VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

DEPARTMENT OF VETERANS AFFAIRS

Justification for Brand-Name Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity: Department of Veterans Affairs, VISN 7, Ralph H. Johnson VA Medical Center 2237 #: 534-22-3-783-0071

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Pathology department has to borrow the Faxitron that is currently available for use from the Radiology department when looking for staples and other metal objects within surgically removed tissues. The Pathology department is looking to purchase a Faxitron Path Specimen Radiography system for use internally within the department.

3. Unique characteristics that limit availability to only one source, with the reason no other manufacturers product can be used:

Hologic provides the Faxitron Path Specimen Radiography system that helps to provide optimal imaging for both large or thin sliced tissue specimens which helps to reduce cutting of the tissue. This radiography system is designed with field of view guides and automatic position detection, while also providing up to 6 X geometric magnification which can locate the smallest calcifications or metal objects in tissue samples to ensure accurate diagnoses. It has a large field of view which accommodates a broad range of specimens and offers fast reporting. It has a one button Automatic Exposure Control that aids ideal image exposure and clarity from breast tissue specimens and intact mastectomies to bones and other tissues. The specific model in question is ideal for Pathology departments.

4. Description of market research conducted and results or statement why it was not conducted:

The end users of the Faxitron were consulted for information and the determination was made that this was the best system available to meet the needs of the Pathology department. Prior use from the Radiology department has familiarized staff with this product making it the best item for purchase.

Also, since we already have the requested model on site, the Faxitron will provide continuity of operations as well as maintenance/repairs. Other products were surveyed to meet the need but the Hologic instrument will be able to meet the Pathologies needs without the retraining of staff thus ensuring continuity of patient care.

5. Contracting Officer's Certification: A Brand-Name purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
