About this file
This is a Past Performance evaluation attachment for a Request for Quotation (RFQ) for Water/Wastewater Lab Testing Services issued by the Department of Agriculture Agricultural Research Service under solicitation number 1232SA26Q0097. The document outlines the Government's evaluation criteria and submission requirements for assessing an offeror's ability to perform the required services based on demonstrated past performance.
Quoters must submit two to three past performance projects from contracts, order vehicles, or BPAs similar to or larger in size, scope, and complexity than the requirements outlined in the Statement of Work. Projects must be recent (within the last three years) and relevant to the effort. Each submission must include the project title, description of scope and responsibilities, contract/order/BPA number, contract type and period of performance, contract award amount, Government agency, Contracting Officer's contact information (name, address, and phone number or email), Contracting Officer Representative's contact information, project status, and a narrative explaining relevance. At least two references must demonstrate the quoter's performance as prime contractor, while additional references may be from subcontractor experience. Current point of contact information is critical, as outdated information may negatively impact evaluation. Past performance will receive a Go or No-Go rating based on the quality of prior performance and probability of success on the required effort. The Government reserves the right to obtain information from other sources to supplement submitted references.

Text version
Past Performance The past performance evaluation factor assesses the degree of confidence the Government has in an Offeror’s ability to provide services that meet Users’ needs, based on a demonstrated record of performance.
Quoters shall provide two (2) to three (3) projects describing their past performance on the projects under contract vehicles similar (or larger) in size, scope, and complexity to those requirements outlined in this RFQ. Recent is defined as within the last three years. Relevant is defined as work similar in complexity and magnitude of the work described in this Statement of Work.
Past Performance Submissions should include the following information:
§ Project title § Description of the project, scope, and responsibilities § Contract/Order/BPA number § Contract type and period of performance
· Contract Award Amount § Government Agency/Organization § COR’s name, address, and phone number § Contracting Officer’s name, address, and phone number or email address § Status (e.g. complete, ongoing, etc.)
§ A brief narrative of why the reference is relevant to this effort
At least two of the references provided shall be based on the Quoter’s performance as the prime contractor, and the others may be from subcontractors. Please make sure the point of contact information is current – failure to provide current information may have an adverse impact on the Government’s ability to evaluate your quote.
The Government may also consider information obtained through other sources. Past performance information will be utilized to determine the quality of the contractor’s past performance as it relates to the probability of success of the required effort and will be given a Go or No-Go rating.
