Monthly and Quarterly Maintenance of Reverse Osmosis Systems

PERFORMANCE WORK STATEMENT

17 September 2012

1. Contractor shall provide preventative maintenance of 8 Millennium Reverse Osmosis (R.O.) Machines + Carbon

Tanks Manufactured by MAR COR P/N 1232429 Rev 1

2. MONTHLY PM Service on 8 Millennium Reverse Osmosis (R.O.) Machines to include:

- Changing R.O. Pre Filter 1 Micron Filter 10” for portable R.O.’s

- Water Testing on product water from each R.O. Machine for Bacteria & LAL Test for Endotoxins. Lab used must be a Clinical Laboratory Improvement Amendments (CLIA) certified lab.

- R.O. Cleaning & Sanitization with MinnClean AC, TF, Test Strips, Disinfectant, Kits, and Disinfectant Residual checks with Test Strips following disinfection

3. QUARTERLY PM Service on 8 Millennium Reverse Osmosis (R.O.) Machines to include:

- Calibrate Water Quality Monitor on R.O. Monitors

- 4 AAMI Chemical Water Test on product water from each R.O. Machine

- Exchange Portable DI & Carbon Tanks (6 each 6”x18” PE carbon Tanks)

4. Consultation with the NCOIC on any required repairs shall be required on each visit. Contractor shall report all repairs completed, as well as repairs not completed at the completion of the visit.

5. Contractor shall be required to accomplish clean up of area after completion of work. Equipment may not be left in a disassembled state when leaving the facility at the completion of a visit.

6. Contractor will be prepared with tools and instructions needed for equipment repair.

7. Contractor shall respond to routine service calls within 72 hours after notification by the NCOIC or the on duty authorized representative.

8. Contractor shall provide receipts for all parts needed, and name, serial number, and label number, of machine repaired, to the Nutritional Medicine staff at the time of the repair. If receipt is not available at the time of repair, part used, cost, if known, and equipment repaired, will be documented in logbook or on invoices, if contractor uses invoices for each job. Receipt must be provided to contract POC before payment will be made. Payment is cost-basis only.

9. Contractor is responsible for all the equipment listed in Attachment 1

10. Contractor shall do nothing to equipment to void the warranty on any equipment. If contractor voids warranty they shall provide maintenance on that equipment until end of original manufacturer’s warranty free of charge to the government.
