All work under this contract shall be in performance with all local, state, and federal laws, regulations, and codes.
Only the awarded contractor shall provide a copy of their insurance certificate required by State and Local Governments that has been acquired for the time period covered under this contract to the Contracting Officer within 10 business days after award. No further insurance requirements will be imposed by the Corps of Engineers for this contract.
Contractors are not required to submit an Accident Prevention Plan (APP) with their quote.
The successful offeror will be required to submit this document upon award. The APP must be accepted by the Government Designated Authority prior to the preconstruction conference. The contractor that receives this award will have 10 calendar days to provide an Accident Prevention Plan (APP). If this deadline is not met, the contractor will be given a cure notice stating that they will have 10 calendar days to submit an APP, otherwise the contract may be terminated for default.

SCOPE OF WORK
Statement of Work and Specifications
Repair and Resurfacing: Entrance Road and Paved Areas in Upstream Recreation Area Beech Fork Lake (Lavalette, WV)
GENERAL/OVERVIEW:
This contract is for repair and resurfacing of various identified sections of paved roadway at Beech Fork Lake, including portions of the primary entrance roadway, the juncture of the primary entrance roadway and the access road atop the dam, and a section of roadway exiting the Marina/Upstream Recreation Area parking lot and small connecting portion of the lot. The contract includes a base contract and three options. The entrance roadway ends at the Marina/Upstream Recreation Area parking lot. Traffic flow is one way and loops through the lot.
To exit the project traffic utilizes a paved exit lane connected to the lot that reconnects to the same two-way roadway.
The base contract consists of repair and resurfacing a section of approximately 210’ X 20’ of the existing entrance roadway to the Beech Fork Lake project. It is a two-lane double yellow-striped roadway. This section of pavement in the base contract is the most severely deteriorated and roughest section of pavement of all areas included herein. In addition to the age of the existing pavement, insufficient drainage issues within this stretch have compounded problems to the roadway. The inadequate drainage issues allow water to pond in the roadway and alongside it for extended durations after precipitation. As a result, water is attempting to migrate subsurface downgrade under the pavement, leading to stability issues in the road base and pavement. The base contract also includes the installation of an 18” culvert at a low elevation point within this stretch to remedy drainage issues and prevent water from laying upon and adjacent to the new pavement. Additionally, the 210’ base section will require replacement of the existing compromised base material to adequately support the new pavement. Further details for the base contract and both options will be listed for each directly below in the “Scope of Work” section.
Option 1 consists of repair and resurfacing of two sections of the existing entrance roadway that adjoin both the start and terminus of the 210’ X 20’ section in the base contract. The section entering the project prior to the base contract section is approximately 685’ x 20’ and the section beyond the base section is approximately 180’ x 20’. These two sections are also double yellow-striped roadway. Both sections have widespread damage to the pavement surface throughout, but not near the extent of the section in the base contract. There is an existing culvert in the 685’ stretch, located immediately after a gravel access road on the left in need of replacement. The culvert inlet is in the left ditch as you enter the project at this location and the outlet daylights on the base of the bank to the right of the roadway. The interior of the existing 15” galvanized culvert is over 50% filled with sediment and debris. A new 18” culvert at this low point is included to allow for adequate drainage for the paved roadway and gravel access roadway that has a ditch that also drains towards the culvert. The remaining 180’ x 20’ section beyond the base contract portion will only require repair and resurfacing with asphalt, no drains or excavations are required in this section of Option 1.
Option 2 of the contract consists of a portion of the paved upstream parking lot and the paved exit access road that it is connected to. The portion of the lot and the connected access exit road total approximately 3,600 square feet of paved surface. There are both pavement deterioration issues and drainage issues within this reach that need addressed. Temporary patching and repairs performed on the parking lot portion and transition to the access road completed almost 20 years ago were not performed correctly. As a result, water repeatedly ponds after precipitation in an 8- 10-foot-long section in this reach. Significant potholes have developed from these ongoing conditions. Grading measures along with adequate drainage features need to be incorporated into the repair in addition to the resurfacing work. Otherwise, any new pavement and resurfacing performed in this area will result in continuing ponding conditions and subsequent damages to new pavement. The project has purchased cold patch asphalt repair materials repeatedly over the years in attempt to temporarily remedy the potholes, but such repairs do not even last a full year.
The parking lot portion and transition area where the water repeatedly ponds are very flat, with the transition area being slightly lower. Just beyond the transition area where the road narrows there is an existing 18” galvanized culvert approximately 3.5-4’ below the roadway and drainage ditch on both sides that catch and convey runoff from other portions of the parking lot and areas upgrade and adjacent. Shortly after the culvert the access road increases in elevation rapidly compared to the transition area. Removal of the existing culvert and installing a slotted inlet riser drain and culvert would require extensive grading work to accomplish and would remove a structurally sound culvert that is performing adequately. To achieve adequate drainage and overcome grading constraints the installation of a load-rated precast grated surface drain is proposed. The grated surface drain would extend perpendicular across the roadway at the far end of the transition area when exiting. Minor regrading work will need to be performed from the beginning of the parking lot portion to this location to ensure water reaches the drain and the drain is at the lowest point from the lot and beyond the drain while exiting. The precast surface drain would need to have built-in slope and drainage would flow across the roadway from left to right and daylight into the same drainage ditch system via an outlet pipe that connects to the low end (east side upon exit) of the grated surface drain.
Option 3 includes repair and resurfacing of a smaller section of paved roadway. This section entails the beginning of the asphalt roadway on the crest of the dam where it ties into the access roadway to the project. The total area of pavement in need of repair and resurfacing at this location is approximately 450 square feet. To accomplish the repair and resurfacing the existing damaged 450 square foot pavement area will need to be milled and removed prior to the placement of new pavement. The new pavement will need to be installed in a manner that creates a smooth transition at the juncture of the two roadways and slightly sloped to allow water to drain readily to the east and prevent it from laying at the interface of the two roads to prevent future pavement damages.
The contractor will have 45 calendar days from the day the Contracting Officer issues the Notice to Proceed to complete all work on the base contract. Then with each Option (Option 1, Option 2 and Option 3) that is exercised, there will be an additional 45 calendar days added for the contract completion.
More information, details, figures, and pictures for the base contract and all three options will be included in the Scope of Work (SOW) section below for each and associated appendices. All figures/maps within the SOW are displayed in a northern orientation.

TIME OF PERFORMANCE:
Normally, all work will be performed on the basis of one (1) 8-hour shift per day between the hours of 0700 and 1700 Eastern Time Zone, five (5) calendar days a week. Unless other times and days are approved by the Contracting Officer. The normal work week will run from Monday through Friday, excluding holidays. The eleven (11) Federal Holidays observed are:

When one of the above-designated holidays falls on a Sunday, the following Monday is observed as a legal holiday. If the contractor plans to work outside the normal working hours or on a Federal Holiday, the contractor must notify the Contracting Officer or his representative in writing one
(1) week prior to beginning an alternate work schedule.

SCOPE OF WORK:
Base Contract: The contractor shall be responsible for having the entire area within the base contract surveyed for any underground utilities to allow them to be avoided or addressed appropriately and ensure safety to workers. The contractor shall utilize West Virginia 811 to survey for any underground utilities.
The general description of work to be performed under the base contract will require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to accomplish the milling/removal of existing pavement, required repairs, subgrade removal and replacement, paving, and striping. The work will also require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to install a new 18” culvert at a specified location within the base contract footprint specified by the government representative onsite. Additionally, the contractor will be responsible for providing all traffic control resources and equipment to accomplish such work and in accordance with all safety requirements contained within Appendix A of this document.
Contractor shall excavate trench and install approximately 40’ of 18” high density polypropylene pipe (rated for highway applications) across the roadway at a location specified by the government representative onsite. The 18” pipe shall have a smooth interior and annular exterior corrugations. The culvert shall have a slope of 1/8” per linear foot from the inlet to the outlet of the pipe. The inlet of the pipe will be off the right shoulder of the entrance road in the ditch line, on the exiting lane of the roadway. The outlet will extend just beyond the shoulder on the opposite side. Rip-Rap (4”x8”) stone protection shall be placed 3’ from the centerline of the pipe in each direction immediately adjacent to the inlet and 3’ from the centerline of the outlet in each lateral direction and extending 6’ below and beyond the outlet. The contractor shall excavate the pipe trench to allow for 18” of uniform trench on each side of the 18” diameter culvert to allow for a vibratory compactor to be used on each side of the pipe to properly compact gravel on each side. A minimum of 4” of crushed aggregate (free of particles less than 1 1/2”/40 mm) shall be placed below the pipe throughout its length prior to placing the pipe in the trench. The bedding aggregate under the center 1/3 of the pipe shall be loosely placed and uncompacted to allow for cradling of the pipe bottom, and the bedding outside of the middle 1/3 shall be compacted.
Contractor shall place and compact crushed aggregate (free of particles less than 1 1/2”/40 mm) on each side of the pipe in no more than 4” lift increments and shall be placed and compacted over the pipe to a level that results in a minimum of 18’’ of cover atop the pipe when combined with asphalt base layer and asphalt wearing course layers. Care shall be taken to compact the material under the haunches of the pipe, to place the backfill evenly on each side of the pipe to retain its vertical axis, and to avoid displacement. The backfill and compaction efforts shall be advanced simultaneously on both sides of the pipe. Compaction must be completed to a level of 90% of standard proctor on all the aggregate backfill material.
Note: The contractor shall install culvert prior to any other work included in the base contract to minimize the amount of time the roadway will have to be closed to traffic. The culvert shall be installed within one day, and work shall be limited to a non-holiday weekday to reduce the impacts to traffic and increased project visitation on weekends. Contractor shall notify the project no less than 24 hours in advance of the day they intend to install culvert. USACE and Beech Fork Lake Marina personnel will make provisions to have personal vehicles beyond the culvert location should they need to exit the project in the event of an emergency or essential circumstance. Upon completion of culvert installation, the contractor shall utilize West Virginia Division of Highways (WVDOH) approved steel road plates if grade conditions require them to allow traffic to pass until final paving work is completed.
Contractor shall install and maintain traffic control, saw cut perpendicular across the full width of the roadway at the start and end of the base contract section of roadway as specified by the government representative on site. These locations will be marked with high visibility marking paint by the government representative. Contractor shall mill/remove all existing asphalt in the marked 210’ x 20’ base section and additionally excavate approximately 12” total which will extend beyond the existing asphalt. Contractor shall remove existing aggregate base materials under asphalt with the grade of the existing section. Once removed to the specified level the contractor shall place and compact approximately 6” of Class 1 aggregate base to match existing grade conditions throughout the base section. Contractor shall apply asphalt bonding agent equally to the entire area of aggregate base and follow with placement of 4” of base course asphalt pavement to the entire section. After the base course has been completed the contractor shall place 2” inches of wearing course asphalt throughout the entire section to create the final paved roadway surface. Pavement shall be double striped at the center of the roadway width throughout the base section and shall be crowned at the centerline with a 2% slope extending to each side of the road shoulders upon completion of final wearing course pavement. Work shall be done in accordance with all current State & Federal Road Construction Specification, Rules & Regulations. Figure 1 on the following page shows the 210’ section of the base contract and where each portion of Option 1 connects to it.
Except for the day the new 18” culvert is installed, the contractor shall maintain traffic access for one lane of traffic, with a maximum of ½ hour delay and total closure during all other repair and paving operations. The contractor shall maintain traffic control and signage throughout the entire duration of the base contract and in accordance with the safety plan.
The contractor shall dispose of any excess soil, aggregate, and any asphalt millings not repurposed nearby onsite on lands owned by the Corps. The site is approximately ½ mile or less from all work areas included in this scope of work and is readily accessible. The disposal site is essentially flat and there are no grade/slope issues for hauling to the site. Figure 2 on page 7 shows the location of the disposal area. The government representative on site will show the contractor the disposal location. The contractor shall be responsible for the proper disposal offsite of all other excess materials not listed above generated during performance of the base contract.

Option 1: The contractor shall be responsible for having the entire area within Option 1 surveyed for any underground utilities to allow them to be avoided or addressed appropriately and ensure safety to workers. The general description of work to be performed under Option 1 will require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to accomplish asphalt milling, leveling, required repairs, paving, and striping to the two sections within Option 1. The work will also require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to install a new 18” culvert at the same location of an existing 15” corrugated metal culvert, upon removing the existing culvert. The existing culvert is near the end of the first section of Option 1 when entering the project. Additionally, the contractor will be responsible for providing all traffic control resources and equipment to accomplish such work and in accordance with all safety requirements contained within Appendix A of this document.

Rip-Rap (4”x8”) stone protection shall be placed 3’ from the centerline of the pipe in each direction immediately adjacent to the inlet and 3’ from the centerline of the outlet on each lateral direction and extending 6’ below and beyond the outlet. The contractor shall excavate the pipe trench to allow for 18” of uniform trench on each side of the 18” diameter culvert to allow for a vibratory compactor to be used on each side of the pipe to properly compact gravel on each side. A minimum of 4” of crushed aggregate (free of particles less than 1 1/2”/40 mm) shall be placed below the pipe throughout its length prior to placing the pipe in the trench. The bedding aggregate under the center 1/3 of the pipe shall be loosely placed and uncompacted to allow for cradling of the pipe bottom, and the bedding outside of the middle 1/3 shall be compacted. Contractor shall place and compact crushed aggregate (free of particles less than 1 1/2”/40 mm) on each side of the pipe in no more than 4” lift increments and shall be placed and compacted over the pipe to a level that results in a minimum of 18’’ of cover atop the pipe when combined with asphalt base layer and asphalt wearing course layers. Care shall be taken to compact the material under the haunches of the pipe, to place the backfill evenly on each side of the pipe to retain its vertical axis, and to avoid displacement. The backfill and compaction efforts shall be advanced simultaneously on both sides of the pipe. Compaction must be completed to a level of 90% of standard proctor on all the aggregate backfill material.
Note: The contractor shall install culvert prior to any other work included in Option 1 to minimize the amount of time the roadway will have to be closed to traffic. The culvert shall be installed within one day, and work shall be limited to a non-holiday weekday to reduce the impacts to traffic and increased project visitation on weekends. Contractor shall notify the project no less than 24 hours in advance of the day they intend to install culvert. USACE and Beech Fork Lake Marina personnel will make provisions to have personal vehicles beyond the culvert location should they need to exit the project in the event of an emergency or essential circumstance. Upon completion of culvert installation, the contractor shall utilize West Virginia Division of Highways (WVDOH) approved steel road plates if grade conditions require them to allow traffic to pass.
Contractor shall install and maintain traffic control, saw cut perpendicular across the full width ofthe roadway at the start of the beginning and end of each section of roadway in Option 1. The contractor shall mill 2” of the existing asphalt throughout each section of Option 1. Contractor shall apply asphalt bonding agent equally to the entire area within each of the section. The contractor shall level milled surface in required spots on shoulders and spots within each section requiring leveling with scratch asphalt. The contractor shall place 2” of wearing course asphalt throughout the entire extent of each section. The pavement shall be double striped at the center of the roadway width throughout each section of Option 1 and shall be crowned at the centerline with a 2% slope extending to each side of the road shoulders upon completion of final wearing course pavement. Work shall be done in accordance with all current State & Federal Road Construction Specification, Rules & Regulations.
Except for the day the new 18” culvert is installed, the contractor shall maintain traffic access for one lane of traffic, with a maximum of ½ hour delay and total closure during all other repair and paving operations within Option 1. The contractor shall maintain traffic control and signage throughout the entire duration of Option 1 and in accordance with the safety plan.
The contractor shall dispose of any aggregate and asphalt millings not repurposed nearby onsite on lands owned by the Corps at the same location discussed in the base contract and shown in Figure 2 above. The contractor shall be responsible for the proper disposal offsite of all other excess materials not listed above generated during performance of Option 1.

Option 2: The contractor shall be responsible for having the entire area within Option 2 surveyed for any underground utilities to allow them to be avoided or addressed appropriately and ensure safety to workers. The contractor shall utilize West Virginia 811 to survey for any underground utilities.
The general description of work to be performed under Option 2 will require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to accomplish asphalt milling, leveling, required repairs, and paving within Option 2. The work will also require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to install a pre-cast heavy duty grated drain at a specified location within the footprint of Option 2 specified by the government representative onsite. The entire area of Option 2 is approximately 3,600 square feet. Figure 3 on page 11 shows the boundary of the area with the “blue” line as the approximate location where the precast drain is to be installed. The exact location will be marked by the government representative onsite with high visibility paint. The larger area with a “red” perimeter (1,150 square feet) that adjoins the parking will require asphalt milling and paving sloped to transition drainage to the drain, as will the smaller area with a “red” perimeter (250 square feet). The gravel outflow parking lot directly to the east of the area of Option 2 has an exit lane that ties into the north portion of Option 2. The contractor shall direct traffic to utilize the overflow lot and its exit lane for all work performed during Option 2 that is south of where the exit lane ties in. This will allow for continual traffic flow for the drain installation which will require concrete placement, formwork, and time to properly cure. The area of Option 2 in Figure 3 with a “yellow” perimeter will only entail milling of 1.5” of existing asphalt, asphalt leveling to any spots and shoulders within the area requiring leveling and paving with a new 1.5” surface course of asphalt. For the duration of the work to be performed in the northern portion of the exist lane the contractor shall direct traffic back through the entry lane just to the west with placement of divider markers in the center of the entry lane.
The contractor shall install the precast grated drain system and associated concrete prior to performing any of the other pavement work entailed in Option 2. The overflow parking lot and exit roadway shall be utilized during all associated drain work and the contractor shall convey traffic via signage and barricades to the route.
The contractor shall install a 24’ long precast drain across the exit lane at the location specified by the government official onsite. The location will also be marked across the roadway with a line of high visibility paint. The precast drain shall have built in slope to allow water to drain readily from left to right (west to east). The drain shall be 4” wide at the bottom throughout its entire length and be capped at the west end with an outlet on the east end that will connect to a 4”diameter PVC drain pipe, that will daylight out to the existing ditch on the eastern side of the exit lane. The precast drain system and grating shall have a “D” load rating (90,000 pounds). The drain shall have tamper resistant iron grating across the entire top of the 24’ long drain.
The contractor shall saw cut and excavate a trench across the exit lane of the parking lot at the location marked by the government representative onsite. The trench must be at a depth to allow for a minimum of 6” of concrete to be under the entire channel system. The contractor shall form the trench at a width no less than 20” and no greater than 24” in width and the drain system shall 10 be situated in the center of the formed trench. The system shall be installed so that the outlet is on the east side of the exit lane where it will discharge into an existing ditch downgrade and an end cap shall be installed at the west side/start of the drain system. There is no inlet on the system as water will enter through the grating from both the south and north side of the exit lane.
The contractor shall perform milling, grading, and paving measures to ensure water from both the south and the north will flow readily to the drain. The final grade of the cover on the drain system shall be 1” lower than the existing surface elevation of the preconstruction pavement to allow for grading and paving measures to align on each side of the drain and allow water to flow to the drain after the final course of pavement.
Upon completion of the installation of the drain system the contractor shall place and compact crushed aggregate (free of particles less than 1 1/2”/40 mm) in lifts no greater than 4” in the voids on each side of the trench and allow for 4” of base course asphalt and 1.5” of wearing course asphalt atop the compacted crushed aggregate to be at grade with each side of the concrete form. Compaction must be completed to a level of 90% of standard proctor on all the aggregate backfill material. The contractor shall not move any vehicles or equipment across the drain once installed as paving operations can be completed from each side of the drain.
Within the larger area with a “red” perimeter (1,150 square feet) shown in Figure 3 on the following page the contractor shall mill/excavate the existing asphalt to a depth of approximately 4” on the southern extent of the area and gradually extend milling/excavation to approximately 6” to the interface of the concrete drain form. Contractor shall apply asphalt bonding agent equally to the entire area and place leveling asphalt in any required spots within the area. The contractor shall follow with the placement of 2.5” of base course asphalt and 1.5” of wearing course asphalt throughout the entire area.
Within the smaller area with a “red” perimeter (250 square feet) the contractor shall mill/excavate starting at 4” at the northern boundary and extending to 6” at the interface of the concrete drain form. Contractor shall apply asphalt bonding agent equally to the entire area and place leveling asphalt in any required spots within the area. The contractor shall follow with the placement of 2.5” of base course asphalt and 1.5” of wearing course asphalt throughout the entire area.
For the remainder of the area of Option 2 within the “yellow” perimeter (2,200 square feet) in Figure 3 the contractor shall mill 1.5” of the existing asphalt within the area. Contractor shall apply bonding agent equally to the area and place leveling asphalt in any required spots within the area. Contractor shall follow with the placement of 1.5” of wearing course asphalt. No striping is required within any of the sections within Option 2. For the milling, leveling, and paving work to be completed in the portion of Option 2 that is north/beyond the tie in lane from the gravel overflow parking lot to the east the contractor shall install a cone/barricade divider line in the center of the entrance lane to the Upstream Recreation parking lot and provide traffic control signage and control throughout this portion of work to be completed in Option 2. The one-way entrance lane is wide enough to allow for opposing traffic, but signage, markers, and control must be maintained throughout.

Option 3: The contractor shall be responsible for having the entire area within Option 3 surveyed for any underground utilities to allow them to be avoided or addressed appropriately and ensure safety to workers. The contractor shall utilize West Virginia 811 to survey for any underground utilities.
The general description of work to be performed under Option 3 will require the contractor to furnish all equipment, materials, labor, supplies, and supervision necessary to accomplish asphalt milling, leveling, required repairs, and paving for Option 3. Figure 4 on the following page shows the boundary of Option 3 outlined in yellow.
Contractor shall install and maintain traffic control, saw cut perpendicular across the full width of the dam access roadway at the start and end of the section specified by the Government official on site. The limits will be marked with high visibility paint as well. The entire section of Option 3 is approximately 450 square feet. Figure 4 on the following page shows the extent of Option 3. Contractor shall mill the entire extent of Option 3 to a depth of approximately 4”.
Contractor shall apply asphalt bonding agent equally to the entire area and follow with the placement of 2.5” of base asphalt. Next, the contractor shall place 1.5” of wearing course asphalt to the entire area. No striping is required for this section. The Contractor shall install the new pavement in a manner that creates a smooth transition at the juncture of the two roadways and slightly sloped to allow water to drain readily to the east and prevent it from laying at the interface of the dam access roadway and Beech Fork Road to prevent future pavement damages.
The contractor shall maintain traffic control and signage throughout the entire duration of Option 3 and in accordance with the safety plan. Additionally, for Option 3, the Contractor shall maintain one lane of traffic on Beech Fork Road throughout the performance of Option 3. Work shall be done in accordance with all current State & Federal Road Construction Specification, Rules & Regulations.
The contractor shall dispose of the asphalt millings nearby onsite on lands owned by the Corps at the same location discussed in the base contract and shown in Figure 2 above. The contractor shall be responsible for the proper disposal offsite of all other excess materials not listed above generated during performance of Option 3.

SAFETY
Appendix A
Contractor Accident Prevention Plan (APP).
A. General. Contractor shall comply with all Federal, State and Local safety requirements including those specified by the Occupational Safety and Health Act (OSHA) in 29 CRF 1910 General Industry Standard, 1926 Construction Standard and the U.S. Army Corps of Engineers Safety & Health Requirements Manual EM 385-1- 1 as currently revised. Where a conflict exists between safety standards/requirements the contractor shall comply with the more stringent requirement. If the Contractor fails or refuses to promptly comply with safety requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.
Contractors shall comply with all current health and safety guidelines from the Centers for Disease Control and Prevention (CDC) and Occupational Safety and Health Administration (OSHA) in order to reduce the risk of spreading any community-based illnesses in the workplace
B. Contractor Accident Prevention Plan (APP). The Contractor shall prepare a site-specific Accident Prevention Plan following the outline provided in EM 385-1-1, Appendix A as a minimum. The contractor shall address each element and sub-element in the outline in the order they appear in Appendix A. Any item from the outline that does not apply must be listed, in its assigned place and a brief explanation of the reason it is not required provided. The APP shall address any unusual or unique aspects of the project or activity. This plan must be accepted by the Contracting Officer prior to commencement of any work. Contractors are encouraged to contact the USACE Safety & Occupational Health Office for questions or information regarding the development of contract Accident Prevention Plans. Office hours are Monday through Friday 8:00 AM to 3:00 PM at (304)399-5032/5326/6961.

(1) The contractor shall have on site an employee designated as a Site Safety and Health Officer (SSHO) who has completed the OSHA 30-hour Construction Safety training or its equivalent as specified in EM 385-1-1, 01.A.17 and has at least five years safety management experience relative to the work to be performed. No work will be permitted unless the SSHO is present at the work site.
(2) Work crews of two or more must have two people holding current certification in First Aid and CPR from the American Red Cross, American Heart Association or a licensed physician. Training must include a hands-on component that cannot be taken online and the contractor must maintain appropriate First Aid kit on site.
Single individuals working alone must have current certification in First Aid as a minimum.
(3) All work that potentially exposes the worker to falls of six feet or more will require the contractor to submit a Site Specific Fall Protection & Prevention Plan in accordance with EM 385-1-1, Section 21.D.
(4) Cranes and hoisting equipment shall be operated only by qualified personnel (operators licensing, physical and drug testing in accordance with EM 385-1-1 Section 16.B). Note: CDL Medical Clearance is not acceptable. Please see attached medical clearance requirements.
(5) Effective November 8, 2010 all “Signalpersons” and “Riggers” shall be qualified.
This is a mandatory requirement in accordance with 29 CFR 1926.1400
Regulations. Riggers must be a qualified person for the performance of specified hoisting activities such as during assembly/disassembly, in accordance with OSHA 1926.1404(r)(1) and 1926.1425(c)(3).
(6) Work orders involving potential exposure to Hazardous, Toxic or Radiological Waste shall require development of Site Specific Safety and Health Plan (SSHP) by qualified personnel as specified in EM 385-1-1 Sections 6 and 33.
C. Risk Management Process. The Contractor shall utilize risk management tools such as Activity Hazard Analysis (AHA), Job Hazard Analyses (JSAs) or similar tools to identify, assess, prioritize risks and develop controls to minimize, monitor and control the probability and/or impact of unfortunate events to an acceptable level. Within each major phase, all activities, tasks or Definable Features of Work (DFOWs) shall be identified that will require an AHA/JSA. For each work activity, task or DFOW contractor shall prepare the initial AHA/JSA.
A Risk Assessment Code (RAC) is assigned to each step, to the risk that remains after controls have been applied (residual risk) a final RAC (cannot be lower than the highest step RAC) shall be assigned. See EM 385-1-1 Section 01.A.14.b. for further guidance or contact the SOHO for assistance. The AHA/JSA shall be reviewed and accepted by the proper authority in order to proceed with the activity. All on-site employees shall review the accepted AHA/JSA prior to performing work and during weekly and “Tool Box” safety meetings as appropriate.
(1) AHA/JSA shall define the activities being performed and identify the work sequences, the specific anticipated hazards, site conditions, equipment, materials, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level of risk. AHA/JSA should indicate compliance with EM 385- 1-1 personal protective equipment requirements (safety-toed work boots, safety glasses, gloves, hard hats, etc.), work crews of two or more must have two people currently qualified in first aid and CPR and the appropriate first aid kit on site.
Single individuals working alone must have current certification in First Aid only.
(2) The names of the Competent Person(s) and Qualified Person(s) required for a particular activity (e.g., excavation, scaffolding, fall protection, or other activities as specified by OSHA and this manual) shall be identified and included in the AHA, as well as proof of their competency/qualification.
D. Accident/Mishap Reporting. The Contractor shall report all recordable mishaps to the GDA within 24-hours in accordance with EM 385-1-1 Section 01.D. Accidents shall be investigated to determine causal factors and corrective actions shall be implemented. Accidents resulting in: days away injuries, days away illnesses, restricted/transfer injuries, property damage of $5,000.00 or more shall be investigated and reported to the GDA using the USACE Accident Investigation Report Form ENG 3394 within 24 hours of the occurrence.
(1) Immediate notification to the GDA is required for all serious accidents:
a. Fatal Injury/illness;
b. Permanent totally disabling injury/illness;
c. Permanent partial disabling injury/illness;
d. One (1) or more persons hospitalized as inpatients as a result of a single occurrence;
e. $500,000 or greater accidental property damage;
f. Three (3) or more individuals become ill or have a medical condition suspected to be related to a site condition, or a hazardous or toxic agent on the site;
g. USACE aircraft destroyed or missing;
The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300 and comply with all accident reporting and recordkeeping requirements specified in 29 CFR 1904.39.
(2) In addition to the above, any mishap occurring in any of the following high hazard areas shall be reported immediately to the GDA, investigated and corrective actions implemented.
a. Electrical (includes Arc Flash, electrical shock, etc.);
b. Uncontrolled Release of Hazardous Energy (includes electrical and non-electrical);
c. Load Handling Equipment (LHE) or Rigging;
d. Fall-from-Height (any level or other than same surface);
e. Underwater Diving

SECURITY REQUIREMENTS
All Contractors and subcontractor employee(s) working on any contract who require access in and/or around any Huntington District facilities, projects, owned or leased facility/space shall receive authorization to be on site at these areas based upon a favorable background investigation in accordance with “Homeland Security Presidential Directive 12: Policy for a Common Identification Standard for Federal Employees and Contractors” (HSPD 12).
