B.4 STATEMENT OF WORK

Background:

Justification for the continuing purchase of rapid molecular testing system reagents/consumables and service and maintenance on bulk funded contract.

In July 2018, VAPORHCS purchased two pieces of molecular equipment from 3rd party leasing agency that had been in use at VAPORHCS for several years prior and is in-use currently. This purchase allowed freedom from restrictive monthly minimum reagent shipments due to leasing contract. All reagents and consumables are on customized based on GSA/FSS contract and do not change based on purchase volume. The continuing use of this now government owned equipment is possible, as long as a contract exists allowing continual supply of:

1. Reagents/consumables

2. Service for continued maintenance of equipment

Reagents/consumables:

Cepheid Inc. has exclusive rights for manufacturing and distributing the GeneXpert System®, GeneXpert cartridges and collection devices, for use with the Cepheid Invitro Diagnostic Xpert system for the performance of currently used testing: SARS-CoV-2 and SARS-CoV2/Flu/RSV kits, Xpress FLU, CT/NG, C.diff, Carba- VAPORHCS ownership of the equipment, matching VAPORHCS usage and needs, we request this acquisition be sole sourced from Cepheid (see sole source document).

Maintenance/Service To maintain the integrity of testing and meet the required turn-around-times needed by providers, a robust system must include a service contract and maintenance contract.

Instruments are continually in use and must remain operational 24/7 365 days a year. Only certified FSE personnel may work on the equipment, scheduled through the vendor. Parts are only available through the vendor.

Service Requirements:

Calibration and calibration confirmation by at least annual PM is required by the manufacturer to continually test patient specimens. This is now performed by FSE from the vendor that must come to the lab to perform this service.

Parts, labor, and travel coverage

Software upgrades included

24/7 365 days a year phone support Onsite for troubleshooting within 2 days on the weekdays

Advantages:

The GeneXpert System has the following advantages and features only found in this offering that will continue to impact our clinical practice:

Current national contract for easy receipt and use of SARS-CoV-2 and SARS- CoV2/Flu/RSV kits It is a fully integrated real-time PCR system that integrates a complete on-board nucleic acid purification system with amplification and detection in one self-contained system.

Moderately Complex PCR tests availability allows for any technologist to operate the system.

6 color excitation and detection for multiplexing; provides thermal and optical data in real time. This increases the number of targets and controls available in tests design and implementation.

Closed system eliminates contamination and does not require special operating environments normally required for PCR activity. This generation GeneXpert provides an even higher degree of enclosure for both active and completed cartridges.

- rapid, easy to perform sample loading in less than 1 minute.

Utilizes proprietary dried reagent technology for ambient temp stability, precision of microfluidics-based reconstitution, and simplification of automated filling.

Automated data reduction and quantitative results interpretation in one package.

Only FDA-approved system available to offer rule-out of Mycobacterium tuberculosis for removal of patient from isolation and contact precautions, including Rifampin resistance call Only FDA-approved system for rapid confirmation or rule-out of carbapenem resistance markers (Carba-R).

Performance Monitoring The microbiology laboratory will monitor the system for minimum performance. In the event performance is compromised by either reagent or instrument failures the company (Cepheid) will be contacted for remediation.

Security Requirements None. All samples supplied to the instrument will have no patient or VA identifying information.

All samples are blinded upon entry.
