1.
Description of Service
The Contractor shall furnish all personnel, equipment, labor, tools, supervision, transportation and other items and services necessary to perform Instructor (Fitness) for the 61st Force Service Support Group (FSSG) Fitness Centers at Los Angeles Air Force Base, El Segundo, California and Fort MacArthur, San Pedro, California.
1.1.
Basic Services
The Contractor shall provide all personnel, equipment, tools, vehicles, supervision, (except as specified as Government-furnished property and services), and other items and services necessary to perform all Fitness Exercise Class tasks and functions as defined in this Statement of Work (SOW).
1.2.
Fitness Exercise Class The purpose of the Fitness Exercise Class is to provide a group exercise program that is intended to help people achieve fitness goals and improve their health. Therefore the Contractor shall:
1.2.1.
Pre- Fitness Exercise Class Promptly notify Fitness Center Staff at the front desk of any broken, improperly functioning equipment, unclean or unsafe conditions encountered before, during, or after any exercise session; instruct class participants not to use such equipment. Brief new class members on safety precautions, effective exercise principles, and proper techniques in order to prevent injuries.
1.2.1.1. Exercise Classes Days and Times
Start each exercise class on the days and times specified in paragraphs 1.3.1.2.1. and 1.3.1.2.2. with no more than 10% of all classes conducted during the month failing to begin on time. In addition, the Contractor shall stage (position) all necessary equipment and supplies that are needed for each class prior to the beginning of each exercise session. Exercise class days and times are subject to change to meet customer demand. The Functional Director (FD) through the Contracting Officer (CO) will direct changes in class schedules by letter with a minimum notice of two weeks. The Government may change or cancel classes with a minimum of two (2) weeks written notice to the Contractor from CO. In the case of an emergency, the Government through the CO may cancel one or more classes by notifying the Contractor Representative (CR) telephonically within two (2) hours or more prior to the class start time.
1.2.1.2. Administration
Ensure class participants sign Government-provided attendance rosters to document attendance statistics. The Contractor shall obtain attendance sheets from the front desk at the Fitness Center and return the completed sheets at the end of each exercise class. In addition the Contractor provide class participants with Government-provided Customer Satisfaction Survey Cards.
1.2.1.2.1.
Exercise Classes at Los Angeles Air Force Base (CLIN 0001) Provide a monthly training calendar to address the list of exercise classes listed in Appendix – A, to Fitness Center staff eight (8) days in advance of month of calendar execution. Execute classes during the following days and start times for one (1) hour:
(Not to exceed 1,144 hours annually)
a. Monday 06:30, 11:30, 17:30, and 18:15
b. Tuesday 06:00, 07:00,11:30, and 16:30
c. Wednesday 06:30, 11:00, 11:45, 16:30, and 17:15
d. Thursday 06:00, 07:00, 11:00, 12:00, and 17:00
e. Friday
06:45, 07:30 and 11:30
f. Saturday 09:30 Maintain a clean and safe environment for exercise classes. Clean all equipment used in the Fitness Exercise Class at the conclusion of each session to remove dirt, body oils, perspiration. In addition all equipment must be dried to prevent rusting of metal surfaces and the deterioration of soft surfaces. All equipment must be returned to the appropriate storage location. Instructors shall require class participants to clean the equipment and return it to storage. Equipment wipes will be provided by the Government and available in each group fitness room. If supplies are not available the instructor shall report the shortage to the front desk.
1.2.1.2.2.
Exercise Classes at Fort MacArthur (CLIN 0002) Provide a monthly training calendar to address the list of exercise classes listed in Appendix – B, to Fitness Center staff eight (8) days in advance of month of calendar execution. Execute classes during the following days and start times for one (1) hour:
(Not to exceed 312 hours annually)
a. Monday 09:30 and 18:30
b. Tuesday 18:30
c. Wednesday 09:30
d. Thursday 09:30 and 18:30

1.2.1.2.3.
Special Group Fitness Events (CLIN 0003) Provide Special Group Fitness Events (not to exceed a total of three (3)) during the time frame of May and June of each year. For planning purposes, the Government will notify the Contractor at least one month in advance before each Special Group Fitness Event. Clean all equipment used in the Fitness Exercise Class at the conclusion of each session to remove dirt, body oils, perspiration. In addition all equipment must be dried to prevent rusting of metal surfaces and the deterioration of soft surfaces. All equipment must be returned to the appropriate storage location. Instructors shall require class participants to clean the equipment and return it to storage. Equipment wipes will be provided by the Government and available in each group fitness room. If supplies are not available the instructor shall report the shortage to the front desk. Note: Special Group Fitness Events will require a minimum of four (4) instructors per event as listed in Appendix – D. (Not to exceed 3 hours annually)
1.2.1.3. Injuries or Other Health Emergencies
Notify immediately the Fitness Center staff of any injuries or accidents that should occur during any Fitness Exercise Class. Fitness Center staff will request medical assistance. If Fitness Center staff is not available, the instructor shall immediately call 911 in El Segundo or 911 at Fort MacArthur from base phones located in either Fitness Center behind the front desk. If using an off base phone (or cell phone) the number to the El Segundo facility is: 310-653-6771 and to the Fort Mac facility: 310-653-8361, respectively.
1.3.
Contractor Manpower Reporting Application (eCMRA) The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via the secure data collection site (i.e. Contract Manpower Reporting Application). The Contractor is required to completely fill in all required data fields using the following web address http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the ECMRA help desk.”

Performance Evaluation Performance of services will evaluated to determine whether or not it meets the performance requirements of this contract. Re-performance of unacceptable services at no additional cost is the preferred course of action when appropriate. In the case of issued Corrective Action Report (CAR), the Contracting Officer (CO) shall take appropriate measures according to the “Inspection of Services” clause.
3.
Base Protocol 3.1.
Facilities
No smoking is allowed in Government facilities. The Contractor shall return the facilities to the Government in the same condition as received. The Government will provide rooms for classes at the Group Fitness Room (Los Angeles AFB Fitness Center) however the group fitness room will be utilized by other patrons for stretching and quiet activities during contracted classes. If the patrons cause a disruption, the instructor shall politely ask the individual(s) to stop, or request a Fitness Center employee to take care of the problem.
3.1.1.
Hazardous Material
Comply with base Hazardous Material (HAZMAT) regulations for all issues of hazardous material, and ensure storage procedures meet all federal, state, and local requirements. Prior to use on Los Angeles AFB, the Contractor shall provide the Base Hazardous Material Pharmacy, through the CO, a list of all hazardous materials the Contractor brings onto Government property and shall provide a Material Safety Data Sheet (MSDS) for each Hazardous Chemical listed in OSHA Hazard Communication Standard 29 CFR 1910.1200 in accordance with LAAFB Hazardous Material Management.
3.2.
Base Training Operations (BTO) The Government will notify the Contractor in writing within three (3) workdays of start of a Base Training Operation (BTO) or other event that could cause unusual traffic congestion hampering instructors’ ability to enter or exit the base. BTO dates will be announced so instructors can plan ahead to ensure classes begin at the published times. Inability to conveniently enter the base or to locate a nearby parking space will not be an acceptable excuse for starting classes after the published time.
3.3.
Parking Los Angeles Air Force Base The Government will notify the Contractor in writing within three (3) workdays of start of a Base Training Operation (BTO) or other event that could cause unusual traffic congestion hampering instructors’ ability to enter or exit the base. BTO dates will be announced so instructors can plan ahead to ensure classes begin at the published times. Inability to conveniently enter the base or to locate a nearby parking space will not be an acceptable excuse for starting classes after the published time.
3.4.
Government-Furnished Services
Government-furnished services include services provided by Government activities as well as contracted services.
3.4.1.
Utilities
The Government shall furnish utility services including water and electric for Contractor use in performing this contract. The Contractor shall ensure employees conserve utilities.
3.4.2.
Communication
3.4.2.1. Telephone
The Contractor shall be allowed the use of the telephone for contacting the CO, Civil Engineer, Fire Department, Security Forces, receiving customer complaints and any other directly contract related communications.
3.4.2.2. Communication Security (COMSEC)
All DoD communications are subject to COMSEC review. Use of DoD telephones and telephone systems constitutes consent to COMSEC monitoring. The contractor shall comply with LAAFB COMSEC procedures and rules. For copies of these documents please contact Jose Torres at 310-653-0027 or 3845.
3.4.3.
Refuse Collection
Waste generated by the Contractor in performance of the services specified with this SOW shall be deposited in Government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. The Contractor shall comply with base recycling regulations.
3.4.4.
Emergency Medical Service
Emergency medical treatment and emergency patient transportation service is only available through the local community.
3.4.5.
Security Forces
The Government will provide general security service. Security Forces phone extensions are 116 for on base emergencies and 911 for off base emergencies.
3.5.
Forms and Publications
The Contractor is responsible for obtaining and maintaining all forms and publications needed to perform the contract.
4.
General Information
4.1.
Contractor Personnel
4.1.1.
Contract Manager
The Contractor shall appoint a Program Manager (PM) to provide daily supervision and quality control, and who shall represent the Contractor and be responsible for the performance of the work throughout the term of the contract. The name of this person, and an alternate(s) who shall act for the Contractor when the manager is absent, shall be designated in writing to the CO and the Heritage Center representative. The Program Manager (PM) shall be able to speak, read, write, and understand English. The PM shall be able to effectively communicate with Contractor employees.
4.1.2.
Contractor Employees
The Government reserves the right to restrict the employment, under this contract, of any Contractor employee, or prospective Contractor employee who is identified as a potential threat to health, safety, security, general well-being, or the operational mission of the installation and its population.
4.1.2.1. Employee Training
The Contractor is responsible for all internal employee training at no additional cost to the Government.
4.1.2.2. Certifications
The Contractor shall ensure that all personnel who are providing services for this contract have all required certifications accordance AFI-34-266 paragraph 2.3. In addition, at a minimum, personnel must have an accreditation or certification from of the organizations listed in Appendix – C. These required accreditations or certifications must be provided to the Government before services begin and as required thereafter.
4.1.2.3. Access to a Department of Defense (DoD) Installation
The Contractor must comply with the Integrated Defense Plan (IDP) for access onto LAAFB and entry into Controlled Areas. If access is required to a Controlled Area the Contractor must be able to possess and maintain a Secret facility clearance at all times during the term of the contract. In the event the Contractor does not possess a Secret facility clearance at time of contract award, the Contractor shall submit a request (within 30 calendar days of performance start date) for an interim facility clearance through the host organization (61 FSS) and the contracting office (SMC/PKE). The requirements for access onto LAAFB and entry into Controlled Areas are listed in Appendix – F.
4.1.2.4. Employment of Government Employees
The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, or the appearance of a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives proper approval. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. Their absence at any time shall not constitute an excuse for nonperformance under this contract. The Contractor is prohibited from employing off-duty COR who are managing any contracts or subcontracts awarded to the Contractor.
4.2.
Hours of Operation
4.2.1.
Normal Base Hours of Operation
The normal LAAFB hours of operation are from 06:00 to 18:00, Monday thru Friday except Federal Holidays. The normal Ft MacArthur hours of operation are from 08:00 to 17:00, Monday thru Friday.
4.2.2.
Federal Holidays
The Contractor is not required to provide services on the following federal holidays:

Presidents Day
Memorial Day
Independence Day
Labor Day
Columbus Day
Veterans Day
Thanksgiving Day
Christmas Day If the federal holiday falls on a Saturday or Sunday, it may be observed on Friday or Monday as directed.
4.2.3.
Performance of Services During Crisis Declared by the National
Command Authority or Overseas Combatant Commander
According to DoDI 3020.37, Continuation of Essential DoD Contractor Services During Crises, and the Air Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined that Instructor Service requirements under this SOW are not essential to be performed during a crisis.
4.3.
Quality Control (QC)
4.3.1.
Performance Evaluation Meetings
The CO may require the Contractor to meet at least weekly with the Fitness Center staff and the CO during the start of the contract. Meetings thereafter will be as often as necessary thereafter as determined by the CO. However, if the Contractor requests, a meeting will be held as well.
4.3.1.1. Scope Changes
Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO only.
4.4.
Administrative Records
All records, documents, and associated papers provided by the Government and generated during the period of this contract become Government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.

Appendix A - Description of Classes
Power Yoga - this strength-based class is suitable for all yogis! Holding active poses for longer periods of time and incorporating core movements throughout. You' ll use muscles you never knew you had and leave this class feeling long, strong and grounded!
Get up and Flow (yoga) - this early am class will provide an invigorating series of yoga postures tailored to your morning movements. Vinyasas uniquely designed to transition you from a waking state into your day by day movement. Leave feel ing centered, focused and ready to tackle the day!
*Body Circuit Challenge - This class focuses on building a strong foundation of biomechanical alignment in your body. Rooted in the whole body movement, from fingers to toes with a core focus, you'll find muscles you never knew you had and move in a fun and functional range of motion.
Yoga Flow & Tune Up - An eclectic blend of yoga, strength, physical therapy and a sustainable practice. Practicing yoga is a process of learning and refinement. The most effective laboratory for self-understanding is through the body. Create alignment in your body and create alignment in your life. *Cycle 45 - A classic and dynamic 45-minute ride of interval zone training to burn calories and condition the heart! Short, sweet and sweaty .... let's ride!
*Studio Spin - Train hard! Interval drills for endurance, power and speed that include time trials, sprints, climbs and pulls. This ride is sure to be exciting and unpredictable.
*Spin & Strength - Speed, Strength & Flex! This 70-min class is a combo of 8-min repeat cycling circuit paired with an 8-min strength format off of the bike. Push your limits in this extended personal circuit-style class and stick around for 'part 2' where we'll begin the recovery phase
Athletic Stretch - Restore the body after a hard push thru your daily routine I We'll stretching, do some foam rolling, breathing and twisting. Come one, come all!
*Weight Room Sweat- Step outside your comfort zone and join us INSIDE the weight room for this circuit-based class! You'll learn weight-room etiquette, how to use new equipment and include heart-pumping blasts of cardio for this calorie melt-down! Be preparred to use ropes, plates, cables, bars and bands to create a stronger you!
*ViPR - Vitality, Performance, Resilliance! Meet your goals head on and fully amped in body and mind! The enhancing benefits of the ViPR equipment-based class are combined in this dynamic full-body strength and endurance workout! We'll use dumbbells, body bars, bosu balls and body weight...and of course the ViPR to take your fitness limits to the next level!
*Barbell Sculpt- A bar-based workout challenge your entire body. Frim up from head to toe, performing the basics to stay fit for life!
*Kettlebell Circuit- This is a 3-2-1 minute circuit will begin with a cardio challenge (3 mins}, followed by a kettlebell strenght move (2 mins) and finally a 1-minute core exercise. Six rounds of this 6-min combo will provide a kettlebell kick start to your day! All levels welcome.
Lengthen & Strengthen - Restore the body and renew your energy as you start your day with this gentle-style of pilates movements. Using body weight to strengthen your mind and muscles through a variety of isometric poses and low-impact movements. This class will leave you refreshed and ready to enjoy the weekend!
*Zumba - This total-body workout combines the elements of cardio, body awareness, muscle conditioning and flexibility. Spend your hour shakin' your hips, movin' your feet and sweating out your frusterations in this high-energy, totally FUN cardio workout! Turn up the beat!
*Strong! - Pairing the upbeat choreographed movements of Zumba and body-weight strenght training this class is sure to get the heart pumping! Time flyes as you follow cues to dynar:nically sculpt your body to the beat!

Instruction shall include, but is not limited to core training, step-n-sculpt, interval (cardio + strength), aerobic dance to include high/low impact, cardio kickboxing, strength training, Power Pump, Boot Camp, indoor cycling, yoga and Pilates.

Appendix D – Definitions Group Fitness Special Event A Group Fitness Special Event is a four-hour event where various instructors alternate teaching aerobic dance, spinning, cardio kickboxing, and yoga. Each teaching session is for one (1) hour.
Appendix E – Applicable Publications and Forms
Publications (to include their supplements at any organizational level) and forms that apply to this SOW are listed below. The publications and forms have been coded as mandatory or advisory. The Contractor is obligated to follow those publications and use those forms coded as mandatory to the extent specified in other sections of this SOW. The Contractor shall be guided by those publications or use those forms coded advisory to the extent necessary to accomplish requirements in this SOW. All publications and forms listed shall be made available by the Government at the start of the contract. Supplements or amendments to listed publications from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those changes that result in a decrease or no changes in the contract price and notify the Contracting Officer in writing of such changes. Before implementing any change that will result in an increase in contract price, the Contractor shall submit to the Contracting Officer a price proposal within 30 calendar days following receipt of the change by the Contractor. The Contracting Officer and the Contractor shall negotiate the change into the contact under the provisions of the contract clause entitled “Changes”. Failure of the Contractor to submit a price proposal within 30 calendar days following receipt of the change entitles the Government to performance according to such change at no increase in contract price (unless the time requirement is waived by the Contracting Officer according to Paragraph C of the changes clause).

Appendix F – Integrated Defense Plan (IDP)
PROOFING AND VETTING FOR EMPLOYEES/CONTRACTORS/VISITORS FOR INSTALLATION ACCESS
1. Criminal Background History (CBH) Requirements. In accordance with (IAW) Directive-Type Memorandum (DTM) 09-012, Interim Policy on Guidance for DoD Physical Access Control, personnel requiring access to a DoD installation must be identity proofed and vetted, as a minimum through the NCIC/CLETS database to determine fitness and eligibility for access to base.
2. Contractor/Newly Hired Employees Pre-Employment Background Checks. A favorable CBH check must be completed for all contractors, subcontractors, their employees and newly hired government employees prior to being granted unescorted access/entry onto LAAFB for a period of 31 consecutive days or longer.
2.1 A CBH will be considered current during a three year period from the date they are completed or if they are a contractor, as their contract dictates.
2.2 Prior to issuing installation entry credentials the 61st Security Forces Squadron (61 SFS) Visitor Control Center will ensure CBHs are conducted on 100% of contractors, subcontractors, their employees and newly hired government employees.
2.2.1 Contracting/Manpower will provide a list of contractors or newly hired government civilians to 61 SFS Visitor Control Center (VCC) at least 14 days prior to the start of their contract/work.
2.2.2 The VCC will run a check of all personnel using the CLETS. IAW AFI 10-245, Antiterrorism Operations, Section 2.18.2 a CBH will consist of a National Criminal Information Center (NCIC) check
3. Contracting Office or Administrator: IAW the AF Federal Acquisition Regulations Supplement (AFFARS), Contracting Officers or administrators will ensure CBH requirements are incorporated into all new, revised, or modified contract documents (statements of work, requests for proposal, invitations for bid, task orders, basic contracts, etc.). In general, existing contracts will not be modified solely to meet this requirement nor will these requirements be added to existing contracts unless the contract requires modification for other reasons and the contract has more than six months remaining life. During the contracting process, the contracting office or administrator will ensure contractors are aware of CBH requirements and local installation procedures and requirements.
4. CBH Requirements: CBH checks will be conducted prior to contract starting date. CBH checks are considered favorable if they do not contain any of the following eighteen disqualifiers.
4.1 The individual is known to be or reasonably suspected of being a terrorist or belongs to an organization with known terrorism links/support.
4.2 The installation is unable to verify the individual's claimed identity.
4.3 There is a reasonable basis to believe the individual has submitted fraudulent information concerning his or her identity.
4.4 There is a reasonable basis to believe the individual will attempt to gain unauthorized access to classified documents, information protected by the Privacy Act, information that is proprietary in nature, or other sensitive or protected information.
4.5 There is a reasonable basis to believe the individual will unlawfully or inappropriately use an access credential outside the workplace.
4.6 There is a reasonable basis to believe, based on an individual's criminal or dishonest history, that issuance of an access credential poses an unacceptable risk to the installation/mission.
4.7 The individual has/had been barred from entry/access to a Federal installation or facility.
4.8 The individual is wanted by Federal or civil law enforcement authorities, regardless of offense or violation.
4.9 The individual has been incarcerated within the past ten years, regardless of offense/violation, unless released on proof of innocence.
4.10 The individual has any conviction for espionage, sabotage, treason, terrorism, or murder.
4.11 The individual’s name appears on any Federal or State agency’s ―watch list‖ or ―hit list‖ for criminal behavior or terrorist activity.
4.12 The individual has been convicted of a firearms or explosive violation within the past ten years.
4.13 The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution.
4.14 There is a reasonable basis to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk to the installation/mission.
4.15 There is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.
4.16 There is a reasonable basis to believe, based on the nature or duration of the individual's illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.
4.17 A statutory or regulatory bar prevents the individual's contract employment; or would prevent Federal employment under circumstances that furnish a reasonable basis to believe that issuance of a access credential poses an unacceptable risk to the installation/mission.
4.18 The individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force.
Note: The above guidelines will be followed as closely as possible, but the Installation CC reserves the right to deny access to any civilian with proper justification. The CBH is protected by the Privacy Act of 1974. All information received is accessible only to Law Enforcement personnel acting in accordance with their assigned duties.
5. Processing Base Access Pass: Before starting work the contractor/employee will go to the VCC and receive a signed letter verifying that a favorable CBH was completed and at that time will be issued a temporary base pass.
LAAFB Integrated Defense Plan2.6.7 Contractor Personnel/Vehicles:
2.6.7.1 All prime contractors/subcontractors/vendors on LAAFB requiring continuous entry/registration of either company or employee-owned vehicles, must submit a complete vehicle listing on company letterhead, to operational contracting (SMC/PK), at 483 N. Aviation Blvd, Los Angeles AFB, El Segundo, CA 90245-2808.
2.6.7.2 If the vehicle listing is approved, the requestor will present the certified list to pass and registration located in Bldg. 272, with the following documents to receive their vehicle pass:
2.6.7.3 Driver’s license
2.6.7.4 Valid vehicle registration
2.6.7.5 Valid proof of insurance
2.6.7.6 Proof of Trusted Agent Sponsorship System (TASS) clearance (via AF FM 2586).
2.6.7.7 If the vehicle listing is not approved, operational contracting/sponsoring activity will provide the contractor/vendor the circumstances surrounding the decision and point-of-contact.
2.6.7.8 Subcontractors may be added to the prime contractor’s vehicle listing by submitting a new request form to operational contracting/sponsoring agency for approval.
2.6.7.9 Successful registration for contractor/vendor vehicles is authority for employees to operate a privately owned or contract/vendor vehicle on LAAFB in pursuit of work, and to drive to and from work. Registration is not, however, authority to drive on base during non-working hours and is not transferable to other vehicles.
2.6.7.10 Before the sale or trade of the vehicle, or upon contract termination, the contractor/vendor must turn-in the vehicle pass to Pass and Identification Office (61 SFS/S5PID).
