Psychological Therapy and Compensatory Services San Felipe Elementary School
I. STATEMENT OF WORK
The contract will provide all necessary certified and/or professional licensed psychologist(s) to provide school based mental health services and evaluations within a comprehensive educational program. In addition, the contractor will provide all necessary supplies, materials, equipment and testing materials to perform the work as described below.
II. WORK REQUIREMENTS
After being provided a copy of the parental permission form for assessment by the school, the contractor shall:
A. Provide psychological services directly to students identified as eligible to receive such services and as provided in the goals of the IEP.
B. Identify severely emotionally disturbed children.
C. Provide psychotherapy to emotionally disturbed children and students identified as eligible for psychological services.
D. Conduct psychological and other evaluations to children following all laws regarding these.
E. The Psychologist will provide psychological services to any student receiving Special Education services at San Felipe Pueblo Elementary School, not just those students who fit the eligibility category of emotionally disturbed.
F. Provide consultation to school personnel.
G. Provide consultation and reinforcement of parenting skills to parents and families.
H. Coordinate all referrals for mental health services. Facilitate, as needed, contacts with mental health providers outside the school.
I. Attend all IEP meetings for identified children.
J. If required, provide training and education to school staff.
K. Maintain records of work performed for monthly invoicing.
a. The contractor will be expected to sign in and keep a log regarding the actual duration of therapy session of individual or group nature.
b. The contractor will be paid for therapy services monthly. Invoices shall be submitted by the contractor directly to the contracting office and school serviced.
L. Compensatory time will be needed only if services not met in a timely manner. No compensatory time is needed at the start of contract.
M. The contractor is also required to log in and out of the school site and to schedule herself/himself for each individual child’s IEP committee meeting at the school’s request. Weekly schedules shall be approved by the school’s special education coordinator.
N. Replacement of key personnel will require prior coordination and approval of the school.
III. REPORTS
All evaluation reports shall contain information required to remain in compliance with all pertinent regulations mandated by Federal or State Laws including IDEA and Educational Standards for the state of New Mexico regarding Special Education Program Students. The reports may be hand delivered or mailed with the original protocols to the school special education coordinator.
A. The contractor will be expected to submit a brief progress report each four weeks for each student being seen for therapy and or compensatory purposes, outlining any progress that has been made regarding goals that have been established relative to motor therapy in the classroom or within the school environment, following consultation with the special education classroom teacher and/or the regular program classroom teacher.
B. The contractor will be expected to meet with the parents of students and/or guardian of students placed for therapy purposes, to provide for consultation during which time the contractor will be available to meet with close family member to discuss those issues, problems or concerns which are pertinent to or impact on the progress of the student within the school environment. Such session will occur as time scheduling allows so that such session do not reduce the therapy time of students placed to receive therapy. These meetings will be scheduled based on the willingness of family members to participate voluntarily and with consultation with the contractor.
IV. EVALUATION REPORT OF PROGRAM EVALUATION:
All reports should contain all necessary information required to remain in compliance with any and all pertinent regulations mandated by Federal or State Laws including IDEA and the Indian School Equalization Program’s guidelines and criteria. In addition, the completion of Psychological evaluation, the contractor shall:
A. The contractor shall be present at IEP/multidisciplinary placement team meetings to develop individual goals and objectives for each student to whom therapy is provided. The goals and objectives must be a part of each student’s IEP, B. Meet the students on an individual basis for the purpose of providing therapy, and to determine the effectiveness of individual program goals and objectives and/or student progress in regard to his/her adaption to the educational environment, potential for integration into the regular program, and ability to cope outside a structured environment.
V. CONFIDENTIALITY
All materials furnished or produced by the contractor under this agreement, including but not limited, to documents, reports, and correspondence will remain CONFIDENTIAL. The contractor shall not release information to any persons, group of persons, firm or organizations other than the Bureau of Indian Education, San Felipe Pueblo Elementary School with information related to work performed, research or information derived from work performed under this agreement.
Breach of confidentiality shall be denied substantial breach of this agreement and may be grounds for rescission.
VI. COST SCHEDULE
Service Fee is an All-inclusive mileage, meetings, etc. conducted on the same day. Hours, On-Site Visit per year and Evaluation Reports are estimated.
Virtual Service is strictly at the discretion of school, not an option to circumvent on-site services.
Scheduling will be determined by the Business Technician based on weather conditions, school testing periods, school-based activities, as well as other unforeseen circumstances.

IX. KEY PERSONNEL REQUIREMENTS:
The contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote and Replacement of key personnel will require prior coordination and approval of the school.
Education: Degree in related field from an accredited program License(s)/Certification(s) Possess a valid NM School Psychologist (PRE K - 12) License
Define “degree in related field” is in reference to psychologist(s).
X. SECURITY REQUIREMENT:
All contracting candidates are required to complete BIE background check before on-site services are permitted.
Note to Awardee: All contractor staff must adhere to the criteria, hours, descriptions of work set forth in this SOW.
No deviations from the SOW are permitted without written direction from the Contracting Officer.
