Statement of Work Beds and Mattresses Install

JBSA-Fort Sam Houston

1. GENERAL

1.1. The contractor shall provide all necessary materials, equipment, labor, transportation, delivery, supervision and management to deliver, install, uncrate, and unwrap furnishings items.

1.2. The Contractor shall send email notification NLT two weeks ahead of time of the date and time the trucks will be arriving to the Technical Representative.

1.3. Building configurations are as follows: Building 3312 has 4 floors with stairs and 2 elevators in the building.

1.4. Building 3312 address: 3880 Williams Way, JBSA Fort Sam Houston 78234

2. DELIVER/UNLOAD/INSTALL/REMOVAL IN BUILDING 3312

2.1. The Contractor shall supply, deliver, uncrate, unwrap, and install beds and mattresses in Bldg. 3312. The Contractor will have access to two elevators and stairwells to the building. The stairwells are located inside the building with no restrictions.

3. LOADING/UNLOADING

3.1. The Contractor and Technical Representative shall verify and inspect all items to ensure that no damage has occurred to the beds and mattresses during loading, transport, unloading, staging and set up. Copies of the discrepancy reports shall be forwarded to the Technical Representative, by the end of each day.

4. STAGE/SETUP - EACH BUILDING

4.1. The beds and mattresses shall be staged in a location designated by the Technical Representative at each location.

5. DISPOSAL/ CLEAN-UP

5.1. The Contractor shall remove and dispose of all trash and residual materials connected with this contract in a Government provided dumpster.

5.2. Personal trash (food wrappers, drinks, etc.) shall be removed from the site on a daily basis by the contractor.

6. PROTECTION OF PROPERTY

6.1. The Contractor shall protect all items from damage during both removal and installation. The Contractor shall take precaution against damage to the building, grounds and other furnishings. The Contractor shall perform a final inspection of the building and grounds after completing work. A corrective action plan shall be provided by the Contractor that details type, source and schedule of repairs.

6.2. The Contractor shall repair or replace any items, building components, or grounds damaged due to negligence and/or direct action by the Contractor using materials of the same quality, size, texture, and color to match existing work. All repairs beyond simple surface cleaning require concurrence of the Technical Representative before the Contractor or anyone other than the facility construction contractor (or its subcontractors) may perform any significant repair work. Use of the facility construction contractor (or appropriate subcontractor) is required for all significant repairs so that the building warranty is maintained.

7. HOURS OF OPERATION:

7.1. Performance of work shall be during normal duty hours between 08:00 am - 4:30 pm, Monday through Friday unless otherwise requested in writing and approved by the Contracting Officer.
