STATEMENT OF WORK

For

Replace Walk in Refrigerator/Freezer

Location Building: 1994

WORK ORDER NUMBER: 67182

DATED: 11 June 2018

1. GENERAL INFORMATION:

This project is intended to replace walk in refrigerator/freezer in accordance with all applicable National, Air

Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, Unified Facilities Criteria (UFC) state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. Effective immediately: Due to safety concerns the use of manhole fixed ladders on Hanscom

AFB is prohibited for the purpose of entry and egress is prohibited. An alternative means is authorized (i.e.

appropriate portable ladder/personnel lowering device).

2. SCOPE: Replace Walk in Refrigerator/Freezer

Remove existing refrigerator/freezer from location (see map for location)

Dispose of refrigerator/freezer properly off Hanscom

Replace with similar Model Harford Duracool DR34788SS1M6DP

Contractor will install fire protection in refrigerator/freezer per NFPA

Contractor will also ensure proper wiring size and voltage

Unit to have 2 condensers

Unit to have defrost cycle

Unit to have heated drain tape

Unit to have insulated viewing window

Safety rails are required on top of unit

Digital LED thermometer and switch

Shelves (4) on each side of the freezer/refrigerator

Unit to have slip resistant flooring in freezer/refrigerator

The Contractor shall repair all asphalt around concrete pad to specifications.

The Contractor shall place the new freezer/refrigerator on the concrete

The Contractor shall test the freezer operation

Red Lines are required

3. GENERAL REQUIREMENTS:

Work includes, but is not limited to the following:

a. Provide resolutions of any technical problems that arise during the construction.

b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.

c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.

d. The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974

(OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air

Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.

e. Any modifications and addition on a facility that contains Lightning Protections System (LPS) This includes paint projects, roofing projects, projects requiring roof penetrations, installation of new HVAC or other metallic equipment, antenna installation, or other work. Additions and modifications to the facility envelope require re-evaluation of the LPS for the completed facility as a whole. The LPS required by the addition or modification shall not be a simple addition to the LPS. The LPS must be considered a single entity and must comply with requirements of a single LPS upon completion. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order.

Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final

LPS in the initial design. Projects of this type shall ensure the final LPS as a whole is compliant with AFI

32-1065 and NFPA 780, in that priority order. The same third-party inspector requirements shall apply.

f. Complete the “builder data inventory and assessment sheet” for newly installed assets, ensuring the bar code number is attached to the new equipment. Transfer bar codes from any removed equipment to the builder assessment ½ sheet form

g. Provide three (3) copies of O & M manuals and warranty letter

h. Period of Performance 90 days after award of contract
