Nevada Skies Youth Wellness Center Propane Service Delivery Service Statement of Work
1. SCOPE OF WORK
Contractor s LP propane gas service to Government residential treatment center at Nevada Skies Youth Wellness Center, 104 Big Bend Ranch Road, Wadsworth, NV.
A. Contractor shall provide all labor, materials, tools, equipment and transportation to provided propane delivery to the service locations B. Propane shall be provided on an as needed basis for four (4) 1000 gal tanks, four (4) 1000 gal tanks at the government residential treatment center. Propane tanks and meters are the property of the Indian Health Service.
C. Coordinate propane services with the point of contact (POC) for delivery of propane as required by Nevada Skies Youth Wellness Center (NSYWC).
D. Contractor shall deliver and provide propane in accordance with all federal, state, local, tribal and IHS regulations.
E. Contractor is responsible for providing a receipt for delivery, quantity, and price.
F. In order to facilitate efficiencies, the Contracting Officer’s Representative (COR) and the contractor may agree on a regular schedule for delivery of propane gas to the various locations dependent upon usage.
G. Contractor is responsible for reading the existing individual meters and billing/collecting payment from the government housing tenants for actual use of propane. In addition, the date subsequent propane charges shall revert back to the Nevada Skies Youth Wellness Center Government Account shall be provided to the contractor. The process for reporting to the contractor shall be agreed upon between the government and the contractor no later than 30 days after contract award and shall become part of the official contract file.
2. Contracting Officers Representative (COR) The Deputy Chief Executive Officer, or their designee, is authorized as the Contracting Officer’s Representative. Only the COR, or alternate, is authorized to place orders under this contract. When placing an order, the COR or designee, shall indicate propane tanks to be filled. The contractor shall provide a delivery ticket for signature by the government representative.
The COR, or designee, shall be responsible for the following:
A. Monitoring the contractor's progress for timely delivery, including the surveillance and assessment of performance and recommending necessary changes to the contracting officer;
B. Interpreting the scope of work;
C. Technical evaluation as required;
D. Inspection of all deliveries and acceptance as required by this contract;
E. Assisting the contractor in the resolution of technical and administrative problems encountered during the performance of the contract;
F. Certify receipt and acceptance of services on contractor's delivery ticket and will order a receiving report from property and supply when delivery ticket/receipt is certified;
G. The contracting officer is the only person with authority to act as agent of the government under this contract. Only the contracting officer has authority to: (1) direct or negotiate any changes in the statement of work; (2) modify or extend the period of performance; (3) change the delivery schedule; and (4) authorize reimbursement to the contractor for any costs incurred during the performance of this contract.
