SCOPE OF WORK
SHOEMOBILE
SERVICE CONTRACT
April 2020
1. The safety shoe mobile will come to DLA Distribution Red River Texas, Texarkana, TX, Bldg. 499 on the first and third Tuesday of each month from 12:30 to 4:30.
2. The Government may reschedule times and dates for shoe mobile service providing advance notice to the contractor within ten (10) days of the desired change.
3. The Government may request five (5) unscheduled visits per year with ten
(10) Calendar days' notice to the contractor to accommodate safety shoe issues for newly hired personnel or for emergency issues.
4. A listing of safety footwear issued during each visit of the shoe mobile shall be compiled by the contractor along with receipts signed by the employee receiving shoes and will be delivered to DLA Distribution Red River POC:
Mr. Jason Weaver, bldg. 499, (903) 716-3010. These listings must include the employee's name, badge number, shoe size, style and price of shoes issued/purchased. The total dollar amount of all shoes issued/purchased during the visit and the total number of pairs of shoes shall be included.
5. The Government subsidy for the purchase of safety shoes is $125.00 for any style shoe that is on the GSA schedule. Any charges incurred by the employee over this amount during the purchase of safety footwear shall be paid in cash by the employee and collected at the time of the sale by the contractor. The Government will accept GSA and open market shoes.
6. Authorized employees will submit a DDRR form signed by their supervisor to be admitted to the shoe mobile for service.
7. All shoes offered must meet ANSI Standard Z41 requirements.
