STATEMENT OF WORK

AQUARIUM SERVICE

460 MSG/SVYC CHILD DEVELOPMENT CENTER

BUCKLEY AIR FORCE BASE

FY 2009

5 September 2008

1. Description of Services. This Statement of Work (SOW) defines the actions for the contractor to provide all tools, labor, parts, and transportation necessary to perform the services in paragraph 2.2.

2. Scope of Work. Contractor shall provide the Buckley Child Development Center (CDC) Bldg 725, located at 18600 E. Crested Butte Avenue on Buckley Air Force Base, with aquarium maintenance service not earlier than 9:00 am and not later than 2:00 pm each Tuesday from 1 Oct 2008 to 30 Sep 2009.

2.1 Requirements. Contractor must have specialized training, knowledge, and skills necessary to perform stated salt-water and fresh-water aquarium maintenance. Contractor shall ensure that all aquariums (one salt-water aquarium and six- fresh-water aquariums) are continuously cleaned and maintained in proper working order. The tanks are located in the foyer (salt-water) and rooms 12, 13, 16, 17, 18, and 19 (fresh-water). Contractor shall ensure that the aquariums are properly stocked with fish and live coral as required to replace any dead, dying, or ill fish or coral. Contractor will provide all necessary food for sustaining fish. Contractor will respond to service calls within 4 hours of notification that aquariums are malfunctioning. Service call response will be at no additional cost to the government. In addition, the contractor must have reliable transportation and credentials allowing them to enter Buckley Air Force Base as necessary to perform aquarium maintenance service.

2.1.1 Contractor must obtain Colorado criminal background check’s at the Contractor’s expense from the Colorado Bureau of Investigation (CBI) for each employee performing aquarium service at the Crested Butte CDC. A copy of the completed background check for each employee needing access to the Crested Butte CDC is required within 7 working days after contract award and shall be sent directly from the CBI to the CDC at the following address:

The criminal background check will be kept on file in the Crested Butte CDC Director’s Office. All information of record provided by the CBI, both favorable and unfavorable, will be assessed in terms of relevance, recentness, and seriousness. Final suitability determination will be made by the Crested Butte CDC Director, Family Member Programs Flight Chief and Human Resource Officer after reviewing the CBI background check. Mandatory disqualifying criteria include any conviction for a sexual offense, a drug felony, a violent crime, or a criminal offense involving a child or children. A determination of “unsuitable” as determined by the designees above shall result in immediate termination of this contract without further cost to the government.

2.1.2 With the consent of the Contracting Office, aquarium service may begin prior to the completion of the background checks. The Crested Butte CDC Director or Assistant Director will ensure a CDC staff member accompanies the contractor at all times.

2.1.3 Contractor and all employees needed to provide aquarium service shall present a neat appearance since the Crested Butte CDC is a high-usage facility on a military installation.

2.2 Performance. Contractor shall enter Buckley AFB to provide service to the Buckley AFB Crested Butte Child Development Center and immediately depart from Buckley AFB unless the contractor is under contract providing service to another facility or the contractor has business to conduct at the Buckley AFB Contracting Office. The contractor shall depart from the Base upon completion of al official business.

2.2.1 Contractor shall provide service within a week of being awarded the contract.

2.2.2 Contractor shall present a valid Colorado Driver’s License to the Crested Butte CDC administrative staff prior to signing in to ensure the Contractor is on the approved access list. The Contractor will sign in and out of the Crested Butte CDC at the start and end of each aquarium maintenance service process.

3 Contractor Registration. Contractor shall be required to invoice through the Wide Area Work Flow (WAWF) System at https://wawf.eb.mil. More information regarding WAWF will be provided upon contract award. The contractor must have current registration through Central Contractor Registration (CCR), www.ccr.gov.

3.1 Pass and Identification. At least five days prior to arrival, the contracting office shall receive Visit authorization Letter listing the full name, state issued I.D. number or driver’s license number and the state of issue, social security number, date of birth, and company name of all of the individuals who will be visiting the installation. The VAL must provide the purpose of the visit, the date of the visit, and a Point of Contact (POC).

3.2 Base Access. The Contractor shall obtain appropriate Base access for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Buckley AFB. Current vehicle registration, current proof of insurance and a valid driver’s license must be presented for all vehicles to be registered. All employees accessing Buckley AFB must be citizens of the United States. Base access requests must be submitted to 460 CONS/LGCB at least five days in advance.
