Instructions to Offerors and Evaluation Criteria Replace Lockers, Building 140

Goodfellow AFB, Texas

PROPOSAL SUBMISSION INSTRUCTIONS

The Contractor shall submit a written price and technical proposal for consideration before the due date and time for this RFP. The proposal shall include the following:

(1) A Cover Letter that provides the following information:

a. Contract Number

b. Small Business Status/Categories as appropriate

c. Company point of contract with email, phone and facsimile number

d. CAGE Code

e. DUNS number

f. A statement that your company understands the requirements specified in the Statement of Work (SOW)

g. A statement that your company does or does not take exception to any of the requirements of the SOW or terms/conditions of this task order.

h. Technical information

(2) Price Proposal:

a. Contractors must submit pricing on CLIN 0001

(3) Past Performance Proposal:

a. Offerors shall submit the following information:

i. Corporate qualifications and capabilities that fully describe its ability to provide the required system equipment, software, and system support to the Government by submitting a company profile.

ii. A list of NO MORE than three (3) completed projects of similar complexity, scope and magnitude, including contract number(s), award date(s), completions date(s), contract value(s), functional performance specification(s), contractual and technical

Atch 2 (Page 2 of 4) contact(s) with telephone number(s), and a comparison of actual completion date with contractual date. Completed projects must be from within the past five (5) years.

(4) Technical Proposal:

a. Offerors shall submit the following information:

i. Simple preliminary design

ii. Specifications as required in the SOW that will be used in the design and install of the system

iii. Documentaion shall address all technical criteria listed in the table below

iv. Provide separate technical criteria for each CLIN.

1 CLIN 0001 – Meets all standards outlined in the SOW.

EVALUATION CRITERIA

The Government anticipates awarding a task order to the lowest priced proposal that meets all technical criteria with an acceptable past performance. Proposals will be evaluated based on the factors described below.

The Government will evaluate proposals based on the following factors:

(1) Technical:

a. The Replace Lockers will be evaluated using the following Technical Criteria

Checklist. Proposals must meet all technical criteria to be deemed technically acceptable. Proposals not meeting these requirements will not be considered for award.

Proposals must be rated technically acceptable to the eligible for award. In order to be deemed technically acceptable the above criteria must be met.

b. Offerors shall provide the technical methodology/approach to be used in performing the requirements of the SOW.

c. Offerors must specifically identify the approach/technology proposed, along with the stands utilized and the tools and methodologies of how the solution/approach is developed. The Government will evaluate the approach/solution to ensure it is viable.

The characteristics of viable solutions (in conjunction with the other evaluation factors) will be used to determine the proposal which provides the best value to the

Government.

(2) Past Performance:

a. Past Performance will be evaluated on a satisfactory/unsatisfactory basis per the below chart. The Government reserves the right to base past performance evaluations on customer surveys, and past performance questionnaires, Supplier Performance

Risk System (SPRS) reports, or the Contractor Performance Assessment Reporting

System (CPARS).

Past Performance Evaluation Rating Criteria

SATISFACTORY Based on the offeror’s performance record, the

Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

UNSATISFACTORY Based on the offeror’s performance record, the

Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

b. The Government may consider past performance information from any other reliable source including, but not limited to, Past Performance Information Retrieval System

(PPIRS) and its own experience with the company.

c. NOTE: In the case of a vendor without a record of relevant past performance or for whom information on the past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the vendor may not be evaluated favorably or unfavorably based on past performance. Therefore, the vendor shall be determined to have unknown past performance. In the context of satisfactory/unsatisfactory, “unknown” shall be considered “SATISFACTORY”.

(3) Price:

a. The total evaluated price (TEP) will be calculated by summing up the price of all

CLINS. Prices will be evaluated for reasonableness. Prices which are excessively high or low may be considered unrealistic and unreasonable and may receive no further consideration.

ASSUMPTIONS, CONDITIONS & EXCEPTIONS

NOTE: Any exception to the solicitation may render a proposal unacceptable.

If a Contractor takes exception to any of the requirements of the SOW or terms/conditions of this task order, they must be clearly identified in your proposal.

These will be reviewed for potential impact on your ability to successfully meet the

Government’s requirements. Any assumptions, conditions or exceptions which may negatively impact performance of this requirement could render the proposal ineligible for award.

BASIS FOR AWARD

The Government will award a task order to the lowest priced proposal that meets all technical criteria with a “SATISFACTORY” past performance evaluation.

The Government will evaluation the pricing for proposals as follows:

(1) The offeror’s price proposal will be evaluated for award purposes, based upon the total price proposed for all priced CLIN(s). The Government will first conduct an initial review of the overall proposal pricing and rank the offerors from lowest-to-highest based on the Total Evaluated Price (TEP).

(2) Failure to comply with these provisions may constitute sufficient cause to justify rejection of a proposal.
