1. Blocks 7, 14:

a. The contractor shall submit the DD Form 250 (Material Inspection and

Receiving Report) electronically via the Internet to iRAPT (WAWF) website address: https://wawf.eb.mil using DODAAC FA8251. MARK FOR: Address in block 14a.

b. The contractor shall send the Contractors Counterfeit Protection Plan (CPP) and notice of posting to (WAWF) via e-mail to the following e-mail address: javad.refahiyat@us.af.mil

2. Blocks 8, 10, 11, 12, 13: The contractor shall deliver the Counterfeit prevention Plan (CPP) NLT thirty (30) calendar days after contract award. The Government comments will be provided to the contractor within fourteen (14) calendar days after receipt. The contractor shall resubmit the CPP NLT seven (7) calendar days after receipt of Government comments.

3. Block 9: The Distribution Statement, Export Control Warning and Destruction Notice shall be placed on the first sheet of each document and be located in a conspicuous location.

a. DISTRIBUTION STATEMENT "D": Distribution authorized to the Department of Defense and U.S. DoD contractors only (Administration or Operational Use). (July 2019). Other requests for this document shall be referred to (416 SCMS/GUMAD, Hill AFB, UT 84056-5826).

c. The contractor shall submit a Letter of Transmittal (LT) in the month the delivery order is completed, covering all reports transmitted for that order. The Letter of Transmittal (LT) shall be submitted via e-mail to the following e-mail address: 416SCMS.CDRL.Workflow@us.af.mil

2. Block 10: The Contractor reporting requirements to CAV AF shall be accomplished daily and/or within one (1) business day of a maintenance action IAW Contractor Users Guide.

3. Block 11: Data shall be current within one (1) business day of the last End Item maintenance occurrence and reflect an accurate representation/status of all assets at a contractor for repair.

4. Block 12: Date of first submission shall be within one (1) business day following a maintenance transaction after the contract award date and Government training as specified IAW CAV AF Users Guide.

5. Block 13: Date of subsequent submittal(s) shall be within one (1) business day of each End Item maintenance action/occurrence.

c. The Final Report submitted following the final reporting period on the contract shall be annotated "Final Report".

a. The contractor shall submit DD Form 250 (Material Inspection and Receiving Report) electronically via the Internet to iRAPT (WAWF) website address: https://wawf.eb.mil using DODAAC FA8251. MARK FOR: Address in block 14a.

b. DCMA inspector/acceptor may request a hard copy at the contractor's facility during the time of inspection and acceptance.

3. Blocks 8, 10, 11, 12, 13: First submission of the report due NLT ninety (90) calendar days after contract award and monthly thereafter. DCMA will review and provide comments in coordination with the F-16 AVIONICS Program Office NLT seven (7) calendar days following receipt. Final report is due NLT seven (7) business days after receipt and incorporation of DCMA comments.

4. Block 9: The Distribution Statement, Export Control Warning and Destruction Notice shall be placed on the first sheet of each document and be located in a conspicuous location.

a. DISTRIBUTION STATEMENT "D": Distribution authorized to the Department of Defense and U.S. DoD contractors only (Administration or Operational Use). (FEB 2019). Other requests for this document shall be referred to (416 SCMS/GUMAD, Hill AFB, UT 84056-5826).
