This is a combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested, and a separate solicitation will not be issued.

10. Description of requirements for the services to be acquired:
Confined Space Entry shall be required for the performance of this contract.
Service will be performed on Hanscom AFB, MA. It will include Infrared Scanning of underground high voltage electrical manholes to ensure the Base High Voltage Electrical Grid remains operational. All manholes shall be pumped empty of any and all standing water, and any and all debris and sludge shall be removed prior to scanning activities. It is anticipated that approximately seven (7) manholes are not confirmed as clean, and will likely need to be addressed. These locations are included in the total shown.
The Contractor shall provide all management, labor, tools, equipment, hoses, gauges, test equipment, hearing protection, eye protection including prescription safety glasses, materials, supplies and parts, transportation and other incidentals necessary to travel to Hanscom AFB, MA, to inspect, test, and perform related actions as identified in the PWS in accordance with all applicable Federal, State and Local codes and regulations.
The Contractor shall be responsible for ensuring all services and parts provided under this contract are in accordance with the terms and conditions outlined herein and conform to the manufacturer’s equipment specifications and this Performance Work Statement (PWS).
The nature of this requirement and the timing of this solicitation shall require the Awardee to physically attend a contract kickoff meeting on Hanscom AFB prior to 23 September 2016.

Addendum to 52.212-1 Instructions to Offerors –Commercial Items
1. BASIS FOR CONTRACT AWARD
This FAR Part 12 acquisition is being set-aside 100% for small business in accordance with FAR 19.502-2(b) under North American Industry Classification System (NAICS) code 541330. The Government intends to award a single firm-fixed-price contract using the Lowest Price Technically Acceptable (LPTA) source selection process (FAR 15.101-2) to the Offeror that is determined to be responsible in accordance with FAR 9.103. Only proposals that are evaluated as Technically Acceptable will be evaluated for Price. The Government intends to award without discussions, but reserves the right to enter into discussions if determined to be in the best interest of the Government. Therefore, it is particularly important that each Offeror provide its best offer. Proposals shall be reviewed to determine whether they conform to the formal requirements of the solicitation. Failure to respond or follow the instructions regarding the organization and content of the proposal volumes and the provisions listed herein may result in the Offeror’ s proposal being eliminated from further consideration for award. The Offeror must complete and provide representations and certifications with the proposal in accordance with FAR 52.212- 3 Offeror Representations and Certifications – Commercial Items (Mar 2015). In accordance with FAR 15.304(c)(iii), Past performance will not be considered as an evaluation factor for this RFP. Prior to any award being made, the awardee must be registered in System for Award Management (SAM) website accessed through http://www.acquisition.gov. The Government reserves the right to make one, more than one, or no award whatsoever under this solicitation.

Addendum to FAR 52.212-1, Instructions to Offerors
The following is to be added to FAR 52.212-1 paragraph (b):
a) Please provide the following documentation in your technical quote:
a. Subfactor 1: For each Technician, Confined Space Entry Certification IAW AFI 91-203, the U.S. Air Force Mishap Prevention Program, and AFFARS Clause 5352.223-9001, Health and Safety on Government Installations; with a current Certificate copy submitted with Offer.
b. Subfactor 2: For each Infrared Thermographer, at minimum Level 1 (One) Infrared Thermographer Training Certification; with a current Certificate copy submitted with Offer.
c. Subfactor 3: For each Infrared Camera, a detailed specification sheet and current calibration
Certification; woth a current Certificate copy submitted with offer.
b) Please provide a completed copy of the Quote Schedule in its original format.
NOTE: The aforementioned documentation is required for a quote to be considered technically acceptable for contract award.

18. The Following Provisions Are Incorporated By Full Text:
52.212-2 Evaluation – Commercial Items (Oct 2014)(Tailored)
(a) The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

In accordance with FAR 15.304(c)(iii), Past Performance will not be an evaluation factor for this solicitation.
Award will be made using the Lowest Price Technically Acceptable (LPTA) source selection process per FAR 15.101-2. Only those proposals that are evaluated as Technically Acceptable will be evaluated for Price.
(b) Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
Addendum to 52.212-2(a) Evaluation – Commercial Items
EVALUATION FACTORS FOR AWARD
TECHNICAL FACTOR EVALUATION
Initially, Technical Proposals will be evaluated on a pass/fail basis assigning ratings of Acceptable or Unacceptable using the ratings and descriptions below:

Technical Proposals will be evaluated against the following Technical Subfactors:
Subfactor 1: For each Technician, Confined Space Entry Certification IAW AFI 91-203, the U.S. Air Force Mishap Prevention Program, and AFFARS Clause 5352.223-9001, Health and Safety on Government Installations; with a current Certificate copy submitted with Offer in accordance with PWS section 1.3.
Subfactor 2: For each Infrared Thermographer, at minimum Level 1 (One) Infrared Thermographer Training Certification; with a current Certificate copy submitted with Offer in accordance with PWS section 1.4.3.
Subfactor 3: For each Infrared Camera, a detailed specification sheet and current calibration Certification; with a current Certificate copy submitted with offer in accordance with PWS section 1.4.2.
Inclusion of the completed Quote Schedule shall also be evaluated with these Subfactors.
All technical subfactors are of equal importance. Technical Proposals must be evaluated as Acceptable for all Technical subfactors to be considered Technically Acceptable.
PRICE FACTOR EVALUATION AND CONTRACT AWARD
Next, only Proposals that are evaluated as Technically Acceptable will be evaluated for price. Price will be evaluated for reasonableness in accordance with FAR 15.404-1. Award will be made to the lowest priced, Technically Acceptable proposal in accordance with FAR 15.101-2 to the contractor who is deemed responsible in accordance with FAR Part 9.

21. The Following Clauses Are Incorporated By Full Text:
FAR 52.209-5 – CERTIFICATION REGARDING RESPONSIBILITY MATTERS (OCT 2015)
(a)
(1) The Offeror certifies, to the best of its knowledge and belief, that --
(i) The Offeror and/or any of its Principals --
(A) Are [_] are not [_] presently debarred, suspended, proposed for debarment, or declared ineligible for the award of contracts by any Federal agency;
(B) Have [_] have not [_], within a three-year period preceding this offer, been convicted of or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public (Federal, State, or local) contract or subcontract; violation of Federal or State antitrust statutes relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, violating Federal criminal tax laws, or receiving stolen property (if offeror checks “have”, the offeror shall also see 52.209-7, if included in this solicitation); and
(C) Are [_] are not [_] presently indicted for, or otherwise criminally or civilly charged by a governmental entity with, commission of any of the offenses enumerated in paragraph (a)(1)(i)(B) of this provision; and

(1) Federal taxes are considered delinquent if both of the following criteria apply:
(i) The tax liability is finally determined. The liability is finally determined if it has been assessed.
A liability is not finally determined if there is a pending administrative or judicial challenge. In the case of a judicial challenge to the liability, the liability is not finally determined until all judicial appeal rights have been exhausted.
(ii) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.
(2) Examples.
(i) The taxpayer has received a statutory notice of deficiency, under I.R.C. §6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(ii) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. §6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability.
This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.
(iii) The taxpayer has entered into an installment agreement pursuant to I.R.C. §6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.
(iv) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).
(ii) The Offeror has [[_] has not [_], within a three-year period preceding this offer, had one or more contracts terminated for default by any Federal agency.
(2) “Principal,” for the purposes of this certification, means an officer; director; owner; partner; or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager; head of a division or business segment;
and similar positions).
(a)This Certification Concerns a Matter Within the Jurisdiction of an Agency of the United States and the Making of a False, Fictitious, or Fraudulent Certification May Render the Maker Subject to Prosecution Under Section 1001, Title 18, United States Code.
(b) The Offeror shall provide immediate written notice to the Contracting Officer if, at any time prior to contract award, the Offeror learns that its certification was erroneous when submitted or has become erroneous by reason of changed circumstances.
(c) A certification that any of the items in paragraph (a) of this provision exists will not necessarily result in withholding of an award under this solicitation. However, the certification will be considered in connection with a determination of the Offeror’s responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.
(d) Nothing contained in the foregoing shall be construed to require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a) of this provision. The knowledge and information of an Offeror is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.
(e) The certification in paragraph (a) of this provision is a material representation of fact upon which reliance was placed when making award. If it is later determined that the Offeror knowingly rendered an erroneous certification, in addition to other remedies available to the Government, the Contracting Officer may terminate the contract resulting from this solicitation for default.

FAR 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES
OR EXECUTIVE ORDERS--COMMERCIAL ITEMS (DEVIATION 2013-O0019) (JUN 2016)
(a) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller
General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(b)
(1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—
(i) 52.203-13, Contractor Code of Business Ethics and Conduct (Apr 2010) (41 U.S.C. 3509).
(ii) 52.219-8, Utilization of Small Business Concerns (Oct 2014) (15 U.S.C. 637(d)(2) and (3)), in all subcontracts that offer further subcontracting opportunities. 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 3 years and 6 months.
52.219-1 SMALL BUSINESS PROGRAM REPRESENTATIONS (OCT 2014)
(a) Definitions. As used in this provision-- “Economically disadvantaged women-owned small business (EDWOSB) concern” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business concern eligible under the WOSB Program.
“Service-disabled veteran-owned small business concern”--
(1) Means a small business concern--
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans;
and

FAR 52.228-5 INSURANCE – WORK ON A GOVERNMENT INSTALLATION. (Jan 1997)
(a) The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required below:
A. Workmen's Compensation and Employers Liability Insurance as required by law except that if this contract is to be performed in a state which does not require or permit private insurance, then compliance with the statutory or administrative requirements in any such state will be satisfactory. The required Workmen's Compensation insurance shall extend to cover employer's liability for accidental bodily injury or death and for occupational diseases with a minimum liability limit of $100,000.00.
B. General Liability Insurance. Bodily injury liability insurance, in the minimum limits of $500,000.00 per occurrence shall be required on the comprehensive form of policy.
C. Automobile Liability Insurance. This insurance shall be required on the comprehensive form of policy and shall provide bodily injury liability and property damage liability covering the operation of all automobiles used in connection with the performance of the contract. At least the minimum limits of $25,000.00 per person and $50,000.00 per accident for bodily injury and $25,000.00 per occurrence for property damage shall be required.
(b) Before commencing work under this contract, the Contractor shall notify the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government’s interest shall not be effective –
(1) For such period as the laws of the State in which this contract is to be performed prescribe; or
(2) Until 30 days after the insurer or the Contractor gives written notice to the Contracting Officer, whichever period is longer.
(c) The Contractor shall insert the substance of this clause, including this paragraph (c), in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the Schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors’ proofs of required insurance, and shall make copies available to the Contracting Officer upon request.

AFFARS 5352.201-9101 OMBUDSMAN (APR 2014)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. AFFARS 5352.223-9000 Elimination of Use of Class I Ozone Depleting Substances (ODS) (NOV 2012)
(a) Contractors shall not:
(1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or
(2) Provide any specification, standard, drawing, or other document that establishes a test, operation, or maintenance requirement that can only be met by use of a Class I ODS as part of this contract/order.

AFFARS 5352.223-9001 Health and Safety on Government Installations (NOV 2012)
(a) In performing work under this contract on a Government installation, the contractor shall:
(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and
(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.
(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
