Statement of Work (SOW)
Project: HVAC Controller System Replacement
GENERAL INFORMATION
1.0 Background: Many Farms High School building 1369 heating, ventilation and air conditioning (HVAC) system controller has reach it end of life and in need of replacement. The current HVAC software can no longer communicate with the control module that adjusts the temperature within building 1369.
The high school supports 9th -12th students and staff, the current HVAC system controller is no longer cost efficient and is not providing adequate heat and cooling for the classrooms and offices. The school HVAC is in need of a full reinstallation of software, computer hardware, pumps, building controller, sensors, thermostats, and values that control the heating and cooling of the classrooms and offices.
2.0 Scope of Work:
Item 1. Contractor will provide all necessary hardware, project management, engineering, programming, graphics, installation, point-to-point, startup/checkout, training, and warranty for a complete, turnkey system HVAC control system for building 1369. This includes replacement of HVAC system controller, support devices to the controller that connected to the HVAC are to be updated and replaced, as well as computer hardware that control the school HVAC system controller. The intent is to have a single point integration interface for the control of building 1369 HVAC system.
The programming should include hardware, software and maintenance, user training both on site and on-line training of the HVAC system. Including testing, and documentation of the HVAC network diagram and all configurations of the HVAC system. This will include travel and per diem to install the HVAC system for Many Farms High School.
Item 2. Contractor will provide all supervision, labor, materials, testing equipment, and transportation to survey the entire HVAC system within building 1369. This would likely include all pumps, motors, HVAC Units, Control Valves, Fan Motors, chillers, dampners, VAV boxes, etc.
Item 3. Contractor will provide all supervision, labor, materials, equipment, and transportation to provide a detail report of all the HVAC system equipment within building 1369 that requires repair or replacement as a result of the survey conducted at item 2. The report shall be provided in excel. The repot shall contain the make, model and serial number of the equipment requiring replacement along with an estimated cost to replace as well as an estimated cost to install the equipment. A detailed narrative shall be included describe the basis of the labor and materials costs provided.
2.1 Location: The project will be performed at Many Farms High School -BIE
Arizona Navajo Central Chinle. The physical address is Hwy 191 Lake View Dr, of Many Farms, AZ 86538.
CONTRACTOR REQUIREMENTS
3.0 General Requirements: Responsible for providing technical assistance and support installations of HVAC system controller in building #1163 with full support of troubleshooting and training for the HVAC system controller
The contractor shall furnish all labor, equipment, materials, employees, and services, insurances to perform the work at Many Farms High School and shall be responsible for the professional quality, technical accuracy, and timely completion of the contract.
The contractor shall proceed to the Many Farms High School Administration office and check-in during schedule days of performances.
The contractor shall install new HVAC system controller and provide remote troubleshooting if necessary or over the telephone services.
The contractor will manage technical vendor relationships and work with vendors to solve technical issues
The contractor will move, add and changes to HVAC controller.
The contractor must include any adjustments to classroom temperature.
The contractor must include equipment with limited warranties The contractor must deliver technical assistance to Many Farms High School. The support and delivery will be properly coordinated in the professional matter.
There will be meeting(s) called, if necessary, to discuss problems or issues that need resolutions.
The contractor shall notify Many Farms High School, Information Technology
Department or Administration Office if there is non-service, and the invoice must be adjusted accordingly or delays of delivery services.
The contractor shall notify the Many Farms High School, Information Technology Department or Administration Office of any changes in the cost of services prior to making changes to the cost.
The contractor shall coordinate all scheduling or changes in delivery or services, scheduling with Many Farms High School, IT Specialist or Principal.
The contractor must ensure their performance in providing service is in a safe, professional matter at any time. Shall contact Many Farms High School of any unsafe or unprofessional conduct immediately.
3.1 Technical Requirements: The Contractor shall provide in accordance with 25 U.S.c. 3201 (P.L. 101-630), Indian Child Protection and Family Violence Prevention Act;42 U.S.c. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duties and responsibilities involving regular contact with or control over Indian children are to a background investigation and determination suitability for employment. A clearance is required to conduct work at the school, this may take up to 60 days to accomplish.
4.0 Government Furnished Equipment & Information: The Contractor shall furnish all labor, equipment, materials, employees and services, insurance to perform all the technical support services at Many Farms High School.
The Federal Task Lead works with the COR and will serve as the day-to-day decision-maker to assign or prioritize tasks. The Contractor assigned will receive work assignments from the Federal Task Lead and shall provide the Federal Task Lead with recommendations in regards to work assignments.
The Contractor shall provide customer relation services to ensure high customer satisfaction. The Contractor shall provide customers with the ability to voice feedback through a Customer Satisfaction Survey. Unresolved complaints shall be forwarded to the Federal Task Lead and if unable to resolve, the Federal Task Lead will escalate the complaint to the COR.
5.0 Deliverables / Schedule:
5.1 Period of Performance: 120 Days From Date Of Award
The contractor is required to provide a documentation of the installation and configuration of the HVAC controller and test report that are functional with BIE IT requirements.

8.0 Security Requirements: A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of the Indian Education Programs (OIDP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract.
