About this file

This is a Cost Proposal document for the Ohio Bureau of Workers' Compensation (BWC) for an Audit Assignment and Tracking Application RFP (Request for Proposal) with solicitation number SRC0000032348. The document outlines a comprehensive cost proposal template for potential vendors, covering project deliverables across multiple phases including project planning, testing, training, and implementation. The proposal requires vendors to provide detailed pricing for project milestones, with specific cost breakdowns for deliverables such as project plans, testing plans and reports, training plans and completion reports, and implementation plans and reports.

The cost proposal includes sections for subscription, software licensing, maintenance, and support costs projected through state fiscal year 2037, with specific instructions for pricing based on either 30 concurrent users or 70 named users. The document mandates vendors complete a detailed rate card listing hourly rates for various project resources including management, change management, technical specialists, and support roles. Additionally, the proposal allows vendors to include value-added services and best practices, though these will not be evaluated in the cost proposal scoring. The template requires precise completion without reformatting, with a warning that any modifications may result in proposal rejection.
