Statement of Work: Demolition and Mold Remediation Services Conte Research Laboratory, Turners Falls, MA
1. Introduction: The Conte Research Lab (CRL) has a requirement to perform mold remediation services in a locker room in the Fish Passage Complex (FPC), including demolishing walls and treating adjacent surfaces for mold contamination. The space will be reconfigured in the future, this project does not include any reconstruction. The Facility Operations Specialist for the CRL, Sam Parker, will be the Contracting Officers Representative and main point of contact for this project.
2. Background: The locker rooms in the FPC are comprised of two identical locker room and shower stalls separated by a partition wall and a toilet room with a sink. Roof leaks and leaks from mechanical equipment caused mold to form behind wallboard and is now visible on the surface.
3. Scope of Work:
a. Isolate contaminated work areas and institute negative air pressure with negative air HEPA equipment. Work areas are 3 bathrooms/shower/locker room in main building displaying water damage and/or growth. (Target areas)
b. Demolish and dispose of all contaminated gypsum, insulation and wall tiles in target areas in main building as needed. Remove all contaminated gypsum and wall tile in substantially contaminated shower area.
c. HEPA vacuum and apply Fiberlock “Shockwave” (or equivalent) cleaner/disinfectant to all exposed areas by removed components. Accessible areas with visible growth will be manually scrubbed. Repeat HEPA vacuum process.
d. Treat and HEPA clean all subsequent exposed areas and components with “Advanced Peroxide Solution, Benefect, RMR and/or Shockwave” (or equivalent) as staining dictates.
If additional work outside this scope becomes necessary to leave the area in a safe, healthy state it will be performed only after a modification to the contract is executed.
4. Applicable Documents:
• Contractor and all site personnel will be licensed to perform remediation services in the Commonwealth of Massachusetts.
• Contractor will provide SDS sheets and either paper or electronic versions of product information for all treatments used in the remediation.
• All equipment will have state inspection stickers and will be in sound mechanical condition. Equipment will not leak oil or other fluids and have all emission control devices installed and functional.
• All equipment operators will be licensed in the Commonwealth of Massachusetts to operate their respective equipment.
• Contractor will furnish a report in electronic form detailing conditions observed and pictures of work area before and after demolition, and all contaminated surfaces before and after treatment.
5. Technical Requirements:
• All debris from demolition and construction will be cleaned daily, either stored in an approved container provided by contractor or removed from site daily. All work areas will be swept or vacuumed, and all hazards removed daily. Tools, equipment and materials will be stored neatly in designated areas. Barriers and signage will be used to prevent access to hazardous conditions by staff members and the public.
• There is approximately 1300 square feet of wallboard and 150 square feet of ceramic tile that will removed from wall and ceiling framing. The wallboard is 5/8” thick and fastened with screws to steel stud framing.
• All wiring will be de-energized by facility staff but will be left in place.
Armored steel cable, conduit, receptacles, switches and light fixtures are present and must not be damaged during demolition.
• Piping associated with the showers, sink and toilet will be present and must also not be damaged. All fixtures will be removed and disposed of.
• All steel stud framing will be left in place unless it is structurally unsound.
Care must be taken to do any more damage to the framing than is necessary to remove contaminated material.
6. Deliverables:
The period of performance for this project is 60 days from Notice to Proceed.
The contractor will have an on-site project manager to coordinate all tasks.
The COR will not instruct sub-contractors, arrange schedules or supervise any work.
All material and equipment are to be provided by the contractor. No tools, equipment or manpower will be provided by the facility. Materials will not be unloaded or moved by facility staff. Equipment may be stored in designated areas during the period of performance, but no inside storage will be provided, and the facility assumes no liability against loss or theft.
The work will be staged to allow access to the facility throughout the period of performance. Work will be considered complete debris has been removed from the site, all adjacent areas have been returned to an orderly condition and all adjacent surfaces are treated as detailed in scope. Proper signage and barriers will be provided to restrict vehicle and pedestrian access to work areas and no hazardous conditions should be present in unguarded areas.
All work is to be performed between the hours of 7am and 5pm, Monday through Friday unless previous arrangements have been made with the Facility Manager. No work will be performed on Federal holidays without exceptions.
The contractor will hold a pre-construction safety meeting with their employees and a designated representative from the facility. All work will be performed in accordance with all local state and federal safety regulations. Any necessary fall arrest systems or other PPE will be provided and used by contractor’s staff members. The contractor will provide verbal progress reports to the COR or other authorized representative at regular intervals during the period of performance. Any delays or cost overruns will be communicated to the COR as soon as they are encountered. The customer reserves the right to have the project inspected for integrity and quality of workmanship by a third party.
The project will be considered completed and authorized for payment only when approved by the COR.
