SECTION I

DESCRIPTION OF SERVICES/GENERAL INFORMATION

1.0. Scope of Work. The contractor shall provide all personnel, tools, supplies, and equipment, materials, repair and replacement parts, transportation and communication necessary to inspect, maintain and repair Mechanized Swing and Sliding Gate systems located within the perimeter of Wright Patterson AFB OH (See Appendix B). The contractor shall provide on-site semi-annual inspection/preventive maintenance and authorized emergency and routine repair service. All maintenance and repair work performed under this Performance-based Work Statement (PWS) shall be in accordance with industry/commercial practices as well as all local, state, and federal regulations. Ensure areas are clean, orderly and free of debris upon completion of each service. If manufacturers’ recommended parts are not available, replacement parts shall be equal to or better than the manufacturer’s recommended part. Technicians performing work under this PWS shall be trained and have a minimum of 2 years certifiable experience inspecting, testing and maintaining mechanized Swing and Sliding Gate systems.

1.1. Preventive Maintenance. The contractor shall perform all required inspections and maintenance of the

Mechanized Swing and Sliding Gate systems to ensure the system is fully operational 24/7 unless otherwise approved by the WPAFB Contracting Officer (CO)/Contracting Officer Representative (COR). Preventive Maintenance shall be performed according to Original Equipment Manufacturer standards and directions.

Preventive Maintenance shall be performed semiannually and shall include all tasks recommended by the Original Equipment Manufacturer. Preventive maintenance shall also include all tasks listed in Appendix D, "Minimum PMI Inspection/Maintenance Items” that are not already covered by the Original Equipment Manufacturer's recommendations in order to keep all mechanized swing and Sliding Gate systems in a fully operational and maintained condition. Mechanized Swing and Sliding Gate system component description is identified as: Foundation, Gate, Gate Electrical Control Panel, Safety detection and warning systems, Vehicle Loop Detection system (if applicable), chains, brakes, Electrical Power Unit (including motor, gearbox, hoses, and fittings), and accompanied components to include card readers/keypad access.

1.1.1. Preventive Maintenance Inspection (PMI) Program. The contractor shall submit a PMI plan to the CO/ COR for approval within 10 calendar days of award. The purpose of the PMI program is to prevent or correct deficiencies with the Mechanized Swing and Sliding Gate systems, minimize equipment breakdowns and service interruptions, extend component service life, and maximize operating efficiency. The PMI plan shall include equipment components to be inspected and maintained, inspection and maintenance techniques (adjustment, lubrication, repair, replacement, inspection, and testing of the swing and sliding gate system and all associated mechanical devices, and safety equipment), inspection and maintenance frequencies, and reporting methodology. The PMI plan shall also include a checklist that incorporates at a minimum the items identified in Appendix D. The contractor shall update PMI plan as necessary to reflect any changes in equipment inventory.

1.1.2. Maintenance and Repair Parts. Maintenance and repair parts are classified as either Major or Minor. Nuts, bolts, cotter pins, shear pins, filters, grease, paint, turnbuckles, assorted relays, activation switches, hydraulic hoses, pressure gauges, and other items are considered Minor parts if they cost $100 or less per piece or item. Minor repair parts costs shall be absorbed by the contractor. Major parts would include items that cost over $100 per piece or item. The cost of Major parts, with prior approval, will be paid to the Contractor.

1.2. Service Call Repairs. The Contracting Officer (CO) will designate in writing, individuals authorized to place service calls to the contractor for repair services. Service calls may be classified as routine or emergency, and the assigned designation will be provided to the contractor with each service call. The contractor shall establish and maintain a 24 hour, 7 day a week capability to receive service call requests. Prior to commencing any repair work the contractor shall provide a cost estimate to the COR. The cost estimate shall include all labor, equipment, material, and supervision required to accomplish major repair services. Parts list pricing from suppliers shall be submitted with all proposals to the COR for all repairs. Parts to be replaced by the contractor shall be Original Equipment Manufacturer (OEM) new and certified. The contractor shall certify the parts are compatible with the specific equipment and are fully warranted by the original manufacturer. Information about the parts shall be submitted to the COR for warranty verification. No work shall be performed until authorized by the Contracting Officer (CO) or COR.

1.2.1. Routine Service Calls. Routine service calls apply to repairs not classified as an emergency. The contractor shall coordinate and schedule service with the COR within 4 hours of notification. Typically routine work shall be performed in conjunction with semi-annual PMI service. In the event a routine call must be performed at a time other than in conjunction with the semi-annual PMI service, the routine work shall be completed within 4 business days after receipt of a routine service call.

1.2.2. Emergency Service Calls. Emergency service calls will be placed for the repair of a mechanized swing or sliding gate system condition that constitutes an immediate danger to personnel, threaten property, or require action to restore essential services (Force Protection). The contractor shall coordinate and schedule service with the CO/COR within 1 hour of notification and shall be on the job site within 4 hours. The contractor shall work continuously until swing and sliding gate systems are fully operational or the condition is downgraded to a routine service by the CO/COR. Emergency service calls may be placed outside normal working hours.

1.3. WARRANTY PERIOD

1.3.1 Contractor shall warrant all labor and materials provided in accordance with the contract, for a period of one (1) year from date of work completion.

1.3.2 Contractor shall warrant swing/sliding gates that have had a major repair as described in section 1.1.2 against defects in labor and materials and, a warranty shall be provided for a period of one (1) year.

1.3.3 Warranty Response - The contractor shall respond within four (4) hours of notification by the Government of a warranty call. The request for warranty repair shall be made during regular work hours (Monday-Friday, 6 AM to 6 PM) but the contractor shall be prepared and available to respond outside normal duty hours at no additional cost to the government.

1.4. Hazardous Waste Materials. The contractor shall transport and dispose all hazardous/non-hazardous contents or materials from the mechanized swing or sliding gate systems to an approved off-base facility. All waste discharge manifests must be signed by the 88 CEG/CEI office, Bldg. 30022 prior to any off-site waste disposal. The contractor shall maintain and have available upon request, all documentation associated with the sampling analysis, transportation and destruction of hazardous and non-hazardous materials.

1.5. Maintenance Reports and Records.

1.5.1. Reports. The contractor shall submit accurate and detailed electronic reports documenting each service or repair performed to the COR via email within 5 business days from the date of service or repair. Reports, as a minimum, shall identify the specific mechanized Swing and Sliding Gate systems, specific location, date of the inspection, date and time of service, name of the inspector, name of repair technician, overall condition of the mechanized swing and sliding gate systems, man-hours by labor rate, work performed, and any other pertinent or useful information.

1.5.2. Records. The contractor shall maintain accurate, current, legible records for each mechanized Swing and

Sliding and Gate systems IAW commercial standards and practices. Each record shall include a checklist of all PMI, repairs and maintenance, scheduled versus actual completion dates, the person(s) who performed the work, and any other recommended work. The contractor shall provide an electronic copy of the maintenance records to the COR via e-mail within 5 business days from the date of the service or repair.

1.6. Hours of Operation. Normal duty hours for Wright-Patterson AFB is 7:00 a.m. to 5:00 p.m. Monday through Friday, except federal holidays and snow and emergency days. The contractor may elect to work daylight hours beyond normal hours or on Saturdays with coordination and prior approval of the government representative at no additional cost to the government.

1.7. Contract Manager. The contractor shall identify a contract manager who is authorized to represent the contractor and make binding contractual agreements with the government. Provide the contact information for the contract manager and alternate(s), to the CO prior to the contract start date and shall be updated as needed.

The contract manager or alternate(s) shall be available by phone or radio within 1 hour to discuss any problem areas, issues, or concerns. The contract manager or alternate(s) shall read, write, speak, and understand English.

1.8. Traffic Control. Security Forces will be solely responsible for all traffic control operations during all mechanized Swing and Sliding Gate systems inspection, testing and maintenance operations. At no time will a contractor direct traffic. The contractor shall be responsible to contact Security Forces to establish a time to meet with designated Security Forces patrols. Only after Security Forces arrives on scene and initiates traffic control may testing begin. The contractor shall develop and provide an annual schedule and a document outlining written procedures technicians will use to safely raise, lower, or operate the mechanized Swing and

Sliding Gate systems to the CO/COR for acceptance 10 days following to the contract start date.

SECTION II

SERVICES SUMMARY

2.0. The contractor service requirements are summarized into performance objectives that relate to mission essential items. The performance thresholds briefly describe the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

Performance Objective PWS para. Performance Threshold Mechanized Swing/Sliding Gate System Preventive Maintenance and Repair Work.

Provide required inspections, maintenance and repairs IAW PWS.

1.1.

No more than 2 customer complaints per month.

Mechanized Swing/Sliding Gate System

Preventive Maintenance/Inspection (PMI) Plan. Provide PMI Plan for review and approval

1.1.1.

No deficiencies permitted.

Routine/Emergency Service Calls. Respond on-site within time specified in SOW and approval and complete required work.

1.2.1 and 1.2.2

Warranty Period Respond within four (4) hours after notification by government.

1.3

Maintenance Reports and Records.

Ensure records are accurate, current, and provided to the COR at the completion of each service.

1.5

No more than 2 discrepancies per month.

Quality Control Plan Maintain a quality control program to ensure maintenance and repair services are performed.

2.2

No valid customer complaints permitted.

2.1. Contracting Officer Representative. The CO will appoint a primary and alternate (COR) for surveillance of the day to day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the contractor after contact award.

2.2. Quality Control Plan. Submit a quality control plan (QCP) to the COR and CO for approval within 10 calendar days of award and changes to the plan. The QCP shall ensure that all Mechanized swing and sliding gate maintenance and repair services are performed in accordance with applicable commercial standards, regulations, laws and codes, and contractor’s PMI Plan. The QCP shall describe the overall quality approach and quality control inspection system tasks to identify and prevent defective services from occurring or recurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the Services Summary and an internal inspection/surveillance system. Maintain all quality control records for the duration of the contract and make them available to the government COR or CO within 24 hours upon request. Provide quality control inspector(s) contact information to the CO and COR for notification of customer complaints, government concerns, or emergency situations. This listing should contain current names and phone numbers, and be updated when changes occur. The plan shall be maintained and updated as necessary throughout the life of this contract.

2.3. Quality Assurance. The government will inspect and evaluate the contractor’s performance to ensure services are received in accordance with requirements set forth in this contract. The COR shall inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, and otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. The COR will use the contractor’s work schedule or modified version thereof, to record surveillance results. Results of the surveillance then become the official Air Force record of the Contractor’s performance. When a performance threshold has not been met or contractor performance has not been accomplished, the COR will initiate and provide to the CO a Corrective Action Report (CAR), SAF/AQCP Template Feb 05, Attachment 1 to the Performance Plan, for issuance to the contractor. The contractor shall respond to the CAR IAW instructions provided and return it to the CO within 10 calendar days of receipt.

2.4. Surveillance Methods.

2.4.1. Periodic Surveillance. This method requires the COR to employ a “spot check” style of evaluation based on the contractor’s schedule. Periodic surveillances will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted to 100% surveillance, based on quality trends. Any unsatisfactory inspection (defect) result shall be recorded, and the Contractor shall reperform the service after notification by the COR. Failing to meet the performance threshold as outlined in this PWS for any of these performance objectives in any one month period shall result in a warning or letter of concern from the CO. Failing to meet the performance threshold as outlined in this PWS of these performance objectives in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate Progress Meeting with the multi-functional team (consisting of COR, Contractor, or CO, if required).

2.4.2. 100% Surveillance. This method requires the COR to inspect the service each time it occurs. Results shall be annotated on the inspection schedule. Any deficiency shall be documented and the contractor shall reperform service immediately, if appropriate, or within 24 hours at no increase in contract amount. Any unsatisfactory inspection identified but re-performed acceptably shall still be counted as an unsatisfactory inspection for trending purposes.

2.4.2.1. Receiving two or more unsatisfactory 100% surveillances within a 12 month period may result in unsatisfactory past performance documentation. Continued receipt of unsatisfactory 100% surveillances during the contract period shall constitute a negative trend and the CO may take any appropriate action in accordance with the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.

2.5. Quarterly Performance Meetings. The Contractor shall be available to meet periodically with the CO and multi-functional team members to discuss any issues with the performance of the services. The COR shall provide a written summary of unsatisfactory inspections and validated customer complaints and provide insight into any identified trends. Minutes will be signed by the CO and distributed.

SECTION III

GOVERNMENT-FURNISHED PROPERTY AND SERVICES

3.1 Government-Furnished Property (GFP). There is no GFP or Facilities provided on this contract. The contractor shall be responsible for the storage and safekeeping of all material to be used in this work.

3.2. Services.

3.2.1. Security Forces. The Government will provide general on base Security Forces services. Security Forces telephone extension is (937) 257-9111 for emergencies and (937) 257-6516 or (937) 257-6517 for routine calls.

3.2.2. Fire Prevention and Protection. The Government will provide fire prevention and protection. The Fire Department telephone extension is (937) 257-9111 for emergencies and (937) 257-4075 for routine calls.

3.2.3. Emergency Medical Service. The government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract. The contractor shall reimburse the government for the cost of medical treatment and patient transportation service at the current in-patient or outpatient treatment rate, as appropriate.

3.3 VOLUNTARY PROTECTION PROGRAM (VPP)

3.3.1 APPLICABILITY: This section of the PWS applies to "Applicable Contractors" as defined below.

3.3.2 DEFINITIONS:

Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant (installation).

Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.

Total Case Incidence Rate (TCIR) - Total number of recordable injuries and illness cases per 100 full-time employees that a site has experienced in a given time frame.

Voluntary Protection Program - The Voluntary Protection Program (VPP) promotes effective worksite-based safety and health. In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.

3.4 REQUIREMENTS – VOLUNTARY PROTECTION PROGRAM

Wright Patterson AFB is in the process of pursuing VPP recognition and has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all "applicable contractors" operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at: http://www/osha.gov/dcsp/vpp/index.html.

It is the contractor's sole responsibility for compliance with the Occupational Safety and Health Act (OSHA) (Public Law 91-596). The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor's plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:

• Demonstrate a management commitment to employee safety and health;

• Identify the application of the safety and health plan to subcontractors;

• Identify the roles and responsibilities of the following individuals with respect to safety and VPP:

Management, Supervisors, Employees, and Safety Coordinator;

• Identify applicable safety rules and regulations;

• Include a worksite hazard analysis to include base-line hazard identification and required control measures;

• Include a job site analysis to include hazards of tasks required to control measures;

• Identify employee safety and health training requirements and the documentation process;

• Include a workplace inspection frequency, to include identifying the individual conducting the inspections;

• Include employee hazard reporting procedures;

• Identify individual(s) responsible for corrective action of hazards;

• Identify first aid/injury procedures;

• Identify procedures for accident investigation and reporting;

• Identify emergency response procedures; and

• Identify the process for tracking controlled hazards in contractor work areas.

The contractor shall submit their Total Case Incidence Rates (TCIR) and Days Away, Restricted, or Transfer (DART) rates and OSHA Form 300A annually to the Contracting Office for consolidation and submission as part of the installation's annual VPP Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.

The contractor's Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.

3.4.1. The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.

3.5 LABOR HOURS DATABASE. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year.

Contractors may direct questions to the Contractor Manpower Reporting Application (CMRA) help desk.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

3.6 Antiterrorism (AT) Awareness Training

LEVEL I ANTITERRORISM (AT) AWARENESS TRAINING

As members of “Team Wright-Patt” contractors and their employees are subject to the same or greater risk of becoming a victim in a terrorist act or inadvertently providing terrorists with a means to target Wright-Patterson Air Force Base. The Department of Defense (DoD) Standard 22 mandates that the Air Force OFFER Level I

AT Awareness Training to all contractors and their employees, under the terms and conditions specified in this contract. You and your employees are strongly encouraged to complete the computer based AT Awareness Training found at http://jko.jten.mil/courses/atl1/launch.html.
