Contractors are not required to submit an Accident Prevention Plan (APP) with their quote.
The successful offeror will be required to submit this document upon award. The APP must be accepted by the Government Designated Authority prior to the preconstruction conference. The contractor that receives this award will have 10 calendar days to provide an Accident Prevention Plan (APP). If this deadline is not met, the contractor will be given a cure notice stating that they will have 10 calendar days to submit an APP, otherwise the contract may be terminated for default.
Only the awarded contractor shall provide a copy of their insurance certificate required by State and Local Governments that has been acquired for the time period covered under this contract to the Contracting Officer within 10 business days after award. No further insurance requirements will be imposed by the Corps of Engineers for this contract.

2 Project Scope
The Contractor shall furnish and install all equipment and materials and perform all tasks and operations necessary for the complete replacement of the following elevator components, systems, or sub‐systems as described in this document.
The Contractor shall be responsible for removing the existing control equipment, materials and appurtenances that will be replaced in this contract. The equipment removed shall remain the property of the Government and shall be delivered in good condition to the Resource Manager of the project. The Contractor shall be responsible for transporting said equipment to an on‐site location at the Resource Manager’s discretion.
It is suggested that the bidder visit the project site before preparing their bid in order to most accurately ascertain the scope of work with regards to existing equipment condition, accessibility, working conditions, and specific working and system design parameters.
The work to be accomplished and equipment and material to be furnished and installed shall include but not be limited to:
1) New Controller. Furnish and install a new microprocessor based controller used in conjunction with a new Variable Frequency Drive (VFD) for traction motor control. The Contractor shall also furnish a new position detection system for the car and new car leveling control system. This task shall also include the demolition and removal of all unnecessary existing equipment.
2) New Control Wiring and Misc. Tasks. The Contractor shall furnish and install all new wiring for the elevator control system, except for wiring inside the car. The wiring is located between the new controller and existing limit switches, hall call stations, car position lights in the hall stations, stop switches in the hoistway pit and any other elevator controls or indicators. The Contractor may install a new raceway or may reuse the existing raceway (provided its condition it suitable). Should the Contractor choose to install a new raceway in the hoistway, the raceway shall conform to the applicable requirements of the specifications below. It shall also include the demolition and removal of all unnecessary existing equipment.
All physically damaged non‐metallic conduit and rust damaged metallic conduit shall be replaced in kind. Replacement shall be determined at the discretion of the Resource Manager. All other conduit may be replaced or reused as needed. It shall also include the demolition and removal of all unnecessary existing equipment.
The Contractor shall furnish and install a new elevator car traveling cable and any conduit required (between controller and termination junction box on car). It shall also include the demolition and removal of all unnecessary existing equipment.
The Contractor shall furnish and install a new car position detection system, including all wiring and raceways required for the leveling controls. It shall also include the demolition and removal of all unnecessary existing equipment.
3) Traveling Cable. The Contractor shall furnish and install a new traveling cable that meets the operating and location requirements of existing elevator system. It shall be similar in type, size, material and grade of the existing cable.
4) Hoistway Limit Switches & Wiring. The Contractor shall furnish and install new top and bottom limit switches and wiring in accordance with the new control system. It shall also include the demolition and removal of all unnecessary existing equipment.
5) Car Top Operating Switch: The Contractor shall furnish and install a new car top operating switch. It shall also include the demolition and removal of all unnecessary existing equipment.
6) Hoist Ropes. The Contractor shall furnish and install new hoist ropes. It shall also include the demolition and removal of all unnecessary existing equipment.
7) Rework Hoistway Piping as Necessary. The Contractor shall rework hoistway piping as necessary to bring elevator into current elevator standards. Equipment shall meet standards for wet locations.
8) Rework Hoistway Door interlocks. The Contractor shall rework door interlocks as necessary to bring elevator into current elevator standards. Equipment shall meet standards for wet locations.
The preceding list is not intended to be fully descriptive, but rather a highlight of each task. Detailed specifications are provided throughout this document.

The contractor is responsible for ensuring their designs and workmanship meet the standards listed above.
4 System Description
4.1 Purpose
The elevator controller and all components, systems and sub systems shall conform to the requirements of ASME A17.1, except that provisions for fire service are not required. The controller shall be designed to have the capacity to lift a live load, exclusive of the car and cable, at a speed as specified in the following schedule. The approximate travel, terminal floors and the number of stops and openings shall be as shown in the schedule. The elevator shall serve the floors with stops and openings in accordance with the requirements indicated.
The ultimate goal is to provide rehab services to the R.D. Bailey Lake Intake Structure Elevator that result in an elevator system that provides project staff with reliable, safe service.
4.2 Elevator Schedule
Elevator Designation: R.D. Bailey Lake Intake Structure Elevator Service: Passenger/Freight Capacity: 1200 lbs.
Speed: 150 Feet per minute

Entrance Type: 36” manually operated Voltage Available: 480vac, 60 Hz, 3phase Motor: Magil; 3 Phase 7.5HP, 1145 RPM, 24 Amps, Southeastern Elevator Company Total Travel: Approximately 260 ft.
4.3 Standard Products
Materials and equipment shall be the standard products of manufacturers regularly engaged in the fabrication of elevator controllers and equipment and shall essentially duplicate items which have been in satisfactory use for at least five (5) years prior to bid opening.
Equipment and components used on this project must be of new condition and manufactured within 300 days of the contract award date. Reconditioned and/or aftermarket or surplus equipment is not acceptable for use on this project.
4.4 Name Plates
Motor equipment shall have the manufacturer's name, address, type or style, model or serial number, catalog number, and electrical and mechanical characteristics on a plate permanently secured to the frame of the equipment.
4.5 Environmental Conditions

5 Submittals
The bidder shall submit descriptive literature with their quotation for review upon receipt in the district office. The descriptive literature submitted shall include manufacturers catalog sheets for the elevator controller including the processor module and other modules that make up the microprocessor based controller and all other components located in the controller enclosure. Following award of the contract, the successful bidder shall provide drawings and literature as set forth below. Government approval is required for submittals with a "G" designation; submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. The following shall be submitted in accordance with the requirements outlined in these specifications:
5.1 Accident Prevention Plan (G‐SO)
After contract award and before beginning work submit an Accident Prevention Plan as required in Appendix A of USACE’s EM 385‐1‐1.
5.2 Shop Drawings and Product Data (G‐EC)
Submit shop drawings consisting of a complete list of equipment and materials, including illustrations, schedules, manufacturer's descriptive and technical literature, performance charts, catalog cuts sheets, installation instructions, brochures, diagrams, complete functional description of the controller, definitions of symbols used, printout of microprocessor logic with complete documentation, setting of all dip switches (if appropriate), and other information required for fabrication and installation of the equipment. Shop drawings shall include dimensioned layouts in the plan and elevations showing the arrangement of the elevator controller and equipment. This submittal must be provided to the government after contract award. Government approval must be granted prior to beginning work.
5.3 Testing (G‐EC)
Upon completion of performance testing and acceptance of the installed system, submit test reports showing all field tests performed to prove compliance with the specified performance criteria. All data and procedures shall be recorded and provided in the report.
5.4 Elevator Mechanics (FIO)
Submit certificates of experience for the elevator mechanics prior to installation of elevators.
5.5 Operation and Maintenance Manuals (G‐EC)
Submit three (3) complete copies of operating instructions outlining the step‐by‐step procedures for system start up, operation, and shutdown. The instructions shall include the manufacturer's name, model number, service manual parts list, and brief descriptions of all pieces of equipment and their basic operating features. Operating instructions shall be submitted and approved prior to the training course discussed Section 8. The Contractor shall provide the name, address, and phone number of the local representative of the elevator controller supplier.
Submit three (3) complete copies of maintenance instructions listing routine maintenance procedures, possible breakdowns and repairs, and troubleshooting guides. The manual shall include a write‐up on how to interpret the troubleshooting codes that are provided by the controller. The instructions shall include equipment layout and wiring and control diagrams of the system as installed. The manuals shall also include complete descriptive literature, catalog cuts and all other pertinent information submitted during the shop drawing process, including printout and documentation of the control program used for operation of the elevator.
Maintenance instructions shall be submitted and approved prior to the start of the training course described in Section 8.
6 Qualifications
Elevator mechanics employed to install, supervise personnel involved in installation, and test the elevator shall be certified to have not less than five years’ experience installing, supervising personnel involved in installation, and testing elevators of the type and rating specified. Helpers or apprentices with less than two years’ experience will be permitted to work only under the direct supervision of a certified elevator mechanic.
7 Project Site Conditions
The contractor shall become familiar with the jobsite, verify all dimensions and space requirements in the field and on shop drawings, become familiar with existing elevator control system and advice the Contracting Officer of any discrepancy before performing any fabrication or installation work.
8 Training
A training course shall be conducted by a qualified individual or individuals employed by the elevator controller supplier and familiar with the installation. The training course shall be for Government personnel as designated by the Resource Manager. The training period shall consist of a total of 8 hours during a normal work day and shall start after the system is functionally completed and tested. The field instruction shall cover familiarization of the new equipment provided, routine maintenance tasks, troubleshooting of the controller, details of the processor and I/O modules including the operation of the system and how all components work together, and shall be based on the items contained in the operating and maintenance instructions. The Contract shall coordinate with the Resource Manager to schedule said training.
9 Equipment Specifications
9.1 Controller
The controller shall feature a non‐proprietary, solid‐state microprocessor suitable for use with the supplied Variable Frequency Drive (VFD). The controller shall be provided with a nameplate, white letters on black background, engraved to read “ELEVATOR CONTROLLER”. Lettering shall be 1” in height.
The control system shall govern the starting, stopping, and direction of travel of the elevator and provide the operation specified herein. Components required for proper performance of the elevator shall be neatly mounted and wired and completely enclosed in a cabinet with a mechanically latched door. The controller processor shall be new, unused, of current manufacture and shall be a standard product of the manufacturer. The programmable logic controller (PLC) system shall be that which is customarily used in elevator controllers.
Replacement of any of the components of the microprocessor control system shall not require purchase of custom manufactured equipment, but instead, purchase of off‐the‐shelf components manufactured in volume. The control program shall be contained in non‐volatile memory such as EPROM, EEPROM or FLASH memory such that upon failure of electrical power service to the controller, the control program will be retained in processor memory without the use of backup batteries. The processor and I/O modules shall be the modular type and shall be replaceable simply by unplugging from the rack or backplane and plugging the new module in. A processor or I/O module that is soldered in to the backplane or rack is not acceptable. Processor programming shall be accomplished in conventional relay ladder logic or as otherwise approved by the contracting officer. The Contractor shall include in their shop drawing submittals the proposed processor logic and all related documentation. The O&M manuals provided by the Contractor shall provide complete documentation on the final version of processor logic provided in the controller.
Additional Controller Specification:
‐ Controller shall be ASME 17.5 labeled and approved by an OSHA NRTL. All assemblies, power supplies, chassis switches, and relays shall be mounted on a type of metal frame in a NEMA rated cabinet that is self‐supporting and that completely encloses the equipment. A means to control the inside temperature of the cabinet and its contents shall be provided. Solid state components mounted within the controller shall be designed to operate between 0 to 104 degrees Fahrenheit, humidity non‐ condensing up to 95 percent.
‐ All controller switches and relays shall have contacts of a design and material to ensure maximum conductivity, long life and reliable operation without overheating or excessive wear, and shall provide a wiping action to prevent sticking due to fusion. Switches carrying highly inductive currents shall be provided with arc shields or suppressors.
‐ Where time delay relays are used in the circuits, they shall be of acceptable design, adjustable and reliable. Condenser timing or electronic timing circuits shall be consistent.
‐ Each device in all panels shall be properly identified by name, letter or standard symbol and labeled in a legible manner with a thermal printed label. Identification markings shall be coordinated with identical markings used on wiring diagrams. The ampere rating shall be marked on all circuit protection devices. All conductors to and within controllers and supervisory panels shall be neatly formed, laced and identified.
‐ Elevator travel is not anticipated to be extended in the future so controllers shall not be provided with wiring and components to accommodate any future travel to additional floors.
‐ Printed circuit board made by the elevator controller manufacturer shall be kept to a minimum so that:

Longevity of components is assured and not compromised.
Hardware changes over the product lifetime are kept to a minimum.
The life of the product is not dependent on the life of the manufacturer.
They are the same across all product lines, to ensure the least amount of inventory.
All parts shall be available through the controller manufacturer or a third party vendor.
‐ The controller electrical drawing and bill of material shall be made available upon request from the end user.
‐ The controller shall provide smooth, step‐less acceleration and deceleration of the elevator, automatically and irrespective of the load in the car.
‐ All non‐conducting metal parts in the machine room shall be grounded in accordance with National Electric Code. All cabinets shall be securely attached to the building structure.
‐ Each device, module and circuit protection devices (with voltage and ampere rating) shall be identified by name, letter or standard symbol in an approved indelible and legible manner on the device or panel. Identification markings shall be coordinated with identical markings on wiring diagrams.
‐ Circuits shall assure fail‐safe operation and to prevent elevator movement should a component malfunction.
‐ All logic symbols and circuitry designations shall be in accordance with ASME and NEC standards.
‐ Wiring connections for operating circuits and for external control circuits shall be brought to terminal blocks mounted in an accessible location within the controller cabinet. Terminal blocks using pierce through serrated washers shall not be used.
‐ All wiring and conduit work shall be in accordance with NFPA 70.
9.2 Leveling System
The contractor shall furnish and install a new leveling device to automatically bring the car to the floor landings. The car shall automatically re‐level at each landing to correct over‐travel or under‐ travel or maintain the level, regardless of load on the car, rope slippage, or stretch of cables. The error in the stopping accuracy shall not exceed plus or minus 1/4 in. Hardware shall be constructed of stainless steel and be water resistant.
9.3 Traveling Cable
The contractor shall furnish and install a new travelling cable, it shall be flexible, jacketed, and suitable strain relief shall be provided. The traveling cable shall also contain 10% spare wiring for future use.
9.4 Hoistway Wiring & Hoistway Limits
Elevator hoistway entrances shall conform to the following:
Each hoistway door is equipped with a tamper‐proof interlock which shall be used by the controller to prevent operation of the car until doors are locked in the closed position as defined by ASME A17.1 and shall prevent opening of the door at the landing from the corridor side unless the car is at rest at the landing or is traveling through the leveling zone, or unless the hoistway access switch is used. Existing retiring cams for hoistway door interlocks shall continue to be used.
Longevity of components is assured and not compromised.
Hardware changes over the product lifetime are kept to a minimum.
The life of the product is not dependent on the life of the manufacturer.
They are the same across all product lines, to ensure the least amount of inventory.
All parts shall be available through the controller manufacturer or a third party vendor.
‐ The controller electrical drawing and bill of material shall be made available upon request from the end user.
‐ The controller shall provide smooth, step‐less acceleration and deceleration of the elevator, automatically and irrespective of the load in the car.
‐ All non‐conducting metal parts in the machine room shall be grounded in accordance with National Electric Code. All cabinets shall be securely attached to the building structure.
‐ Each device, module and circuit protection devices (with voltage and ampere rating) shall be identified by name, letter or standard symbol in an approved indelible and legible manner on the device or panel. Identification markings shall be coordinated with identical markings on wiring diagrams.
‐ Circuits shall assure fail‐safe operation and to prevent elevator movement should a component malfunction.
‐ All logic symbols and circuitry designations shall be in accordance with ASME and NEC standards.
‐ Wiring connections for operating circuits and for external control circuits shall be brought to terminal blocks mounted in an accessible location within the controller cabinet. Terminal blocks using pierce through serrated washers shall not be used.
‐ All wiring and conduit work shall be in accordance with NFPA 70.
9.5 Hoist Ropes
The contractor shall install new hoist ropes and shackles to meet the specifications of the machine manufacturer.
9.6 Car Top Operator
The contractor shall install a new Car Top Operator.
9.7 Rework Hoistway Piping as Necessary
The contractor shall replace any piping in the hoistway deemed unfit for service with a like type.
9.8 Rework Hoistway Door Interlocks
The Contractor shall rework door interlocks as necessary to bring elevator into current elevator standards. Equipment shall meet standards for wet locations.
10 Installation
10.1 Controller
All wiring, except short lengths (6 inches or less) and traveling cables shall be in approved raceway.
Short sections of wire which may require shifting for connection to switches and equipment may be installed in flexible conduit. Wires shall be identified and identification shall match the symbols shown on the schematics and wiring diagrams.
a. Control and signal wires shall be brought to accessible numbered terminal blocks on the controller.
b. Intra‐panel wiring shall be a flame resisting type.
10.2 Leveling System
The existing leveling system shall be removed and the new leveling system installed to the controller manufacturers specifications. If using a tape style leveling system the tape shall be stainless and the reader head shall be watertight in construction.
10.3 Traveling Cables
The contractor shall remove the existing traveling cable installed between their termination point on the car and their termination point at the existing controller. The new traveling cable shall be furnished, installed and secured so that stress relief for conductors is provided. Rungs are available on the car for hanging the multi‐conductor traveling cable(s).
10.4 Hoistway Wiring & Hoistway Limits
The contractor shall remove the existing hoistway wiring andinstall all new hoistway wiring.
Limits shall be removed and new installed.
10.5 Hoist Ropes
The contractor shall install and properly tension the hoist ropes of the type recommended by the machine manufacturer. The contractor shall also ensure the cables are properly tensioned prior to leaving the job.
10.6 Car Top Operator
The cartop operator shall be installed so easily accessible from the hoistway landing doors.
10.7 Rework Hoistway Piping as Necessary
Any piping that does not meet the size requirements for fill space or damaged piping shall be removed and new piping of the same type or quality shall be reinstalled.
10.8 Rework Hoistway Door Interlocks
Remove the existing door interlock components reusing only the body and cover, replacing all contacts, contact blocks, bearings, springs, and seals. Or remove the existing door interlocks and replace with new watertight interlocks to meet the current ASME A17.1 applicable codes.
10.9 Testing
The Contractor shall notify the Resource Manager in writing ten (10) days prior to the date of performing the acceptance test. The Contractor shall furnish all test instruments required for all testing. Any deficiencies noted during testing shall be corrected and the tests repeated. All costs associated with retesting shall be borne by the Contractor.
10.10 Test Period
The elevator shall be tested for a period of 30 minutes of continuous run with specified rated load in the car. During the test run, the car shall be stopped at all floors in both directions of travel for a standing period of 10 seconds per floor. A manual test of the final limits (up and down over travel) shall also be performed.
10.11 Speed Load Tests
The actual speed of the elevator car in both directions of travel shall be determined with the rated load and with no load in the elevator car. The actual measured elevator car speed with the rated load in the up direction shall be within 5 percent of rated speed. The contractor shall provide the weights for load testing.
10.12 Car Levelling Tests
Elevator car's leveling devices shall be tested in both directions of travel for accuracy of landing at all floors with no load in the car and with the rated load in the car. The error in the stopping accuracy shall not exceed plus or minus 1/4 inch as stated above.
10.13 Brake Test
The brake test shall be conducted with the rated load in the car. Brakes shall stop and hold the car with the rated load. Sufficient voltage shall be applied to the brake magnets so that pads do not drag on the braking surface.
10.14 Insulation Resistance Tests
The complete wiring system of the elevator shall be free of short circuits and grounds.
Conductors shall have an insulation resistance of not less than 10 mega ohm between each conductor and ground and between each conductor and all other conductors.
10.15 Operational Check
The contractor shall develop a checklist and submit for Contracting Officer approval. The checklist shall be partially comprised of all operations/features available with the existing elevator and any new features provided by the new controller. The checklist may be used during the 30 minute period of elevator operation described in test period above. The check shall incorporate all tests required to verify operation of all system functions. The Contractor shall furnish and install any lamps that may be required to ensure all lamps installed are working.
10.16 Over‐speed Test
The Contractor shall fully test and verify correct operation of all Over‐speed circuitry.
10.17 Warranty
The Contractor shall provide a warranty for the work described herein. The warranty shall cover the equipment furnished herein and shall provide for all parts and labor required to maintain the system in operation. The warranty shall be valid for a period of one calendar year from the date of acceptance of the equipment for beneficial use on behalf of the Government.
10.18 Changes in the Specification
The right is reserved as the interest of the Government may require, to revise or amend the specifications prior to the date set for opening bids. Such revisions and amendments, if any, will be announced by amendment or amendments to this solicitation. Copies of such amendments as may be issued will be furnished to all bidders. If the revisions and amendments are of a nature which require material changes or quantities or prices bid or both, the dates set for opening bids may be postponed by such number of days as in the opinion of the Contracting Officer will enable the bidders to revise their bids. In such cases, the amendment will include an announcement of the new date for opening bids.
10.19 Site Clean Up
The Contractor shall remove from the site, and legally dispose of all rubbish resulting from the work under this contract. If needed the Contractor shall provide a separate dumpster located on site as directed by the Resource Manager. Rubbish shall be removed daily and not allowed to accumulate or overflow the dumpster.
10.20 Safety
3. Contractor Safety Requirements
3.1. General.
Contractor shall comply with all Federal, State and Local safety requirements including those specified by the Occupational Safety and Health Act (OSHA) in 29 CRF 1910 General Industry Standard, 1926 Construction Standard and the U.S. Army Corps of Engineers Safety & Health Requirements Manual EM 385-1-1 as currently revised. Where a conflict exists between safety standards/requirements the contractor shall comply with the more stringent requirement. If the Contractor fails or refuses to promptly comply with safety and security requirements as specified herein, any project employee may stop all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such work stoppage shall be made subject to any claims for extension of time or for excess costs or damages sustained by the Contractor. Also, the Contractor will not be paid for work not performed as a result of the stop order.
3.2 Contractor Accident Prevention Plan (APP).
The Contractor shall prepare a site-specific Accident Prevention Plan following the outline provided in EM 385-1-1, Appendix A as a minimum. The contractor shall address each element and sub-element in the outline in the order they appear in Appendix A. Any item from the outline that does not apply must be listed, in its assigned place and a brief explanation of the reason it is not required/provided. The APP shall address any unusual or unique aspects of the project or activity. This plan must be accepted by the Contracting Officer prior to commencement of any work.

(1) The contractor shall have on site an employee designated as a Site Safety and Health Officer (SSHO) who has completed the OSHA 30-hour Construction Safety training or its equivalent as specified in EM 385-1-1, 01.A.17 (only authorized online outreach training providers qualify – https://www.osha.gov/dte/edcenters/map.html) and has at least five years experience relative to the work to be performed. No work will be permitted unless the SSHO is present at the work site.
(2) For task orders limited to routine mowing, cleaning and janitorial work SSHO responsibilities may be a collateral duty. Contractor may also elect to use an SSHO with either OSHA 30-hour Construction or General Industry qualifications.
(3) Work crews of two or more must have two people holding current certification in First Aid and CPR from the American Red Cross, American Heart Association or a licensed physician. Training must include a hands-on component that cannot be taken online and the contractor must maintain appropriate First Aid kit on site. Single individuals working alone must have current certification in First Aid as a minimum.
(4) All work that potentially exposes the worker to falls of six feet or more will require the contractor to submit a Site Specific Fall Protection & Prevention Plan in accordance with EM 385-1-1, Section 21.D.
(5) Cranes and hoisting equipment shall be operated only by qualified personnel (operators licensing, physical and drug testing in accordance with EM 385-1-1 Section 16.B). Note: CDL Medical Clearance is not acceptable. Please see attached medical clearance requirements.
(6) Effective November 8, 2010 all “Signalpersons” and “Riggers” shall be qualified. This is a mandatory requirement in accordance with 29 CFR 1926.1400 Regulations. Riggers must be a qualified person for the performance of specified hoisting activities such as during assembly/disassembly, in accordance with OSHA 1926.1404(r)(1) and 1926.1425(c)(3).
(7) Work orders involving potential exposure to Hazardous, Toxic or Radiological Waste shall require development of Site Specific Safety and Health Plan (SSHP) by qualified personnel as specified in EM 385-1-1 Sections 6 and 33.
3.3 Risk Management Process.
The Contractor shall utilize risk management tools such as Activity Hazard Analysis (AHA), Job Hazard Analyses (JHAs), Job Safety Analyses (JSAs) or similar tools to identify, assess, prioritize risks and develop controls to minimize, monitor and control the probability and/or impact of unfortunate events to an acceptable level. Within each major phase, all activities, tasks or Definable Features of Work (DFOWs) shall be identified that will require an AHA/JSA/JHA. For each work activity, task or DFOW contractor shall prepare the initial AHA/JSA/JHA. A Risk Assessment Code (RAC) is assigned to each step, to the risk that remains after controls have been applied (residual risk) a final RAC (cannot be lower than the highest step RAC) shall be assigned. See EM 385-1-1 Section 01.A.14.b. for further guidance or contact the SOHO for assistance. The AHA/JSA/JHA shall be reviewed and accepted by the proper authority in order to proceed with the activity. All on-site employees shall review the accepted AHA/JSA/JHA prior to performing work and during weekly and “Tool Box” safety meetings as appropriate.
1. AHA/JSA/JHA shall define the activities being performed and identify the work sequences, the specific anticipated hazards, site conditions, equipment, materials, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level of risk. AHA/JSA/JHA should indicate compliance with EM 385-1-1 personal protective equipment requirements (safety-toed work boots, safety glasses, gloves, hard hats, etc.), work crews of two or more must have two people currently qualified in first aid and CPR and the appropriate first aid kit on site. Single individuals working alone must have current certification in First Aid only.
2. The names of the Competent Person(s) and Qualified Person(s) required for a particular activity (e.g., excavation, scaffolding, fall protection, or other activities as specified by OSHA and this manual) shall be identified and included in the AHA/JSA/JHA, as well as proof of their competency/qualification.
D. Accident/Mishap Reporting: The Contractor shall report all recordable mishaps to the GDA within 24-hours in accordance with EM 385-1-1 Section 01.D. Accidents shall be investigated to determine causal factors and corrective actions shall be implemented. Accidents resulting in: days away injuries, days away illnesses, restricted/transfer injuries, property damage of $5,000.00 or more shall be investigated and reported to the GDA using the USACE Accident Investigation Report Form ENG 3394 within 24 hours of the occurrence.
(1) Immediate notification to the GDA is required for all serious accidents:
a. Fatal Injury/illness;
b. Permanent totally disabling injury/illness;
c. Permanent partial disabling injury/illness;
d. One (1) or more persons hospitalized as inpatients as a result of a single occurrence;
e. $500,000 or greater accidental property damage;
f. Three (3) or more individuals become ill or have a medical condition suspected to be related to a site condition, or a hazardous or toxic agent on the site;
g. USACE aircraft destroyed or missing;
The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300 and comply with all accident reporting and recordkeeping requirements specified in 29 CFR 1904.39.
(2) In addition to the above, any mishap occurring in any of the following high hazard areas shall be reported immediately to the GDA, investigated and corrective actions implemented.
a. Electrical (includes Arc Flash, electrical shock, etc.);
b. Uncontrolled Release of Hazardous Energy (includes electrical and non-electrical);
c. Load Handling Equipment (LHE) or Rigging;
d. Fall-from-Height (any level or other than same surface);
e. Underwater Diving
3.4 On-site Public Safety.
Work on this contract is immediately adjacent to public recreation facilities. Safety precautions to warn and limit public access shall be addressed in both the APP and AHA.
3.5 Damage Reports.
All instances where Government materials supplies, property, and/or equipment are damaged shall be reported immediately to the Resource Manager.
3.6 Accident Reporting.
The Contractor shall maintain an accurate record of ENG Form 3394 to report to the Resource Manager all accidents within 24 hours of the occurrence. All serious accidents (those resulting in death or injury requiring medical attention) shall be reported immediately. The Contractor shall maintain an OSHA Log of Work Related Injuries and Illnesses, OSHA Form 300.
3.7 Minimum Personal Protective Equipment.
Long pants, safety toe shoes/boots, and shirts with minimum 4 inch sleeves, hard hat, hearing protection as needed, and safety eyewear. Additional Personal Protective Equipment may be needed and should be determined using the Activity Hazard Analysis.
3.8 Pre-Work Conference.
The Contractor shall attend a pre-work conference, prior to the initiation of work under this contract. The Contractors Accident Prevention Plan must be accepted prior to this meeting.
3.9 Contractor Submittals
The successful contractor shall provide, as a minimum, the following documentation:
a. Accident Prevention Plan – EM 385-1-1 Appendix A and APP (Accident Prevention Plan). These forms are located at the following website: http://www.lrh.usace.army.mil/ct/general - all documents are located at the bottom of the page.
b. Activity Hazard Analysis – Template and “Example” forms are located at the following website:

APPENDIX A
MINIMUM BASIC OUTLINE FOR ACCIDENT PREVENTION PLANS
An Accident Prevention Plan (APP) is a safety and health policy and program document. The following areas are typically addressed in an APP, but an APP shall be job-specific and shall also address any unusual or unique aspects of the project or activity for which it is written.
The APP shall interface with the employer’s overall safety and health program, and a copy shall be available on the work site. Any portions of the overall safety and health program that are referenced in the APP shall be included as appropriate. ANSI/ASSE A10.38 should be referenced for Programmatic Issues.
> For LIMITED-SCOPE SERVICE, SUPPLY AND R&D CONTRACTS, for example, mowing (only), park attendant, rest room cleaning, the Contracting Officer and SOHO may allow an ABBREVIATED APP (customized APP requirements and waive the more stringent elements of this section). > See 01.A.11, and Appendix A, paragraph 11.
1. SIGNATURE SHEET. Title, signature, and phone number of the following:
a. Plan preparer (Qualified Person, Competent Person, such as corporate safety staff person, QC);
b. Plan must be approved, by company/corporate officers authorized to obligate the company;
c. Plan concurrence (e.g., Chief of Operations, Corporate Chief of Safety, Corporate Industrial Hygienist, project manager or superintendent, project safety professional, project QC). Provide concurrence of other applicable corporate and project personnel (Contractor).
2. BACKGROUND INFORMATION. List the following:
a. Contractor;
b. Contract number;
c. Project name;
d. Brief project description, description of work to be performed, and location; phases of work anticipated (these will require an AHA).
3. STATEMENT OF SAFETY AND HEALTH POLICY. Provide a copy of current corporate/company Safety and Health Policy Statement, detailing commitment to providing a safe and healthful workplace for all employees.
The Contractor’s written safety program goals, objectives, and accident experience goals for this contract should be provided.
4. RESPONSIBILITIES AND LINES OF AUTHORITIES. Provide the following:
a. A statement of the employer’s ultimate responsibility for the implementation of his SOH program;
b. Identification and accountability of personnel responsible for safety at both corporate and project level.
Contracts specifically requiring safety or industrial hygiene personnel shall include a copy of their resumes.
Qualifications shall include the OSHA 30-hour course or equivalent course areas as listed here:
(1) OSH Act/General Duty Clause;
(2) 29 CFR 1904, Recordkeeping;
(3) Subpart C: General Safety and Health Provisions, Competent Person;
(4) Subpart D: Occupational Health and Environmental Controls, Citations and Safety Programs;
(5) Subpart E: PPE, types and requirements for use;
(6) Subpart F: understanding fire protection in the workplace;
(7) Subpart K: Electrical;
(8 Subpart M: Fall Protection;
(9) Rigging, welding and cutting, scaffolding, excavations, concrete and masonry, demolition; health hazards in construction, materials handling, storage and disposal, hand and power tools, motor vehicles, mechanized equipment, marine operations, steel erection, stairways and ladders, confined spaces or any others that are applicable to the work being performed.
c. The names of Competent and/or Qualified Person(s) and proof of competency/qualification to meet specific OSHA Competent/Qualified Person(s) requirements must be attached. The District SOHO will review the qualifications for acceptance;
d. Requirements that no work shall be performed unless a designated competent person is present on the job site;
e. Requirements for pre-task safety and health analysis;
