4.2. HOURS OF OPERATION. All work shall be performed during duty hours. Work must be done within the hours of 0700 and 1700 Monday through Friday. All federal holidays shall be observed.

4.3. EQUIPMENT DELIVERY. Equipment must be delivered to the contractor’s location at the time of install. No equipment shall be drop shipped to the Government. The Government will not provide any storage.
