About this file

Attachment C is a Government-Wide Debarment & Suspension Certification document from a solicitation related to the City of Rock Hill, South Carolina's street tree pruning and assessment project. The document is a standard federal certification form requiring contractors to affirm their legal and ethical standing in government contracting, specifically confirming that the contractor and its principals have not been debarred, suspended, or excluded from federal transactions, and have not been convicted of fraud, criminal offenses, or other specified misconduct within the past three years.

The certification requires the contractor to verify they are not currently indicted or charged with any governmental offenses, have not had public transactions terminated for cause, and are prepared to attest to the truthfulness of these statements under 31 U.S.C. Sections 3801 et seq. The document is part of a procurement process funded by the United States Department of Agriculture (USDA) and United States Forest Service (USFS), and requires an authorized official's signature, printed title, witness, and date to validate the contractor's compliance with federal contracting responsibility standards.
