About this file

This is an Indian Economic Enterprise (IEE) Representation Form from the Indian Health Service's Office of Management Services, Division of Acquisition Policy. The form is used to self-certify that an offeror meets the definition of an "Indian Economic Enterprise" under the Buy Indian Act (25 U.S.C. 47) when responding to solicitations, sources sought notices, or RFIs.

The form requires businesses to maintain IEE eligibility throughout three key periods: when making an offer, at contract award, and during the full contract performance period. Companies must certify they meet the 51% Indian ownership requirement and provide details including the name of the 51% owner, federally recognized tribal entity, business name, and UEI SAM number. The form includes warnings about penalties for false claims and requires registration in the System for Award Management (SAM). This specific form is associated with solicitation SS-BFSU-25-013 for Laundry & Dry Cleaning Services at Indian Health Service facilities.
