About this file

This document is a past performance questionnaire template for a solicitation seeking carbon analysis services for a period of eight months. Offerors are required to submit between three to five completed past performance questionnaires with their proposals by the date and time specified. The National Park Service contracting office will evaluate proposals and intends to award a contract to improve carbon analysis. The past performance questionnaire requests details on up to five relevant prior projects completed within the last five years, including description of work, value, ratings for quality, timeliness, and an overall satisfaction rating from the reference. It collects contact information for the project owner to verify past performance information as part of the evaluation.
