STATEMENT OF WORK

PROJECT INFORMATION

1.1 PROJECT LOCATION: Project shall be completed as a routine requirement at building 4974. Base Hospital Central Plant, on Joint Base Elmendorf-Richardson, AK. Project hours are standard work hours.

1.2 GENERAL PARAMETERS

PROJECT WORK: Repair by replacement, the above ground duplex sump pump configuration with a submersible duplex sump pumps. Pumps with a minimum 100 GPM flow rate, 1.5HP, 3PH, 460V, submersible guide rail system, and duplex control panel with floats, HOA, audible alarm, run indicators, and thermal overload protection. The pumps shall operate in lead/standby fashion. Upon failure of the lead pump, the standby pump will be enabled, and the lead pump disabled. The pump designated as the lead pump shall rotate weekly. The existing branch circuit shall be reused to power the new control panel and pumps locate the new control panel at the same location as the existing panel, provide all new submersible controls cabling minimum #18 AWG copper. Provide new submersible power cabling per the pump manufacturer’s recommendation, minimum #12 AWG copper conductors.

The new pump motors shall have the following features: The pump motors shall be class B of the submersible type, oil filled, and hermetically sealed. The three-phase motors shall have overloads incorporated into the control panel properly sized for the horsepower and amperage of the pumps. The rotor shaft shall be made of 303 stainless steel and shall be supported by upper and lower ball bearings. The power cord shall be of the quick-disconnect design.

The pumps control panel shall have the following features: Duplex panel controls two 3-phase pumps in water and sewage installations. This panel shall include a duplex controller for pump control, alternation and alarm; including float switch status LEDs, control/alarm power ON/OFF switch with LED indicator, pump run LEDs, HOA switches, pump lead/lag selector switch, auxiliary contacts and more. The control panel features built-in lag pump delay time, pump failure detection, and float switch out-of-sequence notification. The panel shall have the following field programmable operations: alarm steady-state or flashing; alarm auto reset or manual reset; pump failure notification. The enclosure shall be minimum: 12" x 10" x 6"; NEMA 4X – ultraviolet stabilized thermoplastic for outdoor use. Provide the following features:

Magnetic Motor Contactor: Controls pump by switching hot electrical lines. HOA Switches: Offer manual operation of pumps (on circuit board). Green Pump Run Indicators: On circuit board Control ON/OFF Switch: On circuit board Float Switch Terminal Block: On circuit board Control/Alarm Auto Reset Fuses * Multi-tap transformer (208/240/480 VAC primary) to provide 120V control voltage. Motor protective switch provides adjustable overload, branch circuit protection and pump disconnect. Control/Alarm Power ON Indicator: On circuit board. Float Switch Status Indicators: Stop, lead, lag/alarm, and alarm mounted on a circuit board. Provide float switch push-to-test buttons. Circuit Breakers: Provide pump disconnect and branch circuit protection. Provide required connection terminal blocks, auxiliary contacts, and ground lug. The alarms shall have a NEMA 4X red beacon and horn (83 to 85 dB), exterior test/normal/silence switch.

The Contractor may have to modify the 4” drain line to facilitate the connection to the 3” pump discharge line/pipe. This project will require replacing two 4” isolation valves and two 4” check valves with 3” valves and 3” piping that connects into the existing 4” drain line. Some new submersible duplex pumps have a 3” discharge and the existing drain line is 4” in building 4974. The duplex sump pumps will need to be sewer grade, as there is a bathroom that drains into sump tank.

A. Modify and remove as necessary, any piping below the WYE piping connection, to accommodate new 3” piping configuration, see pictures and drawings.

B. Disposal of demolished pipe and valves in an off base approved land fill recycling center.

C. Clean out any sediment in the wastewater collection sump tank of building 4974 before new duplex pumps are put back into operation.

D. Ensure the hospital does not lose any softened water capacity during this project. Arrange for any necessary bypass pumping, as the facility will remain open to staff and patients.

E. Submit 95% drawings prior to work for approval. Provide 100% electrical design drawings stamped by a registered electrical engineer for approval prior to performing electrical work. Provide as-built drawings that reflect all modifications to facility, and provide offset dimensions showing location of newly installed pumps and modified drain line.

1.3 UNIQUE REQUIREMENT: Project is to be implemented while accommodating user needs of the facility. All contractor personnel shall be U.S. Citizens. The introduction of non-U.S. Citizens into this work area is strictly prohibited and shall be grounds to terminate the contract at no-fault to the Government. Contractor shall request approval of any downtime/outages a minimum of 10 workdays in advance. Of particular importance is the necessity to protect users and occupants of the facility from falling debris and life safety issues. Contractor staging areas must be secured against theft and in a manor to prevent the introduction of unauthorized electronic devices affixed/embedded to any contractor installed materials. Contractor staging areas will be identified and approved by the Fire Department, and 673 CES/CENMP. Safety areas and caution notices are to be provided by the Contractor.

SECTION 2: GENERAL REQUIREMENTS

2.1 PROJECT WORKING HOURS: Project hours are standard work hours.

2.2 QUALITY CONTROL:

A. The Contractor shall provide full-time onsite supervision.

B. All demolished materials shall be disposed of according to State & Federal regulations.

2.3 CODE/CRITERIA REQUIREMENTS: In addition to any codes, criteria, or regulations cited throughout this Statement of Work (SOW) and/or the attached Specifications.

Contractor must comply with the requirements below. If the codes, criteria, or regulations contradict each other regarding a specific requirement, the Contractor is to use the most stringent requirement.

2.4 WORKMANSHIP: All work shall be accomplished in accordance with all applicable building and fire codes. Contractor shall field verify all measurements, quantities, or values stated by the Government. Liquidated damages shall be assessed for work that is not finished within the scheduled performance period.

2.5 WORKSITE: The Contractor shall limit the work to the areas indicated by the Government. The Contractor shall make every effort not to disturb or obstruct portions of the facilities still occupied by the Government. The Contractor shall coordinate and schedule work with the 673 CES/CENMP office prior to the start of any operation.

The Contractor shall dispose of all construction debris off base at an approved disposal facility. The main entry to the facility shall be maintained at all times during the construction project. Contractor is to maintain vigilance for life safety of facility users entering and leaving the facility by posting signs and barriers for safety.

2.6 SITE SECURITY AND PROTECTION: The Contractor shall make every effort to secure the work site, allowing only authorized personnel to enter the work site. The Contractor shall protect the site from damage from weather, vandalism, and/or theft.

2.7 DAMAGE TO EXISTING SITE CONDITIONS: The Contractor shall be responsible for repairing any and all damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the Government. Contractor shall not park or drive on sidewalk or grassy/snowy areas. Damage incurred shall be repaired at no cost to the Government.

2.8 HEALTH/SAFETY: The Contractor shall be responsible for compliance with OSHA and Alaska Department of Labor (AKOSH) standards. The Contractor shall maintain a clean and debris free site at all times. The Contractor shall keep driveways and entrances into and around the work site clear at all times. The Contractor shall not use driveways or entrances as employee parking or storage of materials unless otherwise stated. The Contractor shall minimize storage of materials and equipment on site.

2.9 PROJECT SCHEDULE: The Contractor shall prepare a baseline project schedule incorporating all aspects in support of completing the project. Consideration shall be given to the sequence of phases (including review periods and meetings), standard construction/repair activities and duration of tasks, submittals, and procurement of materials. The schedule shall include material submittal dates coordinated with the AF Form 66, and allow for review periods prior to material procurement (Including identification of items with long lead times), and climatic conditions.

A. The project schedule shall be in the form of an AF Form 3064 chart.

B. The initial baseline schedule shall be available for review at the pre-construction meeting. All schedule updates shall be provided to the Government.

2.10 PROJECT STATUS UPDATE / WORK SCHEDULING MEETINGS: At the request of the Contracting Officer, project status/scheduled meetings may be held at a mutually agreed upon day, time, and location on JBER.

2.11 WASTE MANAGEMENT: Provide suitably sized dumpster, rubbish handling and removal services to ensure that all waste is contained and not subject to dispersal. All hazardous waste will be managed in accordance with Environmental Specifications Section 1600 Management of Hazardous Waste.

2.12 TOXICS: Manage and dispose of contaminated waste: (a) PCB in accordance with 40 CFR 761, (b) asbestos in accordance with 40 CFR 61, (c) lead in accordance with 40

CFR 745.

2.13 STAGING/MATERIAL STORAGE: Reference Specification, Section 01100 General Site Requirements & Section 01500 ENVIRONMENTAL PROTECTION ON JBER and 01600

ENVIRONMENTAL PROTECTION ON JBER MANAGEMENT OF HAZARDOUS AND SOLID

WASTE. The Contractor shall not block roads or access lanes, utilize parking lots, building entrances, or any area needed for potential utility servicing during the execution of this project.

2.14 HAZARDOUS MATERIAL USAGE: Reference Environmental Specification Section

2.15 EXISTING CONDITIONS: Facility sump pumps dispose of treated softened water from base hospital, which is currently occupied and shall remain fully functional during the course of this contract. Coordination and planning for shutting down the hospital water system for any length of time during this change over will be critical.
