STATEMENT OF WORK
FOR
MAINTENANCE OF CADDY ULTRA-VIOLET (UV) VENT HOOD SYSTEMS
1. TITLE: MAINTENANCE OF CADDY UV VENT HOOD SYSTEM IN BUILDING
2. PROJECT LOCATION: Fort Sam Houston, TX
Bldg. 1287, Slagel Dining Facility
3. GENERAL DESCRIPTION OF WORK: Provide all labor, transportation, material, and supervision to provide one-time specialized maintenance to the Caddy UV Vent hood Systems, Model # SHBCU-C-1-
84ND.
4. PERFORMANCE SPECIFICATIONS/DETAILED SCOPE OF WORK:
4.1 Work Plan(s):
4.1.1 All work shall be coordinated through 502nd CEOES, Contracting Officer Representative (COR), Fire Department, and dining facility building manager.
4.1.2 Maintenance includes, calibration, system on/off operational adjustments, and replacement of UV bulbs as necessary. Provide instructional procedures on daily operation of UV Caddy System in support of operator continuity due to frequent turnover of dining facility personnel. Maintenance will also include checking and/or cleaning baffle grease filters, grease extractors, grease cups and baffle cartridges as needed, and verification that equipment is functioning properly as part of this maintenance requirement.
4.1.3 All cleaning supplies shall be provided by contractor. If there are any repairs or parts that are found to be needed during this maintenance requirement. The COR will be made aware of and will have the option to approve or decline the repairs. All repairs and parts will be itemized separately on the invoice. Repairs will be performed after business hours, unless otherwise agreed upon.
4.1.4 The contractor shall inform the COR to coordinate for entry into the building.
5. PHASING REQUIREMENTS AND HOURS OF OPERATION/DAILY WORK HOURS:
5.1 DUTY HOURS: The work in Building 1287 (Slagel dining
facility) shall be accomplished after normal duty hour which are Monday thru Friday – 0800 – 1630.
6. NOTED CONTRACTOR REQUIREMENTS:
6.1 Prior to award:
6.1.1 Contractor shall visit the site prior to submitting a bid and determine all existing conditions which may affect his work and account for this in his bid. The contractor, at no extra cost, shall perform the correction of any conditions resulting in extra work arising after award of contract and start of work.
6.2.1 The Contractor shall provide and maintain safe work
environments and procedures which compiles with OSHA's Safety Standards 29 CFR Part 1926 and 29 CFR Part 1910.
7.1 DEBRIS/CLEAN UP: The contractor shall keep the work area
clear of any debris at all times. All debris generated by this project shall be properly disposed of off Post.
8.1 AFTER WORK COMPLETION, PRIOR TO FINAL PAYMENT:
8.1.1 Record of Contractors Final Inspection.
8.1.2 Warranty Package.
