About this file

This document contains a past performance questionnaire template for evaluation of contractors responding to a Peace Corps solicitation. The solicitation seeks health care benefits services and requires completion of the past performance questionnaire by references for evaluation of offerors' past performance. The questionnaire requests ratings on a scale of Excellent to Unsatisfactory for the offeror's performance in areas such as compliance with contract requirements, adherence to schedule and quality standards, and responsiveness in managing changes. It also asks the reference to rate the knowledge and skills of the offeror's staff, ability to satisfy customers, and timeliness in contract transition. The reference is asked if they would do future business with the offeror and provides space for additional comments. Completion of the questionnaire by the offeror's references will allow evaluation of past performance as part of source selection for the Peace Corps health care benefits services contract.
