STATEMENT OF WORK (SOW)
SPECIAL EDUCATION SERVICES FOR SPEECH SERVICES
Tuba City Boarding School
1. BACKGROUND
This requirement falls under the Individuals with Disabilities Education Act (IDEA), 34 Code of Federal Regulations § 300.323. IDEA requires a free appropriate public education to eligible children with disabilities throughout the nation and ensures special education and related services to those children.
2. SCOPE
Contractor shall provide qualified personnel, necessary supplies, materials, equipment and testing materials to perform speech pathology services. The Contractor will be required to perform speech/language therapy for all students referred for evaluation during the course of the contract period. The contractor will be expected to:
✓ Be present during initial placement or multidisciplinary team committee meetings for student evaluated by the contractor
✓ Meet with and advise the special education and classroom teacher in matters regarding the continuous assessment, evaluation and implementation of speech therapy programs for individual students and will meet with individual students on a regular basis to provide suggestions for carryover of therapy strategies into the home when necessary
✓ After being provided a copy of the parental permission form for assessment by the school, the contractor shall:
o Assesses students’ communication skills (e.g., articulation, language fluency, voice, etc.) for the purpose of determining their speech and language deficits and developing recommendations o Consults with teachers, parents, other personnel and/or outside professionals for the purpose of providing requested information, developing plans for services and/or making recommendations o Instruct students for the purpose of implementing goals for re-mediation of speech and language deficits o Participate in meetings: IEP meetings and multidisciplinary team meetings for the purpose of developing plans and/or providing information regarding student’s functional goals o Prepare documentation (e.g., evaluations, observations, progress, contacts with parents, teachers, and outside professionals, etc.) for the purpose of providing written support, developing recommendations and/or conveying information o Present information on various topics related to area of professional expertise or the purpose of communicating information and gaining feedback on treatment issues o Research resources and methods (i.e., intervention and treatment techniques, assessment tools and methods, community resources, etc.) for the purpose of determining appropriate approach for students’ speech and language services o Screen students for the purpose of determining and assessment according to laws regarding minority non-discriminatory practices
Services shall be provided 4 times a week for 8 hours a day for a estimated 40 weeks throughout the school year and coordinated with the school.
3. KEY PERSONNEL MINIMUM REQUIREMENTS
The Contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote/proposal.
EDUCATION: Masters Degree in Speech Language Pathology
CERTIFICATION(S): Certificate of Clinical Competence Speech-Language Pathology (CCC-
SLP) (preferred)
LICENSE(S): AZ State License for Speech Pathology
Replacement of key personnel will require prior coordination and approval of the school.
4. DELIVERABLES
The Contractor shall provide the following deliverables within the specified timeframe:

5. PLACE OF PERFORMANCE
Contractor shall provide services at Tuba City Boarding School which is located at 306 Main
Street. Contractor may bill travel if included and funded in the contract. Incurred costs to travel for performance shall be pre-approved by a Government authorized representative. Travel costs shall be billed at the current year’s mileage and per diem rates in accordance with the Federal
Travel Regulations (FTR).
6. PERIOD OF PERFORMANCE
The period of performance shall begin on July 1, 2022 - June 30, 2023 with 5 optional years.
