ECRI's recommendations for minimum performance requirements for single channel, multichannel, and interpretive electrocardiographs:
The multichannel electrocardiograph should be capable of recording the standard 12 leads (3 bipolar, 3 augmented or unipolar, and 6 chest or precordial). The unit should also have at least 3 channels of simultaneous ECG data, but be capable of simultaneous acquisition, display, and/or printing of all 12 leads. It should measure all basic axes and durations, including RR, PQ, QT, ATC, P, QRS, T, and heart rate.
The unit should have indicators for battery status, loose electrode contact, systems status, and artifacts.
The unit should be capable of storing ECG waveforms for later retrieval, printing, and/or transmission. The electrocardiograph should allow the operator to enter patient data; it should include, at a minimum, on each record, the patient's name or ID number, age, sex, weight, and height. Also, each record should include the recording date and time, sensitivity setting, chart speed, and leads being recorded. The unit should have a digital interface (e.g., RS232, wireless, Ethernet) to permit communications between the unit and other electrocardiographs, computers, or a data management system. The electrocardiograph may include a complete interpretation software package. The unit should store at least 200 ECGs and have expandable storage capability.
The unit should be capable of printing at least three leads of the ECG waveform continuously with manual switching between leads (manual rhythm strip printout). It should offer sensitivity settings of at least 2.5, 5, 10, and 20 mm/mV. The recorders typically have a chart speed of 5, 10, 25, and 50 mm/sec.
Interpretive electrocardiographs should meet all the requirements mentioned above. In addition, these units should detect electrical signals associated with cardiac activity, compare these signals with an internal algorithm, and print out the patient's ECG along with interpretation.
These units may operate on either line or battery power. If the unit is battery operated, the batteries should be rechargeable to reduce the cost of frequent battery replacement; the unit should operate on battery power for at least six hours and should have a charger that is able to charge while connected to line power. Also, battery-powered units should have a low-battery indicator clearly evident on the outside of the device. This indicator should also indicate that the batteries are being recharged, not simply that the charger is plugged into an AC outlet.
In addition to these minimum requirements, ECRI considers the following features or functions to be particularly advantageous: large screen size (25 cm [10"] or greater); full disclosure is preferred; wireless data acquisition from patients; 15-leads or more of data acquisition; or wireless transmission of completed ECG data to an ECG data management system.
