Attachment B Statement of Work
1. Introduction The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the vendor in supplying the reagents and service for the Affirm testing in the Microbiology section of the Pathology & Laboratory Medicine Department for VISN22.
1.1 Background
The Department of Veterans Affairs, VISN22 Pathology & Laboratory Medicine Service, requires a vaginitis panel that is compatible with the BD Affirm microprocessor. The BD Affirm microprocessor is currently a validated instrument within all of the VISN22 laboratories. Obtaining the requested reagents will allow for VISN22 to continue to test for vaginitis for our veterans.
2. Scope The vendor shall supply kits that are equal to or equivalent to the vaginitis panel performed on the BD Affirm.

· These kits allow specimen DNA probe-based diagnostic test for the differential detection and identification of the causative agents for vaginitis: Candida species, Gardnerella vaginalis and Trichomonas vaginalis.

Salient Characteristics
· Vaginitis panels must be 100% compatible with all features, must physically fit without alteration, and not void the warranty of the current VA-owned Becton Dickinson Affirm Microprobe Processor 120 instrument system.
· Vaginitis panels must allow specimen DNA probe-based diagnostic test for the differential detection and identification of the causative agents for vaginitis: Candida species, Gardnerella vaginalis and Trichomonas vaginalis.
· Vaginitis panels must be FDA approved.
· Vaginitis panels must not negatively impact system performance, assay results, safety, and equipment life cycle of the current VA-owned Becton Dickinson Affirm Microprobe Processor 120 instrument system.
· Ambient temperature transport system kit must provide a minimum of 72-hour ambient temperature transport and stability.
· Pricing must include all handling, HAZMAT, dry-ice and shipping fees when applicable.
3. Hours of Operation Normal tour-of-duty hours is 7:30 am to 5:00 pm, Monday through Friday, excluding Federal holidays. However, the service is operating 24/7 and as such there may be instances in which the vendor’s workday may be outside of this period. The vendor is required to coordinate these times with the designated Program Manager.
The locations listed below are most sites that can request the product, however additional locations may request product. This requirement must have an open ship to, so we can send supplies as needed directly to the end user.
This will be ordered on an “as needed” basis to other sites within the VISN 22.
