d. $2,656,307.95 Question 2: Can you please provide me with a clearer understanding of how the Past Performance Questionnaire should be completed and submitted.

· Is only one to be completed? There’s a maximum page limit of 6 and the survey is 6 pages long.

· Is the survey to be included in the response and also emailed to CSOSA?

Answer: The Past Performance Questionnaires, Attachment J-9, should be completed by the Past Performance References and the completed Questionnaires submitted by the Past Performance Reference by email to Eartha.coleman@CSOSA.gov and Zalma.Ross@csosa.gov. Volume II, Past Performance, is limited to the Offeror’s discussion of the relevance of the past performance references to the requirements of this Solicitation.

Question 3: I haven’t been able to figure out how to make an electronic submission. I can’t find a blue click here link to submit a response.

Answer: Offerors should disregard the proposal submission guidance on page 92 directing that proposals be submitted through Federal Business Opportunities (www.fbo.gov). Offerors should submit their proposal by email directly to Zalma.Ross@csosa.gov and cc: William.Hall@csosa.gov.

Question 4: There doesn’t appear to be enough space in 20 pages to include resumes and licenses. Are biosketches acceptable?

Answer: The proposal submission guidance for Volume I on page 91 states the following: “Volume I shall not exceed twenty (20) pages, excluding the cover page, Table of Contents, List of Tables and Figures, List of Acronyms, Legal Offer, Section K, and Key Personnel Resumes.”

Question 5: It appears the amendment has to be submitted in advance. Where do I send that?

Answer: The signed SF-1449 and SF-30’s should be part of the Offeror’s proposal submission. They are excluded from the Volume I twenty (20) page limitation.
