STATEMENT OF WORK
CHEMISTRY ANALYZER
Contractor will provide the analyzer, accessories, calibration, factory original or reconditioned repair parts, labor, preventative maintenance , and unscheduled service calls for the period starting on or about 1 April 2016 – 31 March 2017 with four (4) option years to include, but not limited to, the following:
Contractor to supply:
1. A cost per test agreement for one (1) chemistry analyzer.
2. Contractor to supply all required reagents, equipment, and maintenance of supplied equipment.
3. Equipment to be maintained per original equipment manufacturer’s recommendations. Contractor to supply full service preventive maintenance with unlimited service calls as required and an annual preventive maintenance inspection during normal business hours, Monday through Friday between 0730 – 1630 (7:30 AM – 4:30 PM) CST except for federal government holidays.
4. Laboratory personnel will be responsible for daily maintenance to include cleaning and analyte calibration as required. Contractor, biomedical equipment repair, and laboratory shall maintain all service records for supplied equipment. Contractor to provide the government with maintenance history within 5 business days of request.
5. Contractor to warrant that all supply items are free of manufacture’s defects and will repair / replace any unsatisfactory items at contractor’s expense.
6. Contractor price shall be inclusive of all cost including lease of equipment, maintenance of equipment, reagent, shipping and handling.
7. All items are to have a minimum 6-month shelf life from the date of receipt and shipments will include only one lot number for each individual reagent shipped, (if an order is placed for Na+ reagent, upon filling they will all be the same lot number).
8. All supplied items are to be certified for use within the instrument.
9. All items received are to be approved for use (where applicable) in U.S. based medical treatment facilities.
10. Power requirement is 115V.
11. Footprint must be no larger than 5ft x 5ft x 5ft.
12. The testing platform must include the following testing capabilities within one instrument with no requirement for a water supply or plumbing since these would require facility modification(s):

13. As part of the cost per test agreement, the contractor shall submit invoices and be paid in arrears for actual quantities ordered. The projected annual volume of slides and other consumables is as follows:
