Project: Cyclical Carpet Replacement Chambers 5050 Clarkson S. Fisher U.S. Courthouse

Requirements:

o This is a request for quotation (RFQ) for services in connection with the U.S. District

Court, District of New Jersey cyclical carpet replacement in the Clarkson S. Fisher U.S.

Courthouse (chambers 5050).

o This is a request for open market labor pricing. Carpet can be ordered under Federal

Supply Schedule contract # GS-03F014AA.

o All contractors must agree to the terms and conditions set below.

o A fixed price award from this RFQ will be made based on the lowest priced, technically acceptable offer.

o The chamber is approximately 1900 square feet. Square footage is approximate and field measurements are required. Please contact Amber Barcliff

(amber_barcliff@njd.uscourts.gov) to schedule a site visit. Changes cannot be made after the contract is awarded.

o The contractor must be willing to start work based on a Purchase Order. No down payments or advances will be allowed.

o All work requires escorted access to judiciary facilities. In accordance with Clause 3-3, Contractor employees are subject to security checks including background investigations. If the contractor is unable to supply enough acceptable employees to work on this project, The judiciary may terminate the contract by default.

o Contractor must submit the names, dates of birth, and social security numbers of all onsite workers at least one week prior to the start of the work.

o All quotes shall include estimated times for completion and number of workers needed to accomplish the work.

o Proposal must include the total cost to complete the scope of work in accordance with this RFQ. No additional charges will be accepted.

o Contractors will be responsible for any damage to the building, furnishing, or carpet during the course of the work being performed.

o Award of contract and acceptance of work performed will be dependent on approval from the General Services Administration and the court. Please note, GSA requires a

Certificate of Liability Insurance naming GSA as the beneficiary.

All proposals should detail how the work will be accomplished and acknowledge compliance with the RFQ. Statement of Work:

The scope of work for the RFQ includes all labor and materials including supervision, tools, equipment, transportation, and incidentals. No equipment, materials, or services of any kind shall be provided by the court. Furniture moves will be arranged by the court and should not be included in the quote.

o The contractor will remove the old carpet, padding, and cove base where necessary, clean the floor in preparation for new carpet, and install the new carpet in accordance with the manufacturer’s recommendations ensuring patterns are aligned and seams are tight.

o The contractor will order, receive, and store the new carpet and cove base until installation.

o The carpet selection for the entire chambers is Manufacturer: Bentley Style: Milano KR410 W/KR302 (Bentley GSA contract # GS-03F-014AA) – sample to be provided upon request.

o The cove base selection is TBD.

o Any chemicals used during the carpet installation must be low odor and non-toxic.

o If requested, the contractor shall be prepared to provide the Material Safety Data for all chemicals proposed to be furnished as a result of this bid.

o The court freight elevator is sensitive to excessive weight, movements, and length of doors being open. Crew must be careful when using the freight elevator to transport carpet and equipment. In the event the elevator shuts down, the crew must be able to carry the carpet up the stairs.

o The contractor will work at the court’s convenience and the work will be scheduled accordingly.

o The contractor will clean up and remove any debris or trash associated with the installation including left over carpet. All waste will be disposed offsite.

mailto:amber_barcliff@njd.uscourts.gov o The Contractor will perform thorough vacuuming of the area after the carpet installation is complete, so the area is ready for furniture installation.

o All additional work necessary to complete cyclical carpet removal and installation and to meet industry and manufacturers standards and to the satisfaction and approval of the General Services Administration and the Court; and all additional work necessary to meet all local building, fire and safety codes, and the Terms and Conditions set below.

If this contract is covered by the EO, the contractor must provide employees with 1 hour of paid sick leave for every 30 hours they work, up to 56 hours of paid sick leave each year.

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).

Union Rate Identifiers

A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:

PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the https://sam.gov/wage-determination/NJ20220035/6 12/14 most current negotiated rate, which in this example is July 1, 2014.

Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers

Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

Survey wage rates are not updated and remain in effect until a new survey is conducted.

Union Average Rate Identifiers

Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.

A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.

WAGE DETERMINATION APPEALS PROCESS

1.) Has there been an initial decision in the matter? This can be:

* an existing published wage determination

* a survey underlying a wage determination

* a Wage and Hour Division letter setting forth a position on a wage determination matter

* a conformance (additional classification and rate) ruling

On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour National Office because National Office has responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.) and 3.) should be followed.
