Statement of Work Visual Acuity Eyecharts VA Maryland Health Care System Baltimore, MD
1. PURPOSE:
The overall purpose of this requirement is to procure the Lombart CVS iMac Acuity System or equal with the idea that an acuity system should be easy to operate, while still including the standard acuity charts commonly used in refraction which will be utilized in Surgical Clinical Care Center, (Ophthalmology/Optometry). This increases access and improved patient care; units will provide more accurate vision measurement and do not require light bulbs as the current projectors do. Digital units have multiple vision charts that can be inter-changed and can test for color vision, contrast sensitivity, stereo vision, eye turns, and are also controlled by a remote control.
Increases access and improves patient care by adding five (5) units at Loch Raven, two (2) units at Glen Burnie, two (2) units at Cambridge, two (2) units at Fort Meade. Eleven (11) in total.
2. SCOPE:
2.1. The vendor shall deliver and install the system with all specified salient characteristics. Only new equipment will be accepted.
2.2. The vendor shall provide shipping, transport, installation, and all equipment necessary to meet the requirements and capabilities listed within the scope.
2.3. The vendor shall deliver the system to the Baltimore VA Medical center.
2.4. The assembly and validation of the system must be performed by a vendor service trained representative.
2.5. SALIENT CHARACTERISTICS: Please note, all items listed below must be technically equivalent to or exceed the Lombart CVS Essential.
2.5.1. Eleven (11) Lombart Advance Eyecare Visual Acuity Eyecharts - Lombart CVS Essential
2.5.1.1. Manufacturer: Lombart Advance Eyecare
2.5.1.2. Model: (CP0LOCVSEFG)
2.5.1.2.1. Devices must have Pediatric Optotypes.
2.5.1.2.2. Devices must include ETDRS.
2.5.1.2.3. Devices must include fixation videos.
2.5.1.2.4. Devices must include HOTV.
2.5.1.2.5. Devices must include the ability to import videos and images.
2.5.1.2.6. Devices must include Astigmatic Fan.
2.5.1.2.7. Devices must include Red/Green.
2.5.2. Accessories
2.6. IMPLEMENTATION AND INSTALLATION
2.6.1. Installation and validation shall be included in the procurement cost and shall include but not limited to:
2.6.2. The contractor will deliver the following quantities of eyecharts and accessories to the addresses below:
2.6.2.1. Baltimore VA Medical Center, 10 North Green Street, Baltimore, MD 21201
2.6.2.1.1. Lombart Digital Acuity System
2.6.2.1.2. Accessories
2.7. TRAINING
2.7.1. The vendor shall provide training for all VAMHCS staff that requires training, as needed.
2.8. DELIVERY
2.8.1. The contractor shall deliver necessary equipment to the Baltimore facility as required in the quantities specified above.
3. INSPECTION AND ACCEPTANCE
3.1. The vendor shall conduct a joint inspection with the COR upon delivery equipment.
3.2. The vendor shall provide the VA COR a list of missing and /or damaged items found upon inspection and have those items replaced at no cost to the Government.
3.3. The COR shall ensure all work is completed satisfactorily prior to acceptance. Disputes shall be resolved by the VA Contracting Officer.
4. DELIVERABLES
4.1. Operation and Maintenance Manuals
4.1.1. Deliver compilation of all manufacturers recommended maintenance schedules and operation materials packaged upon delivery. Manuals shall be made available in an electronic format.
