Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity: Department of Veterans Affairs, VISN 22, New Mexico Medical Center 2237: 501-21-2-119-0084
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The NMVAHCS GYN/surgical services requests procurement of the following equipment from Hologic to include the “Myosure” (hysteroscope) and “Novasure” (ablation) devices as well as the “Fluent” fluid management system and extended 3-year warranty.
Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Hologic package includes the Myosure (hysteroscope), the Novasure (ablation) devices, and the Fluent fluid management system. The Hologic package offers the safest option for female veterans for several reasons:
The Myosure procedure offers a morcellating head a more accurate and less invasive tissue removal process that targets and removes fibroids and polyps.
The Myosure procedure can be used with a normal saline solution which reduces the risk of fluid overload and electrolyte abnormalities.
The Novasure procedure offers a safe option for female veterans who require an endometrial ablation. The Novasure treatment utilizes unique patented technology to deliver customized ablation to each patient.
The Novasure procedure emits precisely measured radio-frequency energy to safely remove the endometrium, the uterine lining that sheds during a patient’s period The Novasure procedure can be used with a normal saline solution which reduces the risk of fluid overload and electrolyte abnormalities.
Description of market research conducted and results or statement why it was not conducted:
Market research was conducted and included using GSA/VA Advantage, Vendor Information Pages (VIP), the System for Award Management (SAM) and researching recent Hologic purchases on FPDS.
Since Hologic is the sole manufacturer of the required medical equipment, and not a VO / SDVOSB, called to nquire if they had any small business partners who could distribute their products. w informed that they have a distribution authorization relationship with two SDVOSB concerns. forwarded a letter on company letterhead stating that two SDVOSB concerns, DGA Medical, and Sustainment Technologies, LLC were authorized to distribute the required equipment. he VIP website to verify that both companies are currently certified SDVOSB concerns. Both companies were verified on the website. Per the Non-manufacture decision tree, a SDVOSB set-aside for this requirement.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. .
