About this file
This document contains procurement terms and conditions established by the State of Ohio, Bureau of Workers' Compensation (BWC), effective October 2025, applicable to state and local contract opportunities. The document does not specify particular products, services, quantities, response dates, due dates, site visits, bidders' meetings, award dates, or contract terms. Rather, it outlines the general administrative and compliance requirements that suppliers must meet when contracting with Ohio state agencies for any amount of $5,000 or more.
Key compliance requirements include submission of an Affirmative Action Program Verification Form to the Ohio Department of Development, completion of a Supplier Information Form and IRS Form W-9 for new suppliers through the Ohio Supplier Portal, and adherence to non-discrimination and equal employment opportunity laws under Ohio Revised Code Section 125.111. Suppliers must also complete a Contractor/Subcontractor Affirmation and Disclosure Form confirming compliance with prohibitions on offshore services and services provided outside the United States without a signed waiver, as well as prohibitions on purchases from or investments in Russian institutions or companies. The State encourages suppliers to source goods and services from Minority Business Enterprise (MBE) and Encouraging Diversity, Growth and Equity (EDGE) vendors. A sales and use tax blanket exemption is available upon request. Current suppliers must provide a W-9 form reflecting their registered tax identification number with the State of Ohio.
