This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.
This solicitation is issued as a request for quotation IAW FAR Part 12 and 13. Submit quotes in reference to RFQ number FA480920Q0063.
This requirement is being solicited as a small business set aside. The NAICS code for this requirement is 511210 with a size standard of $41,500,000.
This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06 Effective 05 Jun 2020. Defense Federal Acquisition Regulation 20200618, Effective 18 Jun 2020.

INSTRUCTIONS TO OFFERORS (Addendum to 52.212-1)
QUOTE SPECIFICS:
In order to be considered for evaluation quotes must include a Price Exhibit and a Technical Exhibit.
1. Quote shall include the offeror's Price Exhibit as follows:
Contractor shall complete and return the Bid Schedule (as shown above). Vendors must note that separate Contract Line Item Numbers (CLINs), are not prescribed for purpose of associated costs entailing taxes, fees, transportation, delivery, etc. Accordingly, quoted unit pricing for requirements at the CLIN level under this solicitation, shall be all inclusive of all associated costs (i.e. all material, transportation, labor, delivery, removal, etc.)
2. Quotes shall include the offeror's Technical Exhibit as follows:
Specifications Compliance: Contractor shall provide, in sufficient detail, a description of the products being provided, in accordance with Appendix 1 – “Salient Characteristics”.

BASIS FOR CONTRACT AWARD: (Addendum to 52.212-2)
Evaluation of Quotes: In accordance with FAR 12.602(a) and FAR 13.106-2, the following is provided:
(a) The Government intends to award a purchase order resulting from this Request for Quotation (RFQ) to the vendor whose quotation conforms to the solicitation and is considered most advantageous to the Government, price and other factors considered. Offerors should propose their best solution to meet the stated requirements.
(b) The following factors shall be used to evaluate quotations:
1. Price
2. Technical Capability
(c) Basis of Award: The award will be made to the vendor whose quotation represents the best value to the government. As allowed by FAR 13.106-2(b)(1), the Lowest Price Technically Acceptable (LPTA) selection process shall be applied. Award will be made to the vendor who is deemed responsible and responsive, and represents the best value to the government based on selection of the lowest evaluated price that is deemed technically acceptable. The best value is represented by the lowest priced technically acceptable quotation. To be eligible for award, a quotation must meet all technical requirements, conform to all required terms and conditions, and provide all information required as defined in Addendum to 52.212-1. In order to be rated "technically acceptable" the offer must have an "acceptable" rating in the Technical Factor. The technical area will be evaluated on an "acceptable" or "unacceptable" basis. If the quotation received is determined to be unacceptable, the vendor will be excluded from competition and will not be considered for award. Additionally, if no offers are rated technically acceptable, the CO has reserved the right to cancel the Solicitation in lieu of conducting discussions. In this case, the Government is under no obligation to release a new Solicitation.
EVALUATION FACTORS:
PRICE EXHIBIT
(a) A quote's Total Evaluated Price (TEP) will be determined by the sum of all separately priced CLINs as presented on the combined synopsis/solicitation. If separate CLIN prices AND a TEP (as applicable) are not included, not clearly stated, miscalculated and/or not understandable, the offer will be rejected as non-responsive.
TECHNICAL EXHIBIT
(a) Each technical factor will be evaluated on an acceptable/unacceptable basis and assigned a rating on whether the quote meets the solicitation requirements. Extra credit will not be assigned for exceeding the acceptable/unacceptable criteria. A rating of "Acceptable" will be required in the factor to be eligible for award.

52.219-28 Post-Award Small Business Program Representation. (JUL 2013)
As prescribed in 19.309(c), insert the following clause:
Post-Award Small Business Program Rerepresentation (Jul 2013)
(a) Definitions. As used in this clause- Long-term contract means a contract of more than five years in duration, including options. However, the term does not include contracts that exceed five years in duration because the period of performance has been extended for a cumulative period not to exceed six months under the clause at 52.217-8, Option to Extend Services, or other appropriate authority.
Small business concern means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR part 121 and the size standard in paragraph (c) of this clause. Such a concern is not dominant in its field of operation when it does not exercise a controlling or major influence on a national basis in a kind of business activity in which a number of business concerns are primarily engaged. In determining whether dominance exists, consideration shall be given to all appropriate factors, including volume of business, number of employees, financial resources, competitive status or position, ownership or control of materials, processes, patents, license agreements, facilities, sales territory, and nature of business activity.
(b) If the Contractor represented that it was a small business concern prior to award of this contract, the Contractor shall rerepresent its size status according to paragraph (e) of this clause or, if applicable, paragraph (g) of this clause, upon the occurrence of any of the following:
(1) Within 30 days after execution of a novation agreement or within 30 days after modification of the contract to include this clause, if the novation agreement was executed prior to inclusion of this clause in the contract.
(2) Within 30 days after a merger or acquisition that does not require a novation or within 30 days after modification of the contract to include this clause, if the merger or acquisition occurred prior to inclusion of this clause in the contract.
(3) For long-term contracts-
(i) Within 60 to 120 days prior to the end of the fifth year of the contract; and
(ii) Within 60 to 120 days prior to the date specified in the contract for exercising any option thereafter.
(c) The Contractor shall rerepresent its size status in accordance with the size standard in effect at the time of this rerepresentation that corresponds to the North American Industry Classification System (NAICS) code assigned to this contract. The small business size standard corresponding to this NAICS code can be found at http://www.sba.gov/content/table-small-business-size-standards.
(d) The small business size standard for a Contractor providing a product which it does not manufacture itself, for a contract other than a construction or service contract, is 500 employees.
(e) Except as provided in paragraph (g) of this clause, the Contractor shall make the representation required by paragraph (b) of this clause by validating or updating all its representations in the Representations and Certifications section of the System for Award Management (SAM) and its other data in SAM, as necessary, to ensure that they reflect the Contractor's current status. The Contractor shall notify the contracting office in writing within the timeframes specified in paragraph (b) of this clause that the data have been validated or updated, and provide the date of the validation or update.
(f) If the Contractor represented that it was other than a small business concern prior to award of this contract, the Contractor may, but is not required to, take the actions required by paragraphs (e) or (g) of this clause.
(g) If the Contractor does not have representations and certifications in SAM, or does not have a representation in SAM for the NAICS code applicable to this contract, the Contractor is required to complete the following rerepresentation and submit it to the contracting office, along with the contract number and the date on which the rerepresentation was completed:

AFFARS 5352.201-9101 Ombudsman
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

Appendix 1: Salient Characteristics
4TH Operational Medical Readiness Squadron (OMRS) Product Description
Human Performance Program, OHWS Software
1.0 OBJECTIVE:
The 4 OMRS Human Performance Cell has a requirement for program design and distribution software package.
2.0 BACKGROUND:
The Human Performance Team at 4 MDG will use this software to assist the Human Performance Optimization program by allowing the staff to efficiently create, save and distribute rehabilitation and performance programs, monitor progress and communicate with all Airmen no matter where they are located around the globe.
3.0 DESCRIPTION OF REQUIREMNT:
3.1 Product Name: Brand Name or Equal BridgeTracker Platinum Military Software
Bundle
3.2 Description and Capabilities: Software technology platform to design, deliver, and monitor physical training programs.
3.3 Specifications/Salient Characteristics:
Brand Name or Equal BridgeTracker Platinum Military Software Bundle o The technology platform needs to provide the capability for human performance professionals (HP staff) to design, deliver & monitor physical training programs.
o The technology needs to be web browser, Android and iOS phone & tablet native app accessible.
o The technology platform must provide the following programming criteria:
HP staff need to be able to modify the Airman's program while the program is still active. To clarify, a program should not need to be unpublished in order to make edits
While programming, HP staff needs to be able to move seamlessly from a weekly view (ex: Day 1, Day 2 and Day 3) to a synced daily view over the course of multiple weeks, whereby ‘Day 1’ is visible across multiple weeks. Staff members should be able to adjust any movements or parameters within those prescribed movements while in an expanded view across multiple weeks.
When building a program, must have at least the following complete list of exercise parameters for exercise prescriptions: reps, time, distance (m, km, in, ft, yd, mi), height (m, cm, in, ft), weight (lbs, kg - %1RME, %1RME of a linked exercise, %BW, Absolute), velocity (mi/h, km/h, m/s), power, force, heart rate, heart rate zone, calories, and RPE.
When building a program, HP staff needs to be able to add weeks in the moment. They must not be required to set the number of weeks before beginning the program’s design.
HP staff needs to be able to mark specific sets when prescribing training. These sets should then be pulled out into a separate view, allowing for easy monitoring of key benchmarks.
HP staff needs to be able to design & store template programs containing 1 or more weeks of training. Furthermore, HP staff needs to be able to design a ‘parent’ template program which contains within it 1 or more ‘child’ programs. For example, the ‘parent’ program “2020 Macrocycle” could contain the following ‘child’ programs “General Prep Cycle”, “Hypertrophy Cycle”, “Strength Cycle”. To clarify, there should be separate areas of the tool to store template ‘parent’ programs, template ‘child’ programs and template individual workouts. Each with their own unique naming and search functionality
The technology needs to offer an algorithm to improve individualized weight prescription for each Airman and each exercise, every time the Airman performs the movement. HP staff needs to be able to prescribe an exercise, sets, reps and a % of customized intensity (e.g. 1 to 100). The algorithm needs to take into consideration individual Airman’s history in the exercise, reps prescribed and % customized intensity prescribed to assign an individualized weight prescription.
The algorithm needs to adjust the prescription for the following lift after Airmen update their results.
Technology platform needs to include a recommendation engine for designing programs for the HP staff to program quickly and efficiently via evidence-based recommendations.
HP staff needs to receive a multi-week custom-optimized phase based on the following choices: HP staffs estimation of individuals training age, intent or theme of the program (ex: general prep, hypertrophy, metabolic conditioning, max strength), number of weeks, number of days in the week, equipment access & workout duration. There needs to also be the option to include conditioning, medicine ball, plyometric & speed/agility exercises or groupings of exercises.
Once designed, this custom phase needs to be customizable for the HP staff.
o The technology needs to allow HP staff to monitor and record test sets without notifying Airmen that there is anything different about these sets. These sets need to be filtered out into a separate report marked by date the sets were performed.
o The software must provide efficient User Experience / User Interface (UX/UI) via the following means:
Training programs needs to be capable of being delivered on a calendar or in a calendarless format.
For example, a calendar program would be assigned for specific days such as Monday, Wednesday and Friday for a 3 day a week program. An Airman or coach needs to be able to move workouts from Wednesday to Thursday as needed. A calendarless program does not have specific days of the week assigned, but rather is delivered in a list format whereby Airmen can move down the list of workouts without a set schedule.
Native mobile app needs to allow multiple Airmen & their respective training programs to be displayed on the tablet screen at once. Within this interface, Airmen need to be able to:
visualize and enter data for their workouts access previous exercise history in a table and linear graph access exercise videos record videos & notes of themselves performing the exercise The platform needs a robust library of movements with video demonstrations viewable on all platforms (web, iOS phone, iOS tablet, Android phone, Android tablet). HP staff needs to also have the option to add new movements with videos directly to the platform and replace existing videos if desired.
Native mobile app needs to allow HP staff to edit individual programs, once the workouts are already started, in real-time.
This includes the ability to switch an active workout with a template workout already stored in the system.
HP staff needs to be able to add text cues and still pictures to appear with the exercise videos to further enhance education around correct execution of movements.
The mobile application needs to include a daily wellness questionnaire that soldiers can fill out quickly. Questions related to sleep quality, mood, energy level, stress levels and soreness levels need to be included in this questionnaire and results need to be viewable on both mobile and web platforms. The questionnaire also needs to be customizable.
Printed workout cards need to be able to be printed for a single Airman or an entire unit in the following formats:
a weekly view (ex: Day 1, Day 2 and Day 3)* a daily view of multiple weeks (ex:’‘Day 1’ is visible across multiple weeks) o The software must provide the level of security via the following means:
All data needs to be encrypted and not accessible to non-organizational users at all times.
For example, if a user is added to our organization but is already a member of the technology platform through another client, none of our data should be visible to the other client (external user).
The software needs to be subjected to a penetration test by an outside, third party organization to ensure the highest levels of cyber security. Changes should be made to the software firewall and security features based on reported cyber security breaches from the third party organization.
All user and training data should be hosted and stored in USA, encrypted at rest, encrypted in transit, All user and training data can be accessible and available to be securely exported and deleted upon request.
Data integrations with athlete management systems are secure and employ best practices in authentication, data transfer and ownership.
Software needs to comply with SSL encryption standards Airmen profiles need to be available on web and tablet applications, viewable by the Airman’s coach(s) and the Airman. No profile information should be shared with other Airmen. Profile information needs to include the following:

The Government reserves the right to perform inspections and surveillance to evaluate the Contractor’s compliance to the contract terms to include proper delivery of quantity and type of goods. The Government will make every effort to ensure that the surveillance methods used are conducted in an objective, fair, and consistent manner.
