Attachment 5 - Addendum to FAR 52.212-1 – Instructions to Offerors – Commercial items.

General Information
This acquisition will be solicited and awarded in accordance with Federal Acquisition Regulation (FAR) Part 12 -- Acquisition of Commercial Items and FAR Part 13 – Simplified Acquisition Procedures.
This acquisition is being solicited as an 100% Total Small Business Set Aside
Quote Preparation Instructions-General
Quote must include all data and information requested and must be submitted in accordance with these instructions.
Any offeror who submits an incomplete package maybe considered nonresponsive. Quote is expected to conform to the solicitation provisions and be prepared in accordance with this section. To aid in the evaluation, the proposal shall be clearly and concisely written as well as being neat, indexed (crossed-indexed as appropriate) and logically assembled. All pages of each part shall be appropriately numbered and identified with the name of the offeror, the date and the solicitation number.
Delivery
Offeror shall deliver quote via electronic mail to the above stated point of contacts.
Overall Arrangement of Proposal
The overall proposal shall consist of three (3) separate volumes/documents, individually entitled as stated below:

Offer and Other Documents Past Performance and Key Personnel Value Bid Schedule
Volume I – Offer and Other Documents
This volume shall contain the offeror’s contractual documentation. These documents will be reviewed only for compliance with solicitation, FAR, or other statutory requirements and will not be evaluated as part of the source selection factors identified in the Evaluation Section.
This volume consists of the actual offeror to enter into a contract to perform the desired work. It also includes required representations, certifications, and acknowledgements, justification for noncompetitive proposed subcontracts, identification of technical data to be withheld, any other administrative information, and a summary of exceptions and deviations taken. Although it incorporates them by reference, it does not physically include the other volumes.

Format and Content
Volume I - Offer and Other Documents, shall include the following documents (in the order listed):
a. The quote form fully executed. The Standard Form 1449 is to be executed fully (to include amendments when applicable) and used as the cover sheet (or first page) of Volume I - Offer and Other Documents. The person signing the quote form must have the authority to commit the offeror to all of the provisions of the proposal, fully recognizing that the Government has the right, by terms of the solicitation, to make an award without further discussion if it so elects.
b. Offeror Representations, Certifications, and Other Statements fully executed as required pursuant FAR 52.212-3 – Offeror Representations and Certifications – Commercial Items. This provision is in the System for Acquisition Management (SAM.gov) where offeror should ensure the information is up-to-date. If this information is SAM.gov then the offeror is not required to provide with their proposal.
c. Exceptions and Deviations taken to the contract and/or solicitation requirement and summary of Exceptions and Deviations taken on other Volumes.
Volume II: Past Performance & Key Personnel
Provide past performance references in accordance with the format contained in Attachment 7. A past performance reference is one contract, one task order, or one work assignment. Past performance Information should be provided for three (3) recent (performed during past three (3) years) contracts as the prime contractor that demonstrates experience and customer satisfaction for work similar to requirements as outlined in the SOW. Please provide a list of Key Personnel who will be directly involved in this project, along with education, experience and years of service.
Volume III: Price Quote
All quotes shall include Attachment 2 – Value Bid Schedule. All quotes shall include Services, Personnel, Labor, equipment, and materials.
QUOTE SUBMISSION CHECKLIST
a. Completed Blocks 17a, 17b (if applicable), 30a, 30b and 30c of the Standard Form 1449.
b. Acknowledged receipt of all solicitation amendments (if applicable).
c. Representations and Certifications: Ensure Representations and Certifications are up to date in the System for Award Management (SAM). If offeror does not have an active SAM Registration then print FAR Clause 52.212-3, complete, and provided with the quote submission.
d. Provided Past Performance References
e. Provided document as required in Attachment 2 – Value Bid Schedule.
f. Read entire FAR Clause 52.212-1, Instructions to Offerors-Commercial and Addendum (Attachment 5) to ensure compliance.
