STATEMENT OF WORK

PROJECT #MPLS121010AE, REBUILD/REPAIR GARY AVE.

JBSA – LACKLAND

1. PROJECT SCOPE

1.1 BACKGROUND

The purpose of this project is to replace/repair the damaged Gary Ave. North and South End.

North End stretching from Femoyer St. to Hughes Ave. and the South End stretching from B5023

(Wurtsmith/Gary Ave intersection) to Nellis St. (appx 1091Lft) Over the years, the road has deteriorated and failed and is in need of replacement. This road contains bus stops for training personal; contractor shall keep that in mind during construction. Figure 1 below shows the general project area within JBSA-Lackland.

1.2 GENERAL

1.2.1 The purpose also consists of improving the drainage to reduce any localized flooding, see figures below showing the overall and detailed boundaries of the road (figures 2. – 3.2). The contractor shall follow the boundaries or existing seam, if conflicted or unsure please contact the project manager for decision on where the boundary of the new work should start or stop.

1.2.2 Upon completion of the project the contractor shall provide a complete usable product for roads, storm drains, pedestrian traffic, including but not limited to:

a. The road shall be constructed to meet or exceed UFC’s (3-250-01, 3-270-01) and

TxDOT standards (Items 300, 500, 600).

b. Any signs or signage posts damaged while being removed during construction shall be replaced to meet UFC’s, TxDOT and insulation regulation, at contractor’s expense.

1.3 DESIGN

1.3.1 The contractor shall be responsible for developing design analysis, plans, and specifications to accomplish the provisions of this Scope of Work (SOW). The overall

SOW includes design and rebuild/repair of Gary Avenue with in the provided limits (fig.

2- 3.2).

1.3.2 Contractor shall provide own method of insuring the finish project is complete and usable surface of the road.

1.3.3 The contractor shall be responsible for developing and providing design analysis, plans, and specifications to accomplish the provisions of this SOW.

1.3.4 The overall scope of work (SOW) includes demolition plan for asphalt road along with curbs (as needed), gutters (as needed), and utilities (if needed); design plans and construction/installation of new asphalt road with new curbs (as needed), gutters (as needed), and utilities (if needed).

1.3.5 Contractor shall design the street so that needed areas of Gary Ave. will be a full design rebuild and areas that do not have curbing shall have concrete edging (flushed curb). The rest of Gary Ave. shall be mill and overlay with a smooth transition to any adjoining areas.

1.3.6 Contractor shall use a certified/licensed surveyor, to conduct survey/elevations for use in design.

1.3.7 Contractor shall design for road for Bus and Tractor-Trailer Traffic and shall determine the method of repair to remove the washboard effect and the soft spots in the road.

Contractor shall provide the new elevations for the curb, gutter, drain boxes and road.

1.3.8 Contractor shall maintain the road crown with a 2% slope and no ponding to ensure complete drainage from the road.

1.3.9 Contractor shall use TxDOT Type B Asphalt as the road’s binder course and TxDOT

Type D Asphalt as the road-wearing course, asphalt shall be 2inches to 6inches based on the type of repair.

1.3.10 Contractor shall correct elevations based on the design of the storm drain boxes in this project and shall make sure water drains off the road and into proper drainage areas.

1.3.11 Contractor shall insure/design all crosswalks surfaces comply with Americans with

Disabilities Act (ADA) standards. Contractor shall use permanent truncated domes. See appendix C for ADA standards.

1.3.12 Contractor shall design the street such that it includes the radius of the adjoining streets, and or driveways. Contractor shall design curb cuts to help water drain in other locations, as needed.

1.3.13 Contractor shall design and submit a road striping plan for this project.

1.3.14 Contractor shall use construction-phasing plan to accommodate high active traffic.

Contractor shall submit a Traffic/Phasing Plan for the project. Contractor shall make sure that the street always remains open, Gary Ave. is 2 lanes and should always have 1 lane open at all times.

1.3.15 The Designer of Record (DOR) shall use the Unified Facilities Criteria (UFC) and the requirements contained in this SOW to fully develop the technical specifications and construction drawings.

1.3.16 Contractor shall create and submit road/lane closure plan in the format of a power point presentation. Power point presentation shall include signage, time frame, dates, position of cones, direction of traffic, flaggers, stop lights, site layout (map), barrier location, spacing of cones, barriers, and signs; contractor shall provide TXDOT number for each sign used.

The Lane Closure Request takes a minimum of 8 weeks for review; lane closure request must be approved before start of work. Project Manager will notify building managers of closed road/lane. See section 3 for more information.

1.3.17 Contractor shall submit with all Design packages the following items:

a. Road grades, details, plans, drawings and calculations, including cut and fill, both plan views and profile views. Contractor shall provide specifications for complete list of tests required by Designer of Record (DOR).

b. Crosswalk details, plans and drawings. Contractor shall provide specifications for complete list of tests required by DOR.

c. Contractor shall submit submittals for phases work to include, work plan (schedule)

Striping plan, traffic control plan, laydown yard plan, joint plan, materials list, design schedule, construction schedule, cleaning plan, phasing plan, environmental protection plan, site plan (temporary fencing), list of testing requirements, Storm

Water Pollution Prevention Plan (SWPPP), site plan (see section 2.1.1), outage plan and outage request, dig permit, utility outage schedule, and request to use nuclear density gage (see section 15).

1.3.18 The contractor shall submit for government review and approval, bound paper copies and electronic copies of 50%, 75% design package. Contractor shall submit for government review and approval 100% For Construction Design Package signed (stamped) by a Texas

State licensed Professional Engineer (P.E.), consisting of detailed design analyses, plans, materials list, calculations, and specifications for the accomplishment of all construction work described in section 1.4 below. Mobilization and construction effort will only be allowed to begin upon the receipt of signed and sealed For Construction design package, all submittals are approved, road/lane closure and the traffic plan is signed by the 502 Air

Base Wing. All electronic copies shall be provided to the government in the native file format and PDF format. See Appendix B for Design Guide and Appendix I for As-Build

Standard.

1.4 CONSTRUCTION

All construction work shall be in strict conformance with the “For Construction design package”, specifications, plans, and drawings, as well as shall include the furnishing of all materials, labor, tools, equipment, and services necessary to accomplish the provisions of this SOW. The work performed by the contractor shall result in complete and fully functioning systems consisting of, the minimum following principal items:

a. Road shall provide a smooth ride (no washboard effect).

b. Replacement of any curbs that are missing, broken, or are out of alignment.

c. Road shall have no soft spots and no raveling.

d. All striping shall be compliant to base regulations.

e. Any curb inlets, drainage boxes, and drainpipes that are in need of significant repair (as determined by DOR and CEPM) shall be replaced.

f. All drainpipes, boxes, and curb inlets that are not replaced shall be free of debris, free of cracks, and proper size of drainage.

g. Contractor shall restripe the road and crosswalks in accordance with TXDOT regulations.

h. Contractor shall raise (example with a sleeve) any utility access covers or man holes, as needed.

i. Contractor shall repair/replace to better condition, anything damaged by the contractor, at no cost to the government.

j. Any blue reflectors (pertaining to fire hydrants) shall be installed/replaced to UFC/Base regulation. Contractor shall not mobilize until all submittals are approved, road/lane closure and traffic plan are signed by 502 Air Base Wing and the Contracting Officer permits in writing.

Upon the receipt of Notice to Proceed (NTP), the contractor shall coordinate the mobilization plan with 502 CES Project Manager (PM) to ensure smooth startup. The contractor shall make all effort to minimize any adverse effect to the normal training of the mission beyond the boundaries of the project area.

1.4.1 Demolition

1. Contractor shall mill off 3 inches at North End and the South end, see figures 2 through 2.2 and 3 through 3.2 for more description.

2. Contractor shall demolish all curbing and gutters that are out of alignment by more than ½ inch/damaged or are broken, as needed.

3. Contractor shall demolish any curb inlets if out of alignment by more than ½ inch, has fallen, or if there is significant damage it needs to be demo and replaced. If contractor shall demo the curb inlet, demo the whole joint (section) of piping in both directions.

4. Contractor shall maintain a safe, clean, and dust free environment to prevent any adverse effect to the normal operations for that area or any other adjacent areas.

Contactor can leave milling machine in place only if enclosed by safety fencing.

5. Contractor shall protect government equipment and property from damage during construction. Contractor is responsible for properly documenting the condition of the property prior to the start of construction. Contractor shall be responsible for repairing any damages to government property due to contractor’s negligence, at its own cost.

6. All demolished materials at the conclusion of the project will become the property of the contractor and shall be removed off-site and disposed in accordance with all applicable local, state, and federal laws.

1.4.2 New Work/Construction

1. Contractor shall use a certified/licensed surveyor, string line, or GPS, etc. to conduct a survey/elevations for use in construction.

2. Contractor shall follow “For Construction design package” when repairing/replacing

Rev: 07AUG2017 Page 9 of 21 curbs, gutters, curb inlets, road, sidewalks, utilities, crosswalks, driveways, and anything else included in this contract to include all approved submittals.

3. This is a moderate to high traffic area, which contains the Security Forces training facility. Contractor shall install orange plastic safety fencing per approved site plan and storm water protection per approved storm water plan. Keep area clean and free of trip hazards at all times. Contractor shall maintain safety fence properly by insuring it remains upright, not sagging, and free of debris. Safety fencing shall be replace immediately if it becomes damaged or broken. Contractor shall clean the fence line and storm water protection daily. Contractor shall put up appropriate signs indicating the area is a construction zone, sidewalks are closed, hardhats are required, and anything else required by the traffic plan and safety regulations, see section 2.5 temporary project safety fencing.

4. Contractor shall ensure that any penetration introduced to the existing concrete surface shall be either saw cut or core drilled to maintain a neat and clean cut lines.

5. Existing manholes, water valves and curbing shall be adjusted to the finish street elevation such that it is not a trip hazard or possible damage to vehicles, repair or replace if having significant damage. Based on design contractor shall repair damage curb and gutter segments with reinforced concrete. When repairing curbs, cut curbs at joints. Contractor shall maintain same surface finish to the adjoining curbs.

6. Replace non-reinforced curb and gutter with reinforced concrete curb and gutter, when replacing curbs and gutters at that location due to elevation change or damage.

7. Contractor shall notify project manager when hitting an unexpected utility line/pipe. If utility has been uncovered during demolition that is not according to code contractor shall incase the duck bank in concrete. Before pouring concrete, sand and magnetic detection tape shall be placed.

8. Contractor shall use TxDOT Type B Asphalt as the road’s binder course and TxDOT

Type D Asphalt as the road’s wearing course.

9. Contractor shall overlay 3 inches at North End and the South end; see figures 2-2.2 and 3-3.2 for more description. If any soft spots are found contractor shall use Flex

Base course for repair.

10. Contractor shall establish the road crown with a 2% slope and no ponding to ensure complete drainage from the road.

11. Contractor shall notify project manager before pouring concrete, sand and magnetic detection tape shall be placed. If utility has the encasement/duck bank contractor shall leave as is and work around it, ensuring a smooth transition of new asphalt.

12. Curbing to remain at same elevation, unless shown to be broken/fallen more than ½ inch or needing repairs to help keep the street working as planned.

13. Contractor shall use own method (certified/licensed surveyor, string line, or GPS, etc.)

not gutter elevation to ensure road at proper finish grade elevation.

14. Contractor shall comply with all specifications and regulations.

15. Restriping shall be completed to JBSA-Lackland, TxDOT, and UFC regulation as stated

Rev: 07AUG2017 Page 10 of in the restriping plan done during design. Stripping shall be completed with phase of construction, when pavement is completed.

16. Grass shall be restored. Contractor shall place top soil to a minimum of 3 feet from the sidewalk and any areas of disturbed soil. Any areas where grass is disturbed, shall be restored to or better than it was. Contractor shall use 419 Tifway Bermuda Grass by solid SOD or hydroseed/hyrdomultch.

17. Contractor shall work Saturday and Sunday (as needed), Monday, Tuesday, Wednesday, Thursday and Friday. Contractor may work holidays and nights (as needed with approval).

18. Contractor shall protect government equipment and property from damage during construction. Contractor shall be responsible for repairing any damages to government property due to contractor’s negligence, at its own cost.

19. Contractor shall provide their own fill dirt/soil and clean soil shall be used as fill.

502 CES Environmental specifications define the contractor’s responsibility for moving, disposal, testing, transportation, stockpiling, and fees of any soil, recycled asphalt, debris or recycled concrete. Contractor shall dispose of all spoils.

Contractor shall provide all testing, transportation and fees associated with fill dirt, spoils and debris. Contractor shall remove any materials off base or testing of any materials at no cost to the Government. 502 CES Environmental are the ones who determine if soil needs to be tested. Contractor shall fill out proper paper work for removing any soil, asphalt, or concrete off base. Reference Environmental specifications (appendix A) for required information/specifications for certain materials and submittals, along with testing and removal of any materials.

20. Any outages requested by the contractor to perform any provisions of this SOW, shall be submitted to the 502 CES PM at least 14 calendar days prior to the requested Outage Date.

21. Contractor shall maintain access to all building at all times. Maintain access to the gate and parking areas for vehicle traffic at all times including emergency vehicle access.

22. Upon completion of the construction work, the contractor shall restore and clean the project area and return it back to the government ready for immediate use.

1.5 CODES AND REGULATIONS

All work shall be accomplished in accordance with the latest edition of applicable codes, standards, regulations and other requirements. In the case where two or more standards are applicable, the contractor shall ensure compliance with the more stringent of the standards as determined by the 502 CES PM and Contracting Officer (K.O).

2. GENERAL

2.1 GENERAL REQUIRMENTS

2.1.1 Site Plan

The contractor shall prepare a site plan indicating the proposed location and dimensions of any area to be isolated/fenced for use by the contractor, avenues of ingress/egress to the project area, and laydown/storage area. Any areas, which may have to be graveled to prevent the tracking of mud, shall also be identified. The contractor shall also indicate if the use of a supplemental or other staging area is desired. Contractor site plan shall be submitted for approval by the government with the 50% design package. Contractor shall get with the 502nd CE Project Manager for location of laydown yard.

2.1.2 Identification of Employees

The contractor shall be responsible for furnishing to each employee, and for requiring each employee engaged on the work to display, identification as approved and directed by the Contracting Officer. All contractor and sub-contractor personnel shall wear identifying markings on hard hats and safety vests clearly identifying the company for whom the employee works.

2.1.3 Employee Parking

Contractor employees shall park privately owned vehicles in an area designated by the 502 CE Project Manager and Contracting Officer. This area will be within reasonable walking distance of the construction site. Contractor employee parking shall not interfere with existing and established parking requirements of the military installation. Contractor shall get with the 502 CE Project Manager for location and map.

2.1.4 Job Site Tools and Equipment

All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be IAW applicable OSHA regulations. All tools and equipment shall be at hand prior to the contractor commencing work for the day.

2.2 AVAILABILITY AND USE OF UTILITY SERVICES

2.2.1 Payment for Utility Services

All temporary utilities are the responsibility of the Contractor to provide and fund.

In order to hook up to the fire hydrant on JBSA the contractor shall provide a meter (if water is to be paid for by contractor within scope of contract) and a backflow preventer.

The backflow prevention device shall be approved prior to installation by JBSA

Backflow Program Manager via Project Manager (PM)/ Inspector. In order to test a backflow preventer the contractor must have a licensed backflow prevention assembly tester. A copy of their license shall be provided. The test gauge used by the tester must be tested annually for accuracy and a copy of the last test report must be provided. All backflow prevention devices shall be tested upon installation and annually thereafter until removal. A copy of the T&M report shall be provided. Ensure that the meter

(where applicable) and the backflow are the only things connected to the fire hydrant.

Any other hoses, fittings, pipes etc. cannot be connected until after the device has passed inspection. The PM/ Inspector will witness the test and ensure the Fire Dept is notified of the hydrant being used. The backflow device cannot be the open/close (isolation) valve for the water flow. The hydrant nut must be operated with an approved spanner wrench (no pipe wrenches) when water is needed. Hydrants are not to remain in the “on” position. At the completion of the project or when the hydrant is no longer needed, the contractor shall coordinate with PM/ Inspector to ensure Fire Dept. is aware that the hydrant is no longer in use. If the use of another backflow device at another job site or another location on your current job site is required, the process shall be repeated. Every time a new connection is made to the potable system backflow device must be tested.

Hooking up a backflow device to a non-potable source (i.e. sprinkler system) requires the licensed tester to use a dedicated non-potable test gauge and Backflow must be marked “Non Potable Use Only”. Once a backflow device has been used within a non-potable system, it can never be returned to us on a potable water system.

2.2.2 Sanitation

The contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the Contracting Officer. Government toilet facilities will not be available to contractor’s personnel. Contractor shall bring a sanitary station (Port-O-

John) to have on site or at laydown yard. Sanitary station shall be secured such that high winds will not push it over.

2.2.3 Telephone

The contractor shall make all necessary arrangements to ensure they have telephone capabilities for two-way communications between 502nd CE Project Manager, and K.O.

Phone shall not be provided by government.

2.3 CONTRACTOR'S TEMPORARY FACILITIES

2.3.1 Storage Area

The government will provide a laydown yard. The laydown yard shall be returned to the original condition and grass shall be restored prior to final payment. A submittal for the laydown yard is required and shall be included in the 50% design. Submittal shall include plan of the laydown yard, number of facilities inside the laydown yard, and haul route.

Allow about 3 weeks for approval. Contractor may not mobilize until laydown yard and For

Construction design package are approved. The contractor shall construct and maintain temporary 6-foot high chain link fence around the exterior of the project site perimeter, trailers and stored materials. The fence shall include woven privacy fabric, bound edging with brass 1”-grommets fastening locations; factory cut wind-holes; color: dark brown, so that visibility through the fence is obstructed. Fence fabric shall be installed on the “public” side of the site’s construction fence and maintained in a neat and tight fashion onto the fence. Fence fabric damaged during the construction phase shall be replaced at the direction of the Contracting Officer. Fence fabric material color shall be dark brown and submitted to the Contracting Officer for approval. Fence posts may be driven, in lieu of concrete bases, where soil conditions permit. Materials, or equipment shall not be placed or stored outside the fenced area unless such materials, or equipment are assigned a separate and distinct

Rev: 07AUG2017 Page 13 of 21 storage area by the Contracting Officer away from the vicinity of the construction site but within the military boundaries. Trailers, equipment, or materials shall not be open to public view with the exception of those items which are in support of ongoing work on any given day. Materials shall not be stockpiled outside the fence in preparation for the next day's work. Mobile equipment, such as tractors, wheeled lifting equipment, cranes, trucks, and like equipment, shall be parked within the fenced area at the end of each workday.

2.3.2 Maintenance of Storage Area/Laydown Yard

Fencing shall be kept in a state of good repair and proper alignment. Should the contractor elect to traverse with construction equipment or other vehicles, grassed or unpaved areas, which are not established roadways, such areas, shall be covered with a layer of gravel as necessary to prevent rutting and the tracking of mud onto paved or established roadways;

gravel gradation shall be at the contractor's discretion. Grass located within the boundaries of the construction site and the contractor shall mow 5’ outside the boundaries of the construction site for the duration of the project. Grass and vegetation along fences, buildings, under trailers, and in areas not accessible to mowers shall be edged or trimmed neatly. Contractor shall remove debris from the fence line daily.

2.3.3 Security Provisions

Adequate outside security lighting shall be provided at the contractor's laydown yard, stockpile and during night construction. The contractor shall be responsible for the security of its own equipment; in addition, the contractor shall notify appropriate law enforcement agency requesting periodic security checks of the temporary project field office.

2.3.4 Removal of Temporary Fence

The contractor shall remove the temporary fence upon completion and acceptance of the project. In case the fence needs to be removed for the completion of obstructed areas, such as landscaping, the site shall remain in neat appearance with all proper barricades, as specified.

2.4 BULLETIN BOARD, PROJECT SIGN, AND PROJECT SAFETY SIGN

2.4.1 Bulletin Board

Immediately upon beginning of work, the contractor shall provide a binder displaying the

Equal Employment Opportunity poster, a copy of the wage decision contained in the contract, Wage Rate Information poster, and other information approved by the Contracting

Officer. The binder with all information shall be located at the project site in a conspicuous place easily accessible to all employees, as approved by the Contracting Officer. Legible copies of the aforementioned data shall be displayed until work is completed. Upon completion of work the binder shall be removed by and remain the property of the contractor.

2.4.2 Project and Safety Signs

The contractor shall prominently display any safety signs necessary to prevent any accidents to contractor’s employees, government employees, or any other visitors in the project area. The signs shall be posted within 10 working days after receipt of the notice to proceed (NTP) for construction. The signs posted shall help anyone understand they aren’t

Rev: 07AUG2017 Page 12 of 21 allowed in construction area and allowed an alternate route to their location. Upon completion of the project, the signs shall be removed from the site.

2.5 TEMPORARY PROJECT SAFETY FENCING

As soon as practicable, but prior to the commencement of work, the contractor shall furnish and install temporary project safety fencing at the work site. The contractor shall also furnish and install hazard safety fencing at temporary hazards and work site areas considered to be hazardous to the public. Caution tape shall not be used as a means of fencing. Lite orange barriers shall be placed linked together where fencing crosses the hard surface. The safety fencing shall be a high visibility OSHA orange colored, high density polyethylene 1.5” x 3” oval grid or approved equal, a minimum of 48 inches high, supported and tightly secured to steel posts located on maximum 6-foot centers, constructed at the approved location. The temporary safety fencing shall be maintained by the contractor during the periods listed below. Upon completion and acceptance of the work, the fencing shall remain the property of the contractor and shall be removed from the work site.

2.6 CLEANUP

At the end of each working day, contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the aforementioned items could become airborne, they shall be placed in secure refuse containers immediately.

Clean up of the work site shall include the construction areas, material storage areas, parking and eating areas, and any other areas affected by the construction process. Any dirt or mud, tracked on to any street, parking area, sidewalk, patio, driveway, or building shall be cleaned away daily.

Materials resulting from demolition activities that are salvageable shall be stored within the laydown area described above or at the supplemental storage area. All stored material, whether new or salvaged, shall be neatly stacked when stored and protected from weather damage.

2.7 RESTORATION OF GOVERNMENT AREA

Areas used by the contractor for the storage of equipment or material, or other use, shall be restored to the original or better condition prior to opening to the public or returning to the government.

2.8 ENVIRONMENTAL MANAGEMENT

The contractor shall comply with all provisions of JBSA-Lackland Environmental Protection specification as applicable to this contract. Appendix A below includes a copy of the JBSA-

Lackland Environmental Protection specification. No separate payment will be made to the contractor to ensure compliance with the provisions of the environmental specifications; all cost shall be part of the contractor’s base bid.

3. BUILDINGS AFFECTED

Buildings along Gary Ave. at the North end and South end will be impacted by this construction. Ensure that proper driveway and sidewalk egress is established to keep access to all buildings during construction.

3.1. Traffic Plan: The Contractor shall come up with a plan to partially close the road during construction. The road consists of two lanes and during construction, only one lane shall be closed at a time. See section 9 for barriers.

3.2. The contractor shall submit with the 50% design package a traffic plan. The plan shall show dates, times, signage, traffic route, road detours, sidewalk detour placement of barricades, TxDOT number of all signs and barriers, maps showing placement of signs, barriers and direction of traffic, as well a as paragraph description all that will be incorporated into the partial closure. The base will then get the proper approvals for the closure to commence.

3.3. The contractor shall maintain access to all building at all times. Maintain access to all buildings and parking areas for vehicle traffic at all times including emergency vehicle access.

4. EXISTING UTILITIES

Record drawings showing existing underground utilities that are located on Gary Avenue. See

Appendix E for the existing utility map. Any utility line shown on the record drawings (or made known to the Contractor) and damaged during construction work shall be repaired immediately by the contractor at no cost to the government. It shall be the responsibility of the contractor to protect the utility markings made by the government during the issuance of Digging Permit throughout the duration of the contract period. The cost of subsequent request to remark the site shall be $200 per visit and shall be deducted out of the contract amount.

5. SUBMITTALS

The contractor shall update the provided AF Submittal Registry with additional items for government approval to comply with the intent of this SOW, these items shall include all newly installed products and/or reference material that are to remain government property at the conclusion of this project.

Contractor shall utilize the updated Submittal Registry to provide submittals on AF IMT 3000 Material

Approval Submittal Form for all listed items, prior to work or installation of materials. Do not purchase or conduct work until the approved submittal form has been returned from 502 CONS. At minimum, the contractor shall allow 14 days for government review of all submittals for approval or disapproval.

6. PRE-CONSTRUCTION MEETING

The contractor shall request for a preconstruction meeting with the Contracting Officer prior to the commencement of any construction work. The preconstruction meeting shall be attended by at minimum the contractor’s Project Manager, Site Superintendent, Quality Control, and Site Safety and

Health Officer.

7. QUALITY ASSURANCE

7.1. Provide a Quality Control (QC) person at the work site at all times for continuous surveillance of project activities to ensure workmanship, methods, materials, and equipment conform to contract requirements. QC shall have the “stop work” authority to prevent or stop performance inconsistent with the contract requirements.

7.2. The contractor shall submit a Quality Control Plan (QCP) for government approval to the 502

CES PM at least 7 calendar days prior to commencing work. Work cannot proceed without an approved QCP, the minimum QCP shall discuss the following topics:

8. GENERAL SAFETY REQUIREMENTS

8.1. Provide a Site Safety and Health Officer (SSHO) at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement for the

Contractor. The Contractor QC can be SSHO on this project. Contractor shall use and comply with EM385-1-1 Safety Manual dated 2014 also known as the Red book.

8.2. The contractor shall submit a site specific Accident Prevention Plan (APP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work. Work cannot proceed without an approved APP. All changes to the approved APP shall be made with the knowledge and concurrence of the Contracting Officer and 502 CES PM. At minimum, the

APP shall follow the EM385-1-1abbreviated safety plan.

8.3. Obey the posted base speed limits. Wear Seat belts on base IAW AFI 91-207. All vehicles greater than 11,000 lbs. shall enter through Growden Gate and be inspected.

8.4. Report contractor accidents involving injury to personnel or damage to government property greater than $2,000 to the Contracting Officer, 502 CES PM, 502 CES Inspector and the 502

ABW OL- A/SEG, Ground Safety Officer immediately.

8.5. The Contractor shall familiarize and comply with the general safety requirements IAW DOL regulations, OSHA 29 CFR Part 1926, as well as applicable AFOSH standards and AFRs.

These standards and AFRs are available in the 502 ABW OL-A/SEG, Ground Safety Office, building 2484.

9. BARRICADES AND WARNING SIGNS

Adequate barricades, warning signs and flares shall be furnished, installed, and maintained by the contractor. Barricades and signs shall meet the requirements established and in the National Manual on Uniform Traffic Control Devices for Streets and Highways, pertaining to safety and barricades.

Barricades shall have warning lights with low-intensity electric flashers for night application.

10. DIGGING PERMIT

10.1. Obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE

REQUEST (often called a digging permit) from 502 CES PM. Contact 502 CES/CEOI, (210)

671-3533 to schedule a digging permit (AF Form 103) to excavate on JBSA-Lackland.

Contractor shall request a digging permit at least 14 calendar days in advance. Contractor shall be responsible for getting the AF Form 103 filled out. Contractor shall recertify the dig permit every 30 days with a final signature. See appendix D for current Dig Permit.

10.2. The dispatcher will assign a Job Order Number and schedule a time for the base agencies to meet at the job site with the job superintendent. The superintendent must contact Dig Test at 1-

800-344-8377 for Spectrum and SBC and Base Communications Office (210) 925-0607.

Southwestern Bell utilities are only located in housing areas; base communications is located everywhere else. After each agency including Spectrum and Southwestern Bell has signed the

AF Form 103, deliver the form to the 502 CES Inspector for final approval signatures. The

Inspector will return a copy of the completed and approved AF Form 103 to the contractor.

10.3. All excavations other than building foundations, must have site, completed within 14 calendar days (excluding approved rain delay days) after excavation has been initiated.

10.4. It shall be the responsibility of the contractor to document and preserve markings until they are no longer needed. The government will not be responsible for re-marking an area already covered by an existing digging permit.

11. UTILITY OUTAGES

11.1. Utility outages is defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.

11.2. All utility outages shall be coordinated with the Using Agency through the Contracting Officer or the 502 CES PM. Notice of the necessity for an outage shall be provided by the contractor to the Contracting Officer, in writing, not less than 14 calendar days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer.

All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.

11.3. Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.

11.4. In the event a scheduled utility outage is cancelled by the government, notification will be given to the contractor at least 24 hours in advance of the time for the outage to start. Rescheduling of a cancelled outage must be coordinated with written notification as described above.

12. WELDING

Welding shall not be performed without first obtaining a hot work permit issued by the Base Fire

Department. The contractor shall wait four to eight hours before leaving the project site or location of where the welding was taken place.

13. CONTRACTOR'S USE OF ELECTROMAGNETIC EMISSION DEVICES ON BASE

The contractor shall comply with the AFMTC (Air Force Military Training Center) Regulation 700-

14, Jul 89. The regulation is available for inspection at the Communications Squadron, Building

1050, JBSA-Lackland. (Electromagnetic emission devices include: Radio/Radar Transmitters;

Navigational Aids/ Instrumentations; Signaling, Intrusion Detecting, and Identification Devices;

Mobile and Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur

Radio Stations.)

14. RADIOACTIVE EQUIPMENT

Use of radioisotopes or radiation producing equipment (density gauging, NDE weld testing, etc.)

requires a Nuclear Regulatory Commission (NRC) Radioactive Material License, with all documentation submitted and approved prior to bringing the material on base. To obtain authorization, coordinate the request through 502 CES PM.

14.1. The Contractor shall also comply with all requirements of AFI 40-201.

14.2. Contractor shall forward application to the Contracting Officer 30 calendar days prior to commencing operations using radioactive materials.

14.3. Contractors possessing Agreement State Licenses must also submit an NRC Form 241 to NRC in compliance with 10 CFR 150.20. Request shall include the following:

a. Description of proposed activities on NRC Form 241.

b. Procedures established to ensure radiological health and safety of all personnel.

c. Name of responsible contractor representative.

d. Current copy of application for NRC or Agreement State License. The license must specifically state the installation by name or approval for temporary job sites anywhere in the United States where the NRC or Agreement State maintains jurisdiction.

e. The part of the Air Force contract describing work to be done at the base and the inclusive dates of such work.

f. An acknowledgement that the base RPO can make periodic checks to ensure the contractor is following applicable radiological health and safety practices.

g. Contractors requiring more than 180 calendar days per calendar year of operation must possess an NRC license.

15. FIRE REGULATIONS AND PREVENTION PRACTICES

The contractor shall comply with fire prevention practices as set forth by the National Fire Protection

Association, other recognized fire prevention agencies, and base regulations.

16. EXPLOSIVE OPERATED HANDTOOLS

The contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.

17. PREVENTION OF FUEL SPILLS

Contractor may use temporary fuel tanks upon approval by the Contracting Officer. Tanks shall be surrounded by an earthen berm which provides 110% of the tank capacity. The berm shall be lined with a 40 mil polyethylene sheet held in place with weights located on the outside of the berm. All berms shall be inspected by the 502 CES PM or the PI prior to filling fuel tanks.

18. GENERATED REFUSE

The contractor shall take positive action to prevent work-generated refuse from entering the sewer system. All contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation or on-base when indicated.

This refuse and waste material shall be hauled in trucks with tight-fitting beds and covers to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times. All required manifests shall be obtained prior to the transportation of refuse off-site.

19. WATER CONSERVATION

Conserve water to the greatest extent possible.

19.1. If JBSA-Lackland implements water usage restrictions, the contractor is required to comply with these restrictions, at no additional cost to the government. In the event the contractor cannot comply with water usage restrictions and still meets other contract requirements

(particularly when installing sod or hydro-mulching), then he can request permission from the

Contracting Officer for relief from the restrictions.

19.2. Waivers: Submit request for waivers to water conservation restrictions in writing. The

Contracting Officer will consider such cases on an individual basis and grant waivers only in extreme cases. Submit detailed requests with appropriate manufacturer’s/supplier’s data to show unavoidable noncompliance with the water usage restrictions. In the event a restriction is conditionally waived, use only the minimum amount of water necessary to comply with contract requirements.

20. INSPECTION AND ACCEPTANCE

Inspections and final acceptance will be made as completed by the contractor with the following additional provisions:

20.1. The contractor shall give the government a minimum of three (3) working days’ notice for all requested inspections. Inspections shall be staggered such that two inspections will not occur at the same time, unless it is required.

20.2. The contractor shall make sure that the work is ready for all inspections. The presence of an unreasonable number of deficiencies, as determined by the 502 CES PM, may cause the inspection to be rescheduled.

21. INSPECTION OF CORROSION PROTECTION INSTALLATION

21.1. All metallic, above or underground, water or fuel tanks and all metallic underground water, gas, fuel, or electric piping require corrosion protection. The installation shall be coordinated with the Contracting Officer or the 502 CES PM prior to commencement.

21.2. All work shall be coordinated and arranged by the contractor to ensure the corrosion protection installation is inspected by the 502 CES PM in accordance with the following sequence:

a. Initial Inspection: All work on the corrosion protection installation is in the process of being completed and is properly uncovered for inspection by the 502 CES PM or the

Project Inspector.

b. Final Inspection: All work on the corrosion protection installation has been satisfactorily completed, all deficiencies, if any, corrected and is properly uncovered for final inspection by the 502 CES PM.

21.3. Corrosion protection installation shall be completed in accordance with the plans and specifications and certified prior to approval/acceptance by the Contracting Officer.

22. DETERIORATED CONSTRUCTION AND FINISH

The 502 CES PM will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.

23. AS-BUILT DRAWINGS

The contractor shall provide complete detailed as-built drawings representing the entire project as constructed under the contract as well as post-construction elevations. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Contractor shall use a certified/licensed surveyor to conduct a survey for use in the Design, Construction and As-built drawings. Drawings shall be delivered to the government on 24-inches by 36-inches hard copy, standard black line quality paper and electronically on AutoCAD release AutoCAD 2010/2011 drawing

Rev: 07AUG2017 Page 21 of 21 format or compatible format and as PDF on CD/DVD.

Preliminary as-built drawings shall be submitted with an AF Form 3000 for government review/approval. The preliminary as-built drawings shall be submitted concurrently with the request for pre-final inspection. Upon government approval of the preliminary as- built drawings, the contractor shall submit the entire final set of hard copy and electronic as-built drawings for final approval and acceptance. As-built drawings shall be included as a separate item in the progress schedule AF Form 3064. The final set of hard copy and electronic as-built drawings shall be submitted within 10-calendar days of the final acceptance of the project. As-built drawings shall be all inclusive of everything in the construction site of this contract.

24. DD FORM 1354

The contractor shall prepare and submit DD Form 1354, Transfer and Acceptance of Military Real

Property prior to acceptance of the project by the government. The contractor will use the checklist provided in Appendix F, and complete all applicable information to develop the DD Form 1354. A preliminary copy of the form along with the checklist shall be submitted for review by the government to verify completeness and accuracy at least 30 days prior to the pre-final inspection. The completed DD Form 1354 along with the checklist shall be turned over to the 502 CES PM at least 14 calendar days prior to pre-final inspection. A final copy of the DD Form 1354 shall be submitted within 7-calendar days of final acceptance of the project.

25. DELIVERABLES

All of the contract deliverables and supporting documentation becomes the property of the government upon delivery and shall remain property of the government at the end of the contract period.

25.1. CONTRACT DRAWINGS

“For Construction” drawing and specs (whole design 100% package) must be turned in prior to start of construction. “For Construction” drawings and specs are defined as 100% drawing and specs that include any comments from the government review of the 100% design package and are then signed and stamped by a Professional Engineer. Please allow 1 week for government review, government will send “approved” submittal with no comments (for information only submittal). Put the term “100%” on these drawings.

Drawings shall be prepared on AutoCAD release 2015 or compatible format. All contract drawings shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Extensive explanatory notes on the drawings should be held to a minimum or placed in the specifications. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible. Drawings shall be delivered in hard copy, standard black line quality paper, at review stages in the number listed in paragraph on SUBMITTALS. All review drawings shall be 24 inches by 36 inches in size

(SEE Appendix I: CAD As-built requirements).

25.2. DRAWING STANDARDS

Drawings shall be prepared on AutoCAD release AutoCAD 2010/2011 drawing format or

Rev: 07AUG2017 Page 22 of 21 compatible format. All drawings shall utilize A/E/C CAD Standards, Rel 5.0 or latest release and SDSFIE 2.61 for compliance with the installation standards. Both hard copies and electronic copies of the drawings shall be provided to the Contracting Officer for review at the

50%, 75%, and 100% design package.

Upon the acceptance of the 100% design drawings, the contractor shall provide to the

Contracting Officer signed and sealed For Construction Design Package submittal. Extensive explanatory notes on the drawings should be held to a minimum. Dimensions shall be shown in

Standard English units. Standard building material indications and symbols shall be used to the greatest extent possible.

25.3. SCHEDULED OF DELIVARIBLES

Deliverables shall be submitted and distributed as shown in the Schedule of Materials

Submittals. Contractor shall add the rest of the items to the Schedule of Materials Submittal needed to complete this contract.

26. OCCUPATIONAL SAFETY

The contractor shall be subject to no-notice inspections under Occupational Safety and Health Act

(OSHA) Program by inspectors of the Department of Labor.
