Statement of Work

1.0 DESCRIPTION OF SERVICES

The required services will take place at various entrance locations at both Patrick Space Force Base and Cape Canaveral Space Force Station as per Unified Facility Criteria (UFC) 3-120-01.

Service required is to design, fabricate and install eight (8) entrance signs. A total of four (4) signs at PSFB and four (4) signs at CCSFS. The scope of this project requires the use of tool capabilities such utilizing 3-D technology and black coating capabilities that is currently not available within the squadron to complete this task.

2.0 SERVICES SUMMARY

Utilizing industry means and methods, licensed contractor provides all lift equipment, tools, trained labor and material to perform services that is being requested. The scope of work should be accomplished within 30 days by the contractor. This requirement will include the design (as per attached UFC 3-120-01, Air Force Sign Standard), fabrication, transportation to both bases (PSAB & CCSFS) and installation of:

1. Cape Canaveral (4x10) QTY 2 New Face Panels 4' x 10' .125 Aluminum Sheets with full coverage Black Powder Coating Lettering:

• U.S. SPACE FORCE 6" mounted

• Cape Canaveral Space Force Station 4" mounted

• 24" Logo Stud mounted

• 1/2" Brushed aluminum boarder

2. Cape Canaveral (5x12) QTY 1

New Face Panels 5' x 12' .125 Aluminum Sheets with full coverage Black Powder Coating Lettering:

• U.S. SPACE FORCE 6" mounted

• Cape Canaveral Space Force Station 4" mounted

• 33" Logo mounted

3. Cape Canaveral (3x10) QTY 1

New Face Panels 3' x 10' .125 Aluminum Sheets with full coverage Black Powder Coating Lettering:

• U.S. SPACE FORCE 6" mounted

• Cape Canaveral Space Force Station 4" mounted

• 24" Logo mounted

4. Patrick Space Force Base (5x10) QTY 1

3.0 GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES.

The Government will not provide access to water supply and power source.

4.0 GENERAL INFORMATION:

4.1 LOCATION AND HOURS OF WORK.

Services contained in this SOW requires work at the following locations:

Patrick Space Force Base

1. Entrance to installation on South Patrick Dr.

2. North Perimeter Entrance of A1A and Pineda Beach (Off of Pineda Causeway)

3. East Gate Entrance to installation off of Highway A1A

4. South Perimeter Entrance of A1A off the wall by water plant

Cape Canaveral Space Force Station

1. Entrance to installation on FL 401 (Cape Canaveral Parkway)

2. Entrance to installation between the Kennedy Space Center and CCSFS (NASA

Parkway).

3. Entrance to installation on FL 402 (Kennedy Parkway).

4. Air Terminal sign (Skid Strip) All work will be performed on hours; 7:00 am to 5:00 pm, Monday through Friday.

Period of performance for this project is 30 days. Reporting to and from work should include the necessary time of scheduling of vehicle inspections at both installations.

4.2 BADGING AND IDENTIFICATION.

All contractors to include their direct employees, subcontractors and subcontractor employees entering both Patrick Space Force Base and Cape Canaveral Space Force Station are required to have an individual badge prior to beginning work at the facilities.

Therefore it is essential the contractor completes 45th SFS Credential request form SFS V10 (Subject to Privacy Act of 1974) visitor’s information section.

All employees to include sub-contractors employees who will be working on PSFB and CCSFS must provide the completed forms to the Contracting Officer Representative (COR) within 15 days of being awarded the contract.

Once the COR receives the 45 SFS credential request form it will take approximately 3 Federal working days before a short term entry badge can be prepared. The contractor will be notified by the COR when each employee is to report to facility 577 badge and identification office for photographs and issuance of a badge.

All employees must bring TWO (2) forms of identification and all vehicles cleared with updated Florida registration and motor vehicle insurance.

4.3 Safety and Personal Protective Equipment (PPE).

Contractor shall comply with all applicable Federal, State and local legal requirements regarding workers health and safety. The requirements include but are not limited to those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and1960. Contractor is solely responsible for determining the legal requirements that apply to activities, and shall ensure safe and healthful working conditions for its employees. The Act’s General Duty Clause, Section 5(a) (1), requires employers to provide their employees with a workplace free from recognized hazards likely to cause death or serious physical harm. The Occupational Safety and Health Act requires employers to comply with safety procedures providing fall protection--personal protective equipment not limited to gloves, hard hat, and safety toe shoes.
