STATEMENT OF WORK (SOW)

Special Education Service for

Occupational Therapy, Ojibwa Indian School

1. BACKGROUND

This requirement falls under the Individuals with Disabilities Education Act (IDEA), 34 Code of Federal Regulations § 300.323. IDEA requires a free appropriate public education to eligible children with disabilities throughout the nation and ensures special education and related services to those children.

2. SCOPE

The contractor shall provide all personnel, necessary supplies, materials, equipment, and testing materials to perform occupational therapy services. The Contractor shall provide:

- Screenings and formal evaluations

- Training for teachers, when requested

- Occupational therapy evaluations for all students referred for evaluation

- Participation during the initial placement or multidisciplinary team committee meeting for students evaluated by the contractor

- Advisement to the special education and classroom teacher in matters regarding continuous assessment, evaluation, and implementation of motor therapy programs for individual students and will meet with individual students regularly to provide suggestions for carryover of therapy strategies into home when necessary

After being provided a copy of the parental permission form for assessment by the school, the contractor shall:

- Provide diagnostic services and reports for students who are identified with a disability and require occupational therapy services

- Provide consultation to school staff regarding accommodations and modifications needed for students eligible for occupational therapy services

- Assist the school in consulting with or obtaining services from medical providers

- Participate in Individualized Education Program (IEP) and multidisciplinary meetings to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop individual education and intervention plans to achieve IEP goals.

- Participate in multidisciplinary meetings as required

- Provide training to school personnel as requested

- Responsible for the maintenance of equipment such as wheelchairs, crutches, and adaptive equipment

- Provides school occupational therapy as mandated on IEP

- Provide training for parents as required

- Consult with other service providers to develop a cohesive service plan; may provide cotreatment

- Meet with the parents and/or guardians of students placed for therapy purposes to provide for consultation. During this time, the contractor will be available to meet with close family members to discuss issues, problems, or concerns pertinent to or impacting the student's progress within the school environment.

Services shall be provided twice a week for a total of 12 hours per week for 46 weeks throughout the school year and summer school, if needed. Candidate shall coordinated with the school.

3. KEY PERSONNEL MINIMUM REQUIREMENTS

The Contractor shall provide key personnel who meet or exceed the minimum requirements.

Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote/proposal.

EDUCATION: Occupational Therapy degree from an ACOTE-certified degree program

# OF YEARS OF DIRECTLY RELATED PROFESSIONAL EXPERIENCE: 2 years of experience, preferably with Native American students

CERTIFICATION(S): National Board for Certification in Occupational Therapy (OTR)

LICENSE(S): State license (License in state where services are to be provided)

All Contracting candidates are required to complete background check before on-site services.

Replacement of key personnel will require prior coordination and approval.

Virtual Service:

Strictly at the discretion of school, not an option to circumvent on-site/direct services.

Scheduling will be determined by the Business Technician based on weather conditions, school testing periods, school-based activities, as well as other unforeseen circumstances.

4. DELIVERABLES

The Contractor shall provide the following deliverables within the specified timeframe:

5. PLACE OF PERFORMANCE

The contractor shall provide services at Ojibwa Indian School, located at 9620 42nd Avenue NE, Belcourt, ND 58316. The contractor may bill travel if it is included and funded in the contract.

A Government-authorized representative shall pre-approve incurred travel costs for performance.

Travel costs shall be billed at the current year’s mileage and per diem rates in accordance with the Federal Travel Regulations (FTR).

6. PERIOD OF PERFORMANCE

The period of performance shall begin with a base year with 4 optional years that may be exercised unilaterally by the Contracting Officer if: a. There is an ongoing requirement.

b. Performance is satisfactory;

c. Prices are reasonable based on current market rates; and

d. Funds become available for the option year.

7. SECURITY REQUIREMENTS

Contractor personnel shall present government-issued identification to the front desk and check in with the Special Education Coordinator/School Administrative Office before entering the school campus. Contractors' personnel are subject to background investigations and must successfully pass the background check before performing under the contract.

8. PAST PERFORMANCE ASSESSMENT

The Government will evaluate the Contractor’s performance under this contract which will be shared with other Government agencies and used as past performance references.
