STATEMENT OF WORK (Draft)

FOR

Window Tint, Bldg. 6576

SKYLARK COMMUNITY CENTER

1. DESCRIPTION OF SERVICES / GENERAL INFORMATION. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to remove existing window tint on 11 windows and apply new tint to 23 windows and two doors in building #6576, Skylark Community Center.

1.1. DESCRIPTION OF SERVICES. One window equals 8 panels of glass (4 next to 4) approximately 34”W x 16”H each. See picture. The contractor shall remove old window tint on 11 windows and two doors approximately 26.5”H x 76”H each. Apply new window tint to 23 windows and two doors.

The tint will be ceramic or hybrid window tint film, scratch resistant, approximately 40-50% light transmission, black in color on the 23 windows and approximately 15-25% light transmission, black in color on the two doors. No mirror or highly reflective finish.

Warranty: free replacement (labor and materials) within 2 years if film separates from window, bubbles, winkles or fades.

1.2. GENERAL INFORMATION.

1.3. QUALITY CONTROL. The contractor shall be responsible for quality control for all work accomplished during the performance of the task. The contractor shall provide and maintain a Quality Control Plan (QCP) which ensures the requirements in this PWS are met. The QCP will be considered acceptable and approved unless the contractor is notified by the Contracting Officer (CO) prior to award. The QCP shall be included in the technical proposal for evaluation and acceptance. As a minimum, the contractor shall develop quality control procedures that address the areas identified in section 2, Service Delivery Summary.

1.3.1. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance by appointing a Contracting Officer Representative (COR)(s) to monitor performance to ensure services are received. The COR will evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan (QASP) for this requirement.

1.3.2. HOURS OF OPERATION. 7:00am-5:00pm Tues & Wed, 9am-6pm Thurs & Fri, 1130am-7pm Sat & Sun, closed Monday. Contractor will coordinate with building managers to schedule work to minimize impact to events previously scheduled.

1.3.3. HOLIDAYS OBSERVED. Open most federal holidays

1.3.4. SECURITY REQUIREMENTS. Contractor shall provide the names, driver’s license number and state, and date of birth for all employees to gain access to base.

1.3.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. No services determined to be essential for performance during crisis according to DODI 3020.37.

1.3.6. SPECIAL QUALIFICATIONS. N/A

1.3.7. RECORDS MANAGEMENT. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF records management procedures in Air Force Instruction (AFI) 33 - 322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 37-123,paragraph 7.4, Air Force Manual (AFMAN) 37-138, paragraph 2.13, and the Air Force Records Disposition Schedule (AF RDS) located at https://afrims.amc.af.mil/rds_series.cfm or http://www.amc.af.mil/shared/media/document/AFD-060803-003.pdf “

1.3.8. AIR FORCE ANTITERRORISM (AT) PROGRAM

The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. In accordance with AFI 10-245 paragraphs 2.18 and paragraph 2.25, DOD contractors and sub-contracts personnel shall be offered Level 1 – AT Awareness Training. Compliance with the training will be monitored by the contracting officer representatives.

1.3.9. CONTRACTOR FULL-TIME EQUIVALENT REPORTING.

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the [NAMED COMPONENT] via a secure data collection site. The contractor is required to completely fill in all required data fields at https://afcmra.hqda.pentagon.mil/login.aspx.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

Reporting Period: Contractors are required to input data by 31 October of each year.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
