Description of Requirement The Contractor shall furnish all labor, equipment, supervision, transportation, supplies (except those designated as Government-furnished), and incidentals, to perform all work necessary for Janitorial Services at the Chiloquin Ranger District, Fremont-Winema National Forest, in compliance with the terms, conditions and specifications and provisions of this solicitation.

Delivery Date and Delivery Location:

Location: The project is located on the U.S. Forest Service Administrative site, Chiloquin Ranger Station, Chiloquin Oregon 97624.

Description: The U.S. Forest Administrative site, Chiloquin Ranger Station, 38500 Highway 97 North, Chiloquin, OR 97624.

Base Year = Award thru April 30, 2023 Option Year 1 = May 1, 2023 thru April 30, 2024 Option Year 2 = May 1, 2024 thru April 30, 2025 Option Year 3 = May 1, 2025 thru April 30, 2026 Option Year 4 = May 1, 2026 thru April 30, 2027 The Chiloquin Administrative Office building is a two-story structure with 5 entrances. There are 4 restrooms at the Administrative Office. The building area is est.7,786 square feet and is occupied year-round.

The Chiloquin Exercise Room is a one-story structure with 2 entrances. There are 2 restrooms and one shower. This area is an estimated 50 square feet and is occupied year-round. Only clean restrooms, shower and remove all trash in trash containers.

The Chiloquin Conference Room is a one-story structure with 2 entrances and 1 restroom. This area is an estimated 4,400 square feet and is occupied year-round.

GUIDELINES

1. Glass/mirror surfaces and coverings shall be clean and free of obvious dirt, dust, streaks, and smudges.

2. Furniture and all surfaces less than 70 inches high shall be free of obvious dust and dirt. Items on desks or other work areas are not to be disturbed.

3. All carpets shall be clean and free of dust, dirt, debris and obvious spots and stains. Carpet spotting shall be accomplished as needed. All exposed carpet shall be shampooed twice a year with a one-week notification prior to starting. A bonnet type shampoo cleaner, hot water extraction or other acceptable method of carpet cleaning shall be used.

4. Resilient flooring shall be maintained at a high level of luster, free of all types of marks.

5. All other flooring shall be free of dirt, debris and foreign material and maintain a high level of luster (buffing, stripping, and waxing are included).

6. Drinking fountains shall be cleaned and free of watermarks and other debris or encrustation. Drinking fountains shall be maintained at a high level of sanitation.

7. All trash shall be collected from the administrative office building and Conference Room and shall be removed to the designated on-site location.

8. Restrooms shall be cleaned and disinfected to maintain fixtures at a high level of shine and cleanliness.

9. Floors shall be sanitized and maintained to ensure that there is not a buildup of dirt, debris, or bacteria. Partitions and walls shall be free of obvious dirt, graffiti, dust, or other extraneous matter. Paper products shall be replenished as needed to always maintain an adequate supply.

10. Food service, break areas and hallways shall be cleaned.

11. Railings, risers, ledges, grills, and other surrounding areas shall be free of obvious dirt, dust, stains, debris, and other extraneous matter.

12. Venetian blinds shall have cords and both sides of blind slats free of obvious dust, dirt grime and water spots.

13. Walls and high cleaning surfaces above 70 inches shall be free of obvious dust, cobwebs, or any other extraneous matter (interior only).

14. All other space not specifically mentioned and covered by this contract shall be free of obvious dirt, debris, dust, and present an overall appearance of cleanliness.

15. Window and glass (includes interior windows in reception area, lunchroom, hallways, and exterior entrance door glass) shall be clean, free of obvious dirt, grime, streaks, cloudiness, and water spots. Surrounding areas shall be free of standing water, drips, or watermarks.

16. All Interior/Exterior windows shall be cleaned twice a year (May/November), with a one-week notification prior to starting, except the Reception Area shall follow 3.2 Services to be Performed.

17. All entranceways (7) into the offices shall be kept clean including the ceiling over the doorways and the inside the cement wall.

Schedule Workdays and Hours: When required, cleaning shall commence Tuesday, Thursday, and Saturday after the hours of 4:30 p.m. and to be completed by 6:00 a.m. the next day. No Work shall be performed between 6:00 a.m. and 4:30 p.m. on normal Federal workdays. The Contractor may work on weekends and Federal holidays without restrictions to hours provided advance arrangement with the COR is made.

SERVICES TO BE PERFORMED

The following services shall be performed three times a week (Tuesday, Thursday, and Saturday) year- round for ALL buildings.

a. Sweep or vacuum entire floor, stairs, and rugs.

b. Wet mop all non-carpeted areas.

c. Empty all waste containers.

d. Empty all recycle containers.

e. Perform a thorough cleaning of all restrooms in the Administrative Office, Conference room, and Exercise Building. Disinfecting all toilets, sinks, countertops, floors, and mirrors assuring that all surfaces are clean and sanitized. Check and replace deodorizer in urinals as needed.

f. Refill toilet paper, hand towel and soap dispensers, always keeping them at least half full.

g. Clean sink, floor, tables, and countertops of break room. (Administrative Office)

h. Clean exterior entranceways, sweep main walkways, remove cobwebs from light fixtures/doorways and remove litter.

i. Clean stainless steel drinking fountains. (Administrative Office, Conference Room, and Exercise Building)

j. Clean and disinfect receptionist area including front desk counter and furniture. Clean inside of 1st floor windows and both sides of glass doors in reception area and foyer. Clean glass windows and doors in the Conference Room and Workout Room.

k. Stack the chairs on conference tables once cleaned, so floor can be thoroughly cleaned or vacuumed.

l. Wash handrails to remove accumulated grime.

The following services shall be performed once weekly (Saturday):

a. Empty entrance garbage can.

b. Clean and restock bathroom.

The following services shall be performed once each month:

a. Spot clean carpets, including stairway landings.

b. Perform a thorough cleaning of the shower facility in the Exercise rooms. Disinfect shower, bathroom fixtures and floor.

c. Clean fingerprint marks and other spots from doors, interior windows, restroom stalls, walls, and hallways.

d. Dust all furniture surfaces including tables, file cabinets, windowsills, telephone counters, ledges, bookshelves, and chairs.

e. Remove accumulated dust and cobwebs from the walls and light fixtures.

f. Dust walls, woodwork, high ledges, and light fixtures ensuring that cobwebs are removed from corners, light fixtures, window casings and doorways.

g. Wipe and clean mop floorboards, doorframes, and windowsills.

The following services shall be performed once every six months:

a. Vacuum all offices with hose crevice tool around electrical cords, telephone cables, computer tables, and in corners to remove lint, dust, and other debris, which has not been removed through routine dusting.

b. Light fixtures and air supply and return ducts in hallways, restrooms and offices shall be vacuumed and washed

c. Clean inside and outside of all windows in May and November.

d. A thorough shampooing and deep stain removal shall be performed in May and November.

GOVERNMENT-FURNISHED PROPERTY

The Government will furnish secure areas for storage of Contractor-furnished materials. The Contractor shall maintain the storage areas in an orderly manner. The Government will also furnish the following:

a. Floor mats for exterior entrances and doorways.

b. Dumpsters for both trash and recyclable materials.

The Contractor shall ensure that the building presents an overall appearance of cleanliness. The Contractor shall provide sufficient supplies, materials, and equipment to adequately maintain the premises at an acceptable level of cleaning that is consistent with comparable industry standards for this type of contract. The following guidelines describe what is considered to be the normal industry quality and performance standards for this type of contract.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

452.204-70 Modification for Contract Closeout (OCT 2021) Upon contract closeout for contracts utilizing Simplified Acquisition Procedures (SAP) according to FAR 13:

(a) If unobligated funds in the amount of $1000 or less remain on the contract, the Contracting Officer (CO) shall issue a unilateral modification for deobligation. The contractor will receive a copy of the modification but will not be required to provide a signature. The CO shall immediately proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

(b) If unobligated funds of more than $1000 remain on the contract, the CO shall issue a bilateral modification for deobligation. The contractor will receive a copy of the modification and will be required to provide a signature. (The CO may also request a Release of Claims be completed by the contractor, although not required for contract and orders using SAP procedures.) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Provide a listing of your projects for the past three-year period that most closely relate to this type of work. Include the dates of performance, the company name, the name of contact person, and their telephone number. References will be contacted.

1. QUALITY CONTROL PLAN AND PROPOSED WORK SCHEDULE

Offerors shall submit their Quality Control Plan and proposed work schedule (days and times for weekly and monthly work) showing how they intend to meet Government performance standards for each task and subtask associated with this contract.

The Quality Control Plan (QCP) should recognize the responsibility of the Contractor to carry out its quality control obligations and contain measurable inspection and acceptance criteria corresponding to the Statement of work (SOW). The QCP should focus on the level of performance required by the SOW, rather than the methodology used by the contractor to achieve that level of performance.

2. USE OF BIOBASED PRODUCTS

As required by FAR 52.223-2, biobased products must be used in performance of this contract. Contractor shall describe current or past performance experience with the use of biobased products. For the purposes of this contract, the Contractor shall include a list of products intended for use on the Chiloquin Ranger District Janitorial. Identify each product’s biobased content percentage and the product’s intended use.

b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. 52.204-24 Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment (NOV 2021) The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in paragraph (c)(1) in the provision at 52.204-26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products or Commercial Services. (a) Definitions. As used in this provision— Backhaul, covered telecommunications equipment or services, critical technology, interconnection arrangements, reasonable inquiry, roaming, and substantial or essential component have the meanings provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

(b) Prohibition. (1) Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Nothing in the prohibition shall be construed to—

(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or

(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.

(2) Section 889(a)(1)(B) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2020, from entering into a contract or extending or renewing a contract with an entity that uses any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. This prohibition applies to the use of covered telecommunications equipment or services, regardless of whether that use is in performance of work under a Federal contract. Nothing in the prohibition shall be construed to—

(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or

(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.

(c) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for "covered telecommunications equipment or services".

(d) Representation. The Offeror represents that—

(1) It ☐ will, ☐ will not provide covered telecommunications equipment or services to the Government in the performance of any contract, subcontract or other contractual instrument resulting from this solicitation. The Offeror shall provide the additional disclosure information required at paragraph (e)(1) of this section if the Offeror responds "will" in paragraph (d)(1) of this section; and

(2) After conducting a reasonable inquiry, for purposes of this representation, the Offeror represents that— It ☐ does, ☐ does not use covered telecommunications equipment or services, or use any equipment, system, or service that uses covered telecommunications equipment or services. (e) Disclosures. (1) Disclosure for the representation in paragraph (d)(1) of this provision. If the Offeror has responded "will" in the representation in paragraph (d)(1) of this provision, the Offeror shall provide the following information as part of the offer:

(i) For covered equipment—

(A) The entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the original equipment manufacturer (OEM) or a distributor, if known);

(B) A description of all covered telecommunications equipment offered (include brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable); and

(C) Explanation of the proposed use of covered telecommunications equipment and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.

(ii) For covered services—

(A) If the service is related to item maintenance: A description of all covered telecommunications services offered (include on the item being maintained: Brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable); or

(B) If not associated with maintenance, the Product Service Code (PSC) of the service being provided; and explanation of the proposed use of covered telecommunications services and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.

(2) Disclosure for the representation in paragraph (d)(2) of this provision. If the Offeror has responded "does" in the representation in paragraph (d)(2) of this provision, the Offeror shall provide the following information as part of the offer:

(i) For covered equipment—

(A) The entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the OEM or a distributor, if known);

452.204-71 Personal Identity Verification of Contractor Employees (Oct 2021)

(a) The contractor shall comply with the personal identity verification (PIV) policies and procedures established by the Department of Agriculture (USDA) Directives 4620-002 series.

(b) Should the results of the PIV process require the exclusion of a contractor's employee; the Contracting Officer will notify the contractor in writing.

(c) The contractor must appoint a representative to manage compliance with the PIV policies established by the USDA Directives 4620-002 series and to maintain a list of employees eligible for a USDA LincPass required for performance of the work.

(d) The responsibility of maintaining a sufficient workforce remains with the contractor. Contractor employees may be barred by the Government from performance of work should they be found ineligible or to have lost eligibility for a USDA LincPass. Failure to maintain a sufficient workforce of employees eligible for a USDA LincPass may be grounds for termination of the contract.

(e) The contractor shall insert this clause in all subcontracts when the subcontractor is required to have routine unaccompanied physical access to a Federally-controlled facility and/or routine unaccompanied access to a Federally-controlled information system.

(f) The PIV Sponsor for this contract is a designated program point of contact, which in most cases is the COR, unless otherwise specified in this contract.
