Monthly and Quarterly Maintenance of Reverse Osmosis Systems

PERFORMANCE WORK STATEMENT

10/4/16

Contractor shall provide preventative maintenance of 5 Millennium, 3 WRO300H Reverse Osmosis (R.O.) Machines + Carbon Tanks supplied by MAR COR PIN 920-03-835 Rev 1, and CWP 100, Model

WRO H/S

1. MONTHLY PM Service on 5 Millennium & 3 WRO300H Reverse Osmosis (R.O) Machines to include:

- Changing R.O. Pre Filter 1 Micron Filter 10" for portable R.O.'s

- Water Testing on product water from each R.O. Machine for Bacteria & LAL Test for Endotoxins. Lab used must be a Clinical Laboratory Improvement Amendments (CLIA) certified lab.

- (5) Millenium R.O. Sanitization with MINNCARE® Disinfectant, and Disinfectant Residual checks with Test Strips following disinfection for Millenium RO’s.

- (3) WRO300H R.O. sanitization with heat standard.

- Testing results and maintenance reports are to be turned in/sent to the NCOIC/OIC of the Dialysis Unit.

2. MONTHLY PM service on 1 CWP water purification unit to include:

System Performance Review which includes:

a. Inspection/Recording of Dialysis System Reading Data

b. Inspection/Recording of Operation Log Book Data

c. Inspection/Recording of Feed Water Chemistry Data

d. Inspection/Recording of Blend Valve Performance Data

e. Inspection/Recording of Booster Pump Performance Data

f. Inspection/Recording of Softener Data

g. Inspection/Recording of Carbon Tanks Data

h. Inspection/Recording of R.O Data

i. Inspection/Recording of Storage Tank Data

j. Inspection/Recording of Control Panel Data

k. Inspection/Recording of Mixed Bed(DI) Tanks Data

l. Inspection/Recording of Remote Alarms Data

m. Inspection/Recording of Distribution Pump Data

n. Record Flows

- Replacing System Pre Filter 5M ,20” BB

- Replacing RO Pre Filter 1M ,20” BB

- Disinfecting Membranes(Large Central) with MINNCARE® Disinfectant and rinsing. Once rinsed, MINNCARE® Regeant Test Strips will be used to verify MINNCARE Disinfectant is clear from system.

- Water Testing on product water from R.O. unit for Bacteria & LAL Test for Endotoxins. Quantity (3).

Lab used must be a Clinical Laboratory Improvement Amendments (CLIA) certified lab.

- Testing results and maintenance reports are to be turned in/sent to the NCOIC/OIC of the Dialysis

Unit.

- Every other month, include delivery on 1 case of Minncare.

3. QUARTERLY PM Service on 5 Millennium & 3 WRO300H Reverse Osmosis (R.O.) Machines to include:

- Calibrate Water Quality Monitor on R.O. Monitors

- AAMI Chemical Water Test on product water from each R.0. Machine

- Exchange Portable Carbon Tanks (10 each 8X35" PE carbon Rental Tanks, 6 each PTG520 Carbon Filters)

- Clean (3) WRO300H RO Membranes using Low and High pH cleaners

- Clean (5) Millenium RO Membranes using Low and High pH cleaners

- Testing results and maintenance reports are to be turned in/sent to the NCOIC/OIC of the Dialysis Unit.

4. QUARTERLY PM service on 1 CWP water purification unit to include:

- Cleaning Membranes(Large Central) using Low and High pH cleaners. Contractor will rinse the system and use correct strips to verify all chemical is clear from system.

- Exchange of two (2) rental DI Tanks, 3.6 cu.ft.

5. SEMI-ANNUAL PM service on 1 CWP water purification unit to include:

- Inspecting/Replacing Pyrogen .2M, 20” Posiclear

- Inspecting/Replacing Bicarb Vent Filter

- AAMI Chemical Water Test on RO product water

6. ANNUAL PM service on 1 CWP water purification unit to include:

- Perform rebed/replacement of all media in (2) Carbon Filter Tanks, replacement of seals and spacer kits, replacement of riser tubes, and replacement of riser o-rings and tank o-rings.

-CWP Year 2 Preventative Maintenance Kit -CWP Extended Coverage Service - The CWP RO Water System extended service coverage agreement includes repair or replacement of certain parts (subject to the terms below), emergency service, labor and travel during normal business hours with respect to the covered CWP RO Water System. The CWP service agreement also includes an annual system review and parameter check, one annual Preventative Maintenance Kit, a semi annual pre-filter replacement, and a semi annual endotoxin filter. It does not include pretreatment equipment or cleaning and disinfection unless otherwise expressly set forth herein.

Contractor devised this service agreement to help service and maintain the CWP. Contractor will replace or repair any items on the CWP that are covered under the warranty of the CWP. Contractor is available for servicing equipment 24/7. The emergency service in this agreement only covers service calls that are completed Monday – Friday 8:00AM to 5:00PM (excluding holidays). Contractor will respond by telephone within 48 hours of the initial call. Emergency service calls that are performed outside of this time period will be charged based on time and materials. The emergency service does not cover the cost of membranes, bacteria issues, and equipment issues that are from lack of care by the facility or acts of nature or are otherwise excluded from Contractor’s warranty. The CWP full service agreement does not include media and media tanks. Contractor will replace or repair any parts which prove defective during the term of this agreement, but Contractor will have no obligation to repair or replace parts which fail due to (i) external causes, including, without limitation, accident, vandalism, natural disaster, acts-of-God, power failure, or electric power surges; (ii) abuse, misuse, or neglect or use of unauthorized third party consumables and accessories; (iii) usage not in accordance with product instructions; or (iv) servicing or repair not authorized by Contractor. This is only a guidance of some of the preventative maintenance that should be performed on the CWP. Customer should follow what is stated in the product manual to maintain compliance.

Consultation with the NCOIC/OIC on any required repairs shall be required on each visit. Contractor shall report all repairs completed, as well as repairs not completed at the completion of the visit.

7. Contractor shall be required to accomplish cleanup of area after completion of work. Equipment may not be left in a disassembled state when leaving the facility at the completion of a visit.

8. Contractor will be prepared with tools and instructions needed for equipment repair.

9. Contractor shall respond to routine service calls within 72 hours after notification by the NCOIC/OIC or the on duty authorized representative.

10. Contractor shall provide service tickets or invoices for all parts needed, and name, serial number, and label number, of machine repaired, to the Dialysis Unit staff at the time of the repair. If service ticket or invoice is not available at the time of repair, part used, cost, if known, and equipment repaired, will be documented in logbook or on invoices, if contractor uses invoices for each job. Service ticket or invoice must be provided to contract POC before payment will be made.

11. Contractor is responsible for all the equipment listed in Attachment 1 from the house supply valve to the back of the Hemodialysis unit to include all hoses and connectors. Buyer is responsible for providing adequate physical access to equipment components for servicing.

12. Contractor shall do nothing to equipment to void the warranty on any equipment. If contractor voids warranty they shall provide maintenance on that equipment until end of original manufacturer's warranty free of charge to the government.
