STATEMENT OF WORK

FOR

B2045 Remove, Purchase and Install Roll-Up Doors

1. DESCRIPTION. Contractor shall provide all necessary labor, tools, equipment and materials to purchase and replace existing commercial overhead coiling door system and two rolling aluminum security grille systems, to include the door/grilles, associated operators, controls, hardware and electrical system connections at Building 2045, see Appendix A to reference building and door locations. Upon completion the contractor will provide fully operational door systems as described in this Statement of Work. All work shall be performed in accordance with all Federal, State, local laws and regulations, manufacturer’s recommendations and requirements set forth in this Statement of Work (SOW).

2. SCOPE OF WORK. The Contractor shall provide and install all material, equipment and services including labor, unless otherwise specified herein, required under this contract.

i. Coordinate disconnect of electrical power system of existing door operators at least two days prior to start of work with 92 CES point of contact, Mr. Michael Ehrman at 509-247-5562.

ii. Remove existing commercial door system(s) and dispose of materials at an approved off-site location.

iii. Prepare opening to accommodate new door installation. Door and curtain salient characteristics are provided in Appendix A.

iv. Furnish, deliver and install 3 each door systems. See Appendix A for salient characteristics.

v. Furnish, deliver and install a standard 3-button wall station, electric sensing safety edge, and/or photo electric safety eyes (sensors).

vi. Coordinate reconnection of electrical power system for new door operators with 92 CES point of contact. Adjust all controls and safety devices to ensure doors are fully operational.

vii. Contractor shall ensure installation is complete by installation presenting a neat, clean and professional appearance with no defects that allow weather to infiltrate the facility.

viii. Contractor shall cleanup work area at end of each work day and upon completion of work.

3. GENERAL INFORMATION

3.1 HOURS OF OPERATION: Hours of operation are 0700-1630 Monday through Friday, excluding Federal Holidays and any other designated down day. Work can be done at other than the normal hours of operation with the approval of the Contracting Officer. The contractor shall coordinate in advance all hours of work and any service interruptions with Civil Engineering points of contact.

3.2 SECURITY REQUIREMENTS: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the government installation, shall abide by all Fairchild Air Force Base security regulations.

3.3 GOVERNMENT FURNISHED PROPERTY AND SERVICES:

The Government shall furnish a fully qualified Electrician to perform electrical power disconnect prior to removal of existing door systems and connection of electrical power to the new operators with coordination made at least 2 workdays prior. The Government shall furnish site access, restroom facilities, and electrical power. In the event of an emergency, dial 911.

4. CONTRACTOR HAZARDOUS MATERIAL MANAGEMENT:

4.1 CONTRACTOR HAZARDOUS MATERIALS. In accordance with AFMAN 32-7002, Environmental Compliance and Pollution Prevention, a hazardous material includes all items covered under EPCRA tracking requirements, the OSHA HAZCOM standard, and all Class I and II Ozone Depleting Substances. Hazardous materials includes all material hazardous to health, safety, or the environment, such as but not limited to, hazardous chemicals, hazardous substances, toxic chemicals, and extremely hazardous substances, as defined by any of the following: 29 Code of Federal Regulation (CFR) 1910.1200; 40 CFR Part 302, Table 302.4; 40 CFR Part 355 with Appendices; and 40 CFR 372.65.

The Contractor shall use non-hazardous materials whenever possible. The Contractor shall be responsible for the proper storage, management, and disposal of any hazardous material brought on Base for contractor use.

4.2 HAZARDOUS MATERIAL STORAGE. For any single hazardous material stored on-site in a quantity equal to or exceeding 55-gallons at any time, the contractor shall provide secondary containment for the entire quantity of that hazardous material. Hazardous material shall be stored within a locked secure area not accessible to the base populace after hours. All containers of hazardous material shall be clearly and properly labeled; closed when not in use; and stored in an upright, secure position.

4.3 HAZARDOUS MATERIAL AUTHORIZATION. Hazardous materials must be approved by the HMMP Team prior to being brought onto the installation. The Contractor shall submit a list of all hazardous materials intended for use, along with corresponding Safety Data Sheets (SDS), to the HMMP Team, through the Contracting Officer (CO), at least fourteen (14) days prior to beginning work or bringing hazardous materials onto the installation. The listing shall include product name, chemical type, and quantity to be brought onto the installation, process for which it will be used, and disposal method for empty containers and any resulting hazardous or nonhazardous waste. The Contractor shall report how and when approved hazardous materials are used, including any amount spilled, evaporated as an air emission, or properly disposed of as solid or hazardous waste. Special reporting may be required at the Contractor's expense. The Contractor shall submit this information to the Government POC, prior to using hazardous materials (HAZMAT), using a Contractor Hazardous Material Work Sheet and corresponding SDS for each hazardous material to be brought onto the base during the performance of the contract. This work sheet and associated SDSs shall be completed for each and every hazardous material to be used by the Contractor. If during the course of the contract, items not previously identified are to be brought on the base, the Contractor shall submit a Work Sheet and SDS for approval prior to bringing the HAZMAT onto the base.

4.4 HAZARDOUS MATERIAL USAGE. Within seven calendar days of completion of the contract, the Contractor shall submit to Government POC, the quantities of each hazardous material actually used on the base during the course of the contract, utilizing and annotating the government-provided chemical authorization list, Contractor Hazardous Material Worksheet or other approved document. The authorization list can be obtained from 92 CES/CEIE via the Government POC.

4.5 HAZARDOUS MATERIAL REMOVAL. The Contractor is responsible for removal of their hazardous materials. No hazardous material shall be left on-site at the end of a contract.

5. NON-HAZARDOUS SOLID WASTE.

5.1 NON-HAZARDOUS SOLID WASTE DISPOSAL: Non-hazardous solid waste must be managed and disposed in accordance with federal, state, and local regulations at facilities permitted in accordance with the WA State Solid Waste Handling Standards.

5.1.1 SALVAGING, RECYCLING, AND DISPOSAL OF WASTE

5.1.1.1 The Contractor shall make every reasonable effort to reduce the amount of solid waste generated by salvaging and recycling materials to the greatest extent possible. Materials capable of being recycled, shall be recycled at facilities licensed and permitted by the county or state for such recycling. The Base Recycling Center may be available to accept corrugated cardboard (flattened) for recycling.

5.1.1.2 The Contractor shall be responsible for the proper handling and disposal of all solid waste accumulated or generated on FAFB in performance of the contract. All non-hazardous solid waste shall only be disposed of and/or recycled at facilities licensed and permitted by the county or state for such disposal or recycling. There are no disposal areas on the base.

5.1.2. NON-HAZARDOUS SOLID WASTE DISPOSAL PLAN

5.1.2.1 A plan for non-hazardous solid waste disposal shall be submitted to the Contracting Officer for approval prior to physical work NTP. Such a plan shall provide a project name; project number; contract number; a brief description of the project; summarize the activities that will generate waste; itemize all waste (including waste or material to be salvaged and/or recycled) expected to be encountered during the project; how the materials will be managed on base; and include proposed disposal/recycle locations and corresponding applicable Solid Waste Permit(s) showing the disposal/recycle facility is licensed and permitted. A Non-Hazardous Solid Waste Disposal Plan Template can be found at the end of this section.

5.1.2.2 If, during the course of the project, the specifics of the Non-Hazardous Solid Waste Disposal Plan change (e.g., materials disposed/recycled, disposal/recycling facilities used, etc.), then an updated plan must be submitted to the Contracting Officer for approval prior to implementing the changes.

5.1.2.3 The contractor shall ensure that all subcontractors, vendors, suppliers follow the approved Non- Hazardous Solid Waste Disposal Plan. It is the contractor's responsibility to coordinate with subcontractors to ensure awareness and participation.

5.1.2.4 Approval of the Contractor's Solid Waste Disposal Plan will not relieve the Contractor of responsibility for compliance with applicable environmental local, state and federal regulations

5.1.3 SOLID WASTE DISPOSAL TRACKING

5.1.3.1 All solid waste including salvaged and recycled materials shall be tracked by the type, quantity, weight, (including totals for each column), and disposition. This information shall be submitted on the Solid Waste Disposal Tracking Sheet (found at the end of this section) to the Contracting Officer within 7 days of the end of each calendar quarter (i.e. submit in January, April, July, and October) and at project completion.

5.1.3.2 Provide dump weight receipts to the Contracting Officer for all disposed solid waste.
