STATEMENT OF WORK

Purchase, installation and disposal of Emergency Lighting, Exit Signs and Wired guard

INTRODUCTION:

San Simon School is a K-12 education system which operates on a multiple building campus located in Sells, Arizona. San Simon School is responsible for maintaining a safe, clean, and ADA compliant facility for staff, students and public.

SCOPE OF WORK:

1. Contractor to remove and replace all Emergency Lighting unit., guards to be installed where as needed. To all seven Buildings

2. Contractor to remove and Replace all Exit Signs.

3. Contractor will all lighting units with LED type of system

Both Emergency Lighting.

4. Contractor will be responsible for disposal of all removed components. Including the old used batteries from the old Emergency light units.

5. This will be in accordance with all codes.

Hours of Operation San Simon School hours of operation are from 7:30 A.M. through 4:00 P.M. Monday through Friday. It will be required that the Contractor schedule their working hours during non-operational hours of the San Simon School, evening or weekends.

Federal Holidays Do not schedule work on Federal Holidays or days observed in lieu thereof. The Federal holidays listed below are observed. If a holiday falls on a scheduled serviced day, the Contractor will be responsible for rescheduling services.

PROJECT OVERVIEW

Remove and Replace all School Wide, (Emergency Lighting Units, Replacement of all school wide Exit sign units).

SAFETY

Contractor shall implement an appropriate safety control program applicable to the work being performed. A Fall Protection Plan shall be instituted if workers are to be performing duties where their feet are greater than six (6') feet from the ground or finished floor. All personnel on roadway projects shall wear appropriate Personal Protective Equipment (PPE), ie: hard hat, safety glasses, steel toed work boots, hearing protection and safety vest. Contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents and injuries to individual's, both contractor and general public, in and or around the work area. GOVERNMENT PROVIDED

MATERIALS

CONTRACT ADMINISTRATION:

A. The contractor shall not perform work that is outside the SOW unless directed in writing by the Contracting Office (CO). Any work done by the Contractor outside the SOW without direction from the Contracting Officer will be at the Contractor’s own risk.

B. The Contractor shall accept directions only from the Contracting Officer or the duly authorized Contracting Officer's Representative (COR). Requests or desires of the Users or Using Agency made directly to the Contractor will be immediately brought to the attention of the COR. Any changes to the project scope or other provisions of this Statement of Work (SOW) must be authorized in writing by the CO.

C. Contractor shall notify Lee Ivey at (520) 362-2231 in scheduling an initial site visit and scheduling work. Mr. Ivey shall ensure that a School Maintenance Worker is present to accompany the contractor in accessing the campus.

D. Contracting Officer’s Representative (COR) for the San Simon School shall be designated for the administration of this contract. The COR shall coordinate schedule for each phrase, approve technical submittals, inspect work, monitor progress, accept completed work, review invoices, review change orders, and issue/receive Government Furnished Items. The Agency COR has the authority to direct the Contractor to stop performance of work if unsafe work conditions are observed.

E. Completed work shall be approved and signed off by the COR after the following are completed:

• Final inspection performed by the COR/Maintenance Personnel and Contractor

• Training of local maintenance staff if needed.

• Warranty and operating manual are provided to the COR or Maintenance Personnel.

F. Any unsafe work practices shall be reported to the COR for immediate reconciliation.

DELIVERABLE SCHEDULE

A. The Contractor shall commence work under this contract promptly, execute the work diligently, and achieve final completion and acceptance including final cleanup of the premises within the contract period specified.

SCHEDULE AND PAYMENT

Schedule Work will commence within thirty (10) days of issuance of Award Notification.

All deliveries will be provided and within thirty (30) days of start of work.

Period-of-Performance Projection date of completion is 40 days after award.

However, Period of Performance shall not exceed December 31, 2024, for unexpected supply chain issues.
