Statement of Work Southwestern Indian Polytechnic Institute (SIPI)
Medical Nursing Services 11 May 2023
1. Scope of Services
The contractor will provide general nursing services and medical emergency services to students, staff, and faculty. Services are needed to address COVID related symptoms, provide temperature readings, triage, and referrals for other medical issues. The contractor shall provide Nursing services in the area of prevention and intervention and shall provide training in health education.
School Hours of Operations and Contractor Work Hours:
• School Hours of Operation: 10 am to 2 pm
• The contractor is expected to work five (5) days a week for estimated period of 4 hours per day.
• The contractor is expected to work approximately 1,040 hours per year (but could be as much as 2,080).
• The actual work schedule will be provided by the government upon award.
2. Consultants Tasks and Responsibilities
The contractor shall provide all labor, materials, transportation, and incidentals necessary to perform the work. The contractor shall have sufficient professional knowledge, skill, and experience in Nursing Management.
The principal tasks and responsibilities are:
A. Provide Medical Services on the SIPI Campus.
1. The contractor shall provide the medical personnel and medical equipment necessary to provide medical services to students on SIPI campus.
2. The contractor will provide audiometer, stethoscope, blood pressure cuffs, vision charts and scales.
3. The contractor shall provide preventative health educational training.
4. The contractor shall provide medical referrals for students when necessary.
5. The contractor shall provide medical expertise and triage to students and make appointments with students.
6. The contractor shall work closely with the Director of Student Affairs on recordkeeping to ensure compliance and privacy of student records.
7. Due to the sensitivity of medical records, and privacy, the contractor is designated as the keeper of record.
B. Services.
1. General services include health plans, screening of immunization records, vision screening, stereo acuity, hearing testing, medical evaluation and training, height-weight.
2. Additional services include educational health trainings to students on diabetes, birth control, drug awareness, breast cancer awareness and exams, blood borne pathogen education, first aid/CPR training, medication evaluation training and COVID awareness and training.
3. COVID services will include COVID-19 testing and screening, referrals, and vaccinations if available.
4. Services will include IEP evaluations for students with disabilities.
5. Services include the establishment of wellness and health policies, update state and federal health requirements in schools including COVID-19 and providing an annual health service report.
6. Maintain implementation of student health records.
7. Services include setting up an isolation room and supervision of health assistants.
8. Will provide services for student appointment and arrangement of student transportation, assist in finding funding for student health, and encourage parent participation.
9. Services will include health fairs, drug awareness week, vaccine clinics and will have a notary on staff.
3. Objectives
The goal of this effort is to increase Native American participation and success in higher education by ensuring students have the opportunity to complete required courses in a timely and cost-effective manner for the Federal Government. This contract will ensure the improved access, retention, and success of Native American students in higher education in addressing student, staff, and faculty healthcare needs.
4. Deliverables to SIPI
The following final deliverables are required on or before June 14, 2024 (and the last day of the period of performance for any subsequent exercised Option Years):
· Report in digital form with the following information and supporting documentation regarding the medical services provided to SIPI:
o Summary of all activities (submit weekly reports).
o Assessments of the number of health trainings, medical referrals, and number of students and staff seeking contractor services as walk-ins or by appointment.
o Recommendation for additional medical services for students.
· Report in digital form with the following information and supporting documentation, regarding the services provided.
o Summary of medical services provided within a health service report.
o Assessments of the medical health plans, educational health trainings, and IEPs provided to students.
o Student wellness and health policies o Update on state and federal health requirements
5. Government Furnished Property
SIPI will accommodate the needs of the consultant by providing access to the necessary
IT equipment, office space and furniture, triage room and other resources available within the Student Affairs Division.
6. Security
The contractor will have to complete the Federal background check as required. The consultant will ensure the highest level of data security, and privacy, and will utilize data solely for the purpose of completing the scope of work and validating the quality and accuracy of the completed work.
7. Place of Performance
The Nursing Contractor will be working with Dr. Cecelia Cometsevah. Project activities will occur primarily at the Student Success Center, Bldg. 101 on Southwestern Indian Polytechnic Institute’s campus in Albuquerque, New Mexico, as deemed appropriate or necessary.
