STATEMENT OF WORK

Lahontan National Fish Hatchery

Well #6 Casing, Pump and Motor Replacement

1. GENERAL: Lahontan NFHC has a compelling need for the repair/replacement of well #6 casing, pump and motor. This well is necessary for safe and reliable operation of the facility. The facility normally has four operational wells, and they are rotated with three wells running with one well as a backup. Since this well is not functional at this time, the facility is at risk in an emergency situation.

2. SCOPE: Contractor shall provide all equipment, materials, vehicles, managerial, administrative, and direct labor personnel to perform the following tasks:

2.1. Swaging of the casing to remove approximately four foot bulge in pipe.

2.2. Bail debris from approximately 668 feet of casing.

2.3. Furnish 140 feet of 12”x .250 wall blank steel casing.

2.4. Furnish 560 feet of 12" x .250 wall quad slotted perforated pipe (48 rows of slots per foot).

2.5. Furnish approximately 1/4” x 1/8” silica resources well gravel.

2.6. Install casing and gravel pack.

2.7. Install new franklin submersible pump and motor (or equal), and any and all gaskets, retainers, piping, and connectors needed to complete the job.

3. PLACE/DELIVERY AND PERIOD OF PERFORMANCE: All work will be performed on site. The address is Well #6 Highway 395, Gardnerville, NV 89410.

4. INSPECTION AND ACCEPTANCE: All work will be inspected by the technical coordinator to ensure electrical system is functioning properly.

5. GOVERNMENT FURNISHED EQUIPMENT OR MATERIALS: Contract shall provide all materials and equipment for performance of this project.

6. AVAILABILITY: The work will be performed outside of a permanent structure between the hours of 7a.m and 3p.m. Access to facility will be granted by hatchery personnel.
