About this file
This document outlines procurement terms and conditions for the State of Ohio, administered through the Procurement Bureau of the Department of Administrative Services and applicable to state and local agencies conducting business with Ohio. The document does not specify particular products, services, quantities, response dates, or contract terms, as it serves as a general framework of requirements applicable to state procurement activities rather than a specific solicitation.
Key compliance requirements include submission of an Affirmative Action Program Verification Form for contracts valued at $5,000 or more, completion of a Supplier Information Form and IRS Form W-9 for new state suppliers, and adherence to non-discrimination and equal employment opportunity standards under Ohio Revised Code Section 125.111. Additionally, suppliers must complete and submit a Contractor/Subcontractor Affirmation and Disclosure Form affirming compliance with the prohibition on expenditure of public funds for offshore services and services performed outside the United States without a duly signed waiver, as well as prohibiting purchases from or investments in Russian institutions or companies. The State maintains the right to recover any funds paid in violation of these provisions. New suppliers must register through the Ohio Supplier Portal, while current state suppliers must provide updated W-9 forms. A Sales and Use Tax Blanket Exemption is available upon request, and the State encourages suppliers to purchase from Minority Business Enterprise (MBE) and Encouraging Diversity, Growth and Equity (EDGE) vendors.
